The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system. All systems eventually fail, and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. No sewer main, sewer connection or sewer extension may be covered or backfilled until inspected and approved by an authorized representative of the sewer utility, who shall be notified 24 HOURS in advance of when the construction or installation will be ready for inspection, exclusive of Saturday, Sunday and Holidays. 2. An Application for Service must be completed to initiate monthly billing before the water service will be turned on. 3. All commercial and industrial structures require the installation of a control manhole for monitoring and sampling purposes. 4. The developer, owner or contractor shall require all permits and pay all fees established by any governmental unit as a condition for the installation of a sewer connection. 5. Before beginning construction, the developer, owner or contractor shall deliver a copy of the State of Alaska Department of Transportation and Public Facilities permit if such a permit is required, to AWWU. All terms and conditions of the ADOT & PF utility permit are incorporated by reference herein, and the developer, owner or contractor named as permittee on the reverse side hereof shall be solely responsible for compliance with such terms and conditions until the service connection receives final acceptance by AWWU. 6. To the fullest extent permitted by law, the permittee shall indemnify, defend, and hold harmless the Municipality, AWWU and their agents and employees, from and against all claims, damages, losses, and liabilities, including violations or alleged violations of the ADOT & PF utility permit, arising or resulting in whole or in part, from the performance of work pursuant to this permit. 7. Permit issuance does not guarantee availability of sewer. It shall be the developer's, owner's, or contractor's responsibility to check elevations of existing sewer mains to insure gravity service is possible. 8. All on site sewer systems including but not limited to cesspools, septic tanks and on-site lift stations must be decommissioned in accordance with guidelines provided by the Municipal Development Services On Site or the Alaska Department of Environmental Conservation, 9. Permit fee is neither refundable nor transferable. 10. This permit expires DECEMBER 31st in the year issued. 11. A triplex or larger, or commercial property must have an engineered set of plans approved by AWWU. After construction of the water service connection, AWWU requires an approved record drawing prepared by a professional engineer be submitted to AWWU prior to acceptance of service. 12. All construction shall be in accordance with the most current edition of the Municipality of Anchorage Standard Specifications (MASS) for Streets and Utilities as currently amended and the AWWU Design and Construction Practices Manual. 13. Upon exposing an existing sanitary sewer service connection and/or extension, the contractor must ensure that the line has proper slope, bearing , and is free and clear of obstructions prior to continuing the work.
