The proposed installation will serve a proposed five -bedroom (5) house. This lot is currently undeveloped. Surrounding lots are served by private wells and private on-site wastewater treatment systems. Currently there are no private wells within 100' of the proposed installation. 1. Soils: A test hole was performed on this lot by DHI Consulting Engineers on 3/29/2001, the soils log is attached. Groundwater was not observed to a depth of 17' below the ground surface. Pannone Engineering will perform 2 confirmation test holes shown on the attached site plan during construction of the proposed installation to confirm soil profile, percolation rate, and water table in the area of the proposed drain fields. Based on the DHI test hole and surrounding test holes an application rate of 1.0 gpd/sf was used. 2. Soil Absorption System Design. a. See Sheet 1 of the design package. 3. Surface Water: Stuckagain Creek is located approximately —60 feet from the northern trench. Berms will be constructed as shown on the site plan and sheet 2 of the design package. These berms will create a minimum of 100' of overland flow for any overflowing effluent before contacting Stuckagain Creek. The worst-case scenario flow is shown on the site plan, effluent following this path will travel a minimum of 123' before contacting Stuckagain Creek. 4. Topography: See attached site plan for area topography. S. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that the review is complete and no further comments are received from MoA On -Site Department, the note will be removed and "For Construction" drawings will be issued.
