Waiver Request
Ladies and Gentlemen:
I am writing to request a separation distance waiver down to 98.6 feet, between the existing well on the
subject property and the new drain field on the same lot. The well radius to tank distance was staked at
the time of installation, but the final survey as -built shows the tank just inside the 100' well offset
distance.
The groundwater in the area generally slopes from east to west in the area, with the wellhead ground
surface elevation slightly higher than that at the septic system.
The worst-case scenario is the separation distance between the absorption field and the private well as
outlined under 18 AAC 80.020 and 18 AAC 72.021(a), I have calculated the following points:

Septic System to Steep Slope
Waiver Request
Ladies and Gentlemen:
I am writing to request a separation distance waiver between the proposed septic system and a slope of
greater than 25% to eight feet (8').
The soils in this area are typically Hydrologic Soils Group A which contains organics underlain by silty
sand and silty gravel (sm/gm), with a well graded to poorly graded gravels below, as shown on the soils
log.
Due to the nature of and drainage capacity of the soils found in the area (HSG A) the designed
infiltration line of 45% for septic system installation on steep slopes should be used instead of the
assumed/worst case soil classification infiltration line of 25%. The slopes and soil types enable
installation of any system without the system effluent "daylighting" at any distance of concern.
Granting this waiver will not pose a threat to the safety of this lot or the surrounding lots.

Septic System Permit Request
Design Narrative
This is a design narrative for a permit to install a septic system to be issued for this property. The proposed systems
will serve a newly constructed three-bedroom (3) house. Currently the lot is undeveloped. This lot and few
surrounding lots are served by private wells. Currently there is no septic systems within 100 feet of the proposed
system.
J. Soils. Located central on the subject lot is a pre-existing test hole at an approximate elevation of 1645'. At this
test hole, ground water was not observed to a depth of 13.0' and bedrock was not encountered. Based on the -
results of this test holes percolation test; an application rate of 1.2 gallons/day/square foot was used for
conventional wastewater system placed west of the existing test hole. Additionally per AMC 15.65.210c all
surrounding lots on this block (uphill of Birdsong Drive) have had test holes completed. The surrounding soils
logs are attached. The test holes were all dry to the bottom, depths being 12'-17', and percolation rates were
consistent with the subject lot, the only outlier being one hole at 20-mpi that is located over 300' away.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. The
proposed drain field upgrade will maintain at least 100 feet from all surface water.
4. Topography: See attached site plan for area topography. The lot slopes generally from west to east at
approximately 30%. All provisions of AMC 15.65.210a will be met. No slope within 100' of the system exceeds 46%.
5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that the review
is complete and no further comments are received from MoA On -Site Department, the note will be removed and
"For Construction" drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

Septic System Owner -installer Agreement
The On-site Water and Wastewater Section (On-site) may issue an approval for a homeowner
to perform work on an on-site wastewater disposal system to serve that individual's owner -
occupied, single-family or duplex home if the homeowner meets and agrees to the following
