4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
• A sieve analysis is required in the location of the proposed absorption fields. If results require a design
change, construction shall stop pending approval of a change order. Please submit results with the
inspection report (or change order, if required).

Well and Septic System Permit Request
This is a design narrative for a permit to install an initial septic system and well on the subject property. The
proposed system will serve a proposed three‐bedroom (3) house. This lot and surrounding lots are served by private
wells.
1. Soils. 28 test holes were performed in this subdivision by PES for the approval of the subdivision, and more were
performed during the design of septic systems on lots in the subdivision. The shallowest groundwater observed
in the subdivision was at 13’ on lot 26, directly adjacent to a stream located on the far end of the subdivision.
Bedrock was not encountered anywhere in the subdivision. The test holes in the area were all found to have a
percolation of less than 5 MPI, most less than 1 MPI. See soils logs for subdivision, S12577. The test hole on lot
18 was performed further to the northwest than the system will actually be installed. We are using surrounding
test holes as a justification. Based on the results of the surrounding percolation tests and known soil types in the
area, per 15.65.210 C; a conservative application rate of 1.2 gallons/day/square feet was used for a conventional
wastewater system. A sieve analysis will be performed on the soil in the area of the drain field to verify the in‐
situ sand, and a change order will be submitted if the soil is found to require a sand filter.

4. Surface Water:
There is no surface water within 100 feet of the proposed septic tank and drain field.
5. Topography:
See attached site plan for area topography. There are no slopes exceeding 25% less than 50ft from the proposed
drain field.
6. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the
review is complete and no further comments are received from MoA On‐Site Department, the note will be removed
and “For Construction” drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on
procedures outlined in the Certificate of On-Site Systems Approval Guidelines, indicates that the on-site water supply
and/or wastewater disposal system appears to comply with applicable Municipal and State codes, ordinances,
and regulations in effect at the time of installation, unless noted otherwise.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in
accordance with ADEC & MOA guidelines and regulations. The reported results describe the performance of the system
under the conditions encountered at the time of the test, and separation distances measured to readily identifiable
features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that
may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside
the control of the evaluator of this system. All systems eventually fail, and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore, we
cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this
report is for the sole benefit of the owner listed above.
