All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing

-Special Provisions:
1. The entire subsurface disposal field and/or alternate is not within a 30 foot radius of a percolation test.
The Engineer needs to do an additional percolation test prior to the construction of the septic field. Please
submit stamped and signed results with the As -built Inspection Report. If the results require a design
change, construction of the system will stop pending On -Site review and approval.
2. The upper 4" of a mounded disposal field shall consist of topsoil and the mound shall be seeded to ensure
vegetation. AMC 15.65.210E9

Installation Information & Conditional COSA Request
The conventional on-site system installed on the subject lot consists of a Greer-1250gallon septic tank accompanied
by two mounded five (5.0’) foot wide trenches both one-hundred (100.0’) feet in length. PES was present for the
installation of the septic tank for the subject property on (02/14/2024) following 24-hour and two-hour notice
forwarded to the on-site department. PES was not notified by the contractor or present for the installation of the
drain fields on the subject property. PES performed an additional percolation test per the special provision listed on
the subject lot’s permit OSP231232 issued on (08/03/2023)—on (03/06/2024); along with final grade inspection to
collect measurements and elevations for the installation. A field was found to be installed within (100’) of the newly
installed well on the subject lot. Please see the attached waiver request for the reduced separation.
Per provision #2 on the permit for this lot, 4’’ of topsoil and the addition of seed was to occur over the installed
wastewater disposal fields per AMC 15.65.210E9. Due to winter conditions, this work was not done upon installation
and is to occur in spring of 2024. We would like to request a conditional COSA be issued for this property due to the
unmet permit provision. The cost of this work is to be under $1500.

Septic System to Well on lot
Ladies and Gentlemen:
We are writing to request a separation distance waiver between the new well and new soil absorption field.
The field was inadvertently installed (93.3’) from the well.
The subject lot generally slopes from northeast to southwest, with slopes ranging from 10-15%. The existing
well head lies at a higher elevation than the drain field so it is not likely that effluent would travel toward the
well head. The soil composition of this lot as documented by the well driller: consist of three feet (3.0’) of
overburden/organics, seventeen feet (17.0’) of sand and gravel w/brown clay, thirty one feet (31.0’) of sand a
gravel, and 349.0 feet of underlying bedrock. The static water level of this well was measured at 64.0 feet
below ground surface. Nitrate levels are elevated for this well with the attached water sample results
showing a level of 1.02 mg/L. The existing neighboring well located down gradient of the subject lot on
Canyon View Estates Block 2 Lot 1—had a measured static water level of (132.0’) below ground surface in
August of 2023 and has a nitrate reading of 2.6 mg/l. The ground water flow/gradient is to the southwest
from the well to the soil absorption field. Absorbed wastewater will infiltrate away from the well. The
existing absorption fields on the subject lot were installed the week of (02/19/24 – 02/23/24). The existing
well on the property was initiated on (09/29/23) and completed (10/03/2023). PES was not notified by the
contractor or present for the installation of the drain fields on the subject property—please see the attached
installation letter.
We used the ADEC Separation Distances Waiver Guidelines Memorandum, dated January 3, 1985--to
evaluate the acceptability of the waiver. Our evaluations are tabulated below:

The sum of the above values indicates the following condition: “Almost sure to be free from any form of
contamination from household sewage.’’ Based on the area’s topography, soil composition, the elevation of
bedrock and the measured ground water elevation; it is the opinion of this office that granting this waiver
does not pose a risk to public health or the environment.
