Well drilled to 445'. Production rate checked by bucket off rig showed a production rate
of approximately .30 gpm. Well was fracked, then pumped to remove ir►jected water.
After injected water was removed, the well recovery rate was tested at .42 gpm. At
hydrologist recommendation well was fracked again. After injected water was removed
the well tested at 1.1 gpm for the 24 hour subdivision flow test. Work completed on
611612020.
Lot 1
Well drilled to 305'. Production rate checked by bucket off rig showed a production rate
of approximately .30 gpm. Well was fracked, then pumped to remove injected water.
After injected water was removed, the well recovery rate was tested at 1.14 gpm. Work
completed on 3/13/202.

2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7). -
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing

Jasmine Subdivision is composed of 1.1 Lots and 1 Tract and is located off of Jasmine Road in
Chugiak. It is surrounded by developed lots with onsite private wells. The lots proposed for the
subdivision are in excess of 40,000 square feet and are proposed to have on site wells and septic
systems.
Three wells are proposedf orplacementin the subdivisionon proposed Lots 1, 2 and 11. The wells
will be placed at sites which will represent standard locations relative to septic system sites and
home sites on typical lots in the subdivision. Each well will be pumped in accordance with AMC
15.55.060171 to establishthe sustain edy iel das d eterminedby the well driller. Waterqualitytesting
from each well will be accomplished in accordance with AMC 15.55.060172. A formal report
including sustained yield results for each well will be submitted once all data is gathered and
analyzed. The well locations have been detennined with the assistance of Jim Munter, the
hydrogeologist for the project.
Permit applications are attached for placement of the three wells in the subdivision. We hereby
requesty ourapproval to complete the well and waterstudyto determine the capacityof the aquifer
to support the proposed wells for the subdivision.
The location of the wells and the proposed location of the houses and septic systems on the lots
will not impact adjacent properties
