Tank to Foundation Waiver Request
Dear On-Site Services Engineer:
During the installation of this septic tank, the contractor was unable to maintain 10’ separation to
the building foundation due to the significant side slopes on the lot. In lieu of that, they took careful
elevation shots of the bottom of the footing and placed the tank to ensure that they were out of the
soil bearing prism of the foundation. Please refer to the Foundation/Tank Profile for a visual
representation of the measurements.
We are requesting a waiver for 5.3’ separation from the tank to the foundation. If this is approved
there will be no negative effects on the home or property.

b. Covered, sealed, and heated to prevent freezing

Special Provisions:
iA test hole is required prior to construction of the absorption field, in the location shown on the site plan, to
Ej confirm minimum separation to impermeable soil and seasonal high groundwater, as well as percolation
rate. Construction may proceed at your own risk prior to 7 -day groundwater monitoring. If results require
a design change, construction shall stop pending approval of a change order. Please submit results with

Septic system design
Dear On-Site Services Engineer:
The owner of the above lot intends to build a 3-bedroom home on the property, so we are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home as well as the proposed well, septic location
and alternate site. No conflicts exist between this proposed system and any other well or septic
system, whether on this lot or adjacent lots.
The ground surface on the lot slopes upward fairly steeply towards the back of the lot, and the
septic system is located towards the front of the lot. Contours are shown on the site plan showing
the grade and direction of flow. There are slopes greater than 25% where the trenches are located
within 50 feet downslope of both the primary or alternate site, but no slopes greater than 47%.
Please refer to the Slopes Profile sheet do see that the design meets the criteria for the Steep Slopes
provision of the code. We are asking for permission to use 2:1 slopes on the sides of the mound,
as 3:1 slopes will extend past the property line on the lower trench. We propose using jute matting
to stabilize the soil until grass is established.
Stormwater drainage will not impact this septic system. The new trench will be constructed parallel
to the slope as much as possible. Wells on this and adjacent lots are shown. The new system will
be a minimum of 100’ from all wells and surface water, and more than 5’ away from the septic
tank.
Please refer to the attached test hole logs, plan and profile pages for the septic design. If this design
is followed, there will be no adverse impacts to adjacent properties.

Aquifer Test Wells
Dear Onsite Services Engineer:
Jasmine Subdivision is composed of 11 Lots and 1 Tract and is located off of Jasmine Road in
Chugiak. It is surrounded by developed lots with onsite private wells. The lots proposed for the
subdivision are in excess of 40,000 square feet and are proposed to have on site wells and septic
systems.
Three wells are proposed for placement in the subdivision on proposed Lots 3, 4 and 9. The wells
will be placed at sites which will represent standard locations relative to septic system sites and
home sites on typical lots in the subdivision. Each well will be pumped in accordance with AMC
15.55.060F1 to establish the sustained yield as determined by the well driller. Water quality testing
from each well will be accomplished in accordance with AMC 15.55.060F2. A formal report
including sustained yield results for each well will be submitted once all data is gathered and
analyzed. The well locations have been determined with the assistance of Jim Munter, the
hydrogeologist for the project.
Permit applications are attached for placement of the three wells in the subdivision. We hereby
request your approval to complete the well and water study to determine the capacity of the aquifer
to support the proposed wells for the subdivision.
The location of the wells and the proposed location of the houses and septic systems on the lots
will not impact adjacent properties
