This is a design narrative for a permit to install a private well to be issued for this property. Currently the lot is
undeveloped. This lot and the surrounding lots are to be served by public sewer. Currently there are no sewer mains
or private septic systems within 100’ of the proposed installation.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on
procedures outlined in the Certificate of On-Site Systems Approval Guidelines, indicates that the on-site water supply
and/or wastewater disposal system appears to comply with applicable Municipal and State codes, ordinances,
and regulations in effect at the time of installation, unless noted otherwise.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in
accordance with ADEC & MOA guidelines and regulations. The reported results describe the performance of the system
under the conditions encountered at the time of the test, and separation distances measured to readily identifiable
features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that
may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside
the control of the evaluator of this system. All systems eventually fail, and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore, we
cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this
report is for the sole benefit of the owner listed above.

A water sample revealed an arsenic concentration of 17.5 micrograms per liter
(ug/L). The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 ug/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. information on arsenic is available from the On -Site Water and
Wastewater Program website (www.muni.org/onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -Site
Systems Approval.

Well On lot to Existing Sanitary Sewer Manhole & Sewer Main
Ladies and Gentlemen:
We are writing to request a separation distance waiver between the new well installed on this lot and the
existing sanitary sewer main and sanitary sewer manhole serving the subject property. The well on the
subject property was not installed in the proposed location and inadvertently installed approximately (85.0’)
from an existing sanitary sewer manhole, and approximately (89.0’) from an existing sanitary sewer main in
which the well radius encapsulates approximately (10.0’) of sewer main.
The subject lot generally slopes from east to west, with slopes ranging from 10-20%. The existing well head
lies at a relatively the same elevation as the existing sanitary sewer manhole; but per subdivision contours—
the north end of the lot and existing cul-de-sac on Red Sky Circle are graded to drain off to the northwest. In
the event that the existing watertight sanitary sewer manhole were to overflow or leak; effluent is likely to
flow away from the existing well head. The soil composition of this lot as documented by the well driller:
consists of thirty-eight feet (38.0’) of brown clay with fine gravel, twenty-five feet (25.0’) of fine gravel with
sand, thirty-nine feet (39.0’) grey clay with gravel, three feet (3.0’) of underlying boulder, seventy feet (70.0’)
of grey clay with gravel, five feet (5.0’) of glacial till, twelve feet (12.0’) of grey clay with pea gravel, and five
feet (5.0’) of coarse gravel with water. The static water level of this well was measured at (99.0’) feet below
ground surface bentonite was installed around the casing to a depth of (20.0’). Nitrate levels were observed
as ‘non-detect’ and arsenic levels are elevated for this well with the attached water sample results depicting
a level of 17.5 mg/L. The existing neighboring well located down gradient of the subject lot on Sky Ridge Lot
8—had a measured static water level of (84.7’) below ground surface in February of 2023 and has a nitrate
reading also depicted as ‘non-detect’. The ground water flow/gradient appears to be slightly down-gradient
to the northwest from the well to the existing sanitary sewer manhole. Absorbed wastewater will infiltrate
away from the well. The existing well on the subject lot was initiated on (03/27/2024) and completed on
(03/30/2024) –without the notification to PES of the well location change from the approved permitted
location. PES was not notified by the contractor or present for the installation of the well on the subject
property.
We used the ADEC Separation Distances Waiver Guidelines Memorandum, dated January 3, 1985--to
evaluate the acceptability of this waiver. Our evaluations are tabulated below:

waiver does not pose a risk to public health or the environment.
