easement along the north boundary of the following property; .

This non -objection is granted with the full understanding and agreement by all parties
having an interest in the above property that this septic system encroachment will in no
way restrict or limit the current or future ability of MFA to fully enjoy the benefits of the
easement for any and all utility purposes that it presently enjoys ender the easement. '
This -non-objection is further conditioned by the stipulation that MEA will be held
harmless from liability for any and all damages to the encroachment that may resLilt

This letter of non -objection in no way precludes GCI Cable from full use and enjoyment
of any. rights it may have within any portion of the utility easement and or the right-of-
way, including unlimited access for servicing its facilities. Also any additional and
extraordinary costs incurred during any future required construction, repair or
reconstruction of GCI's facilities to accommodate any or all of the encroachments shall
be paid by the property owner.

Acceptance and use of this letter of non -objection by yourself, your heirs, your
assigns, or your successors, will constitute agreement to the following stipulations:
ENSTAR will be held harmless, now and forever for any damages or injury to
any person or property as a result of this encroachment.
Any ENSTAR facility damaged or destroyed, as a result of this encroachment
-will be repaired at no cost to ENSTAR.
Any costs incurred by ENSTAR for special construction necessitated by this
encroachment will be borne by the property owner.
All applicable safety code regulations will be observed and maintained.
This letter of nonobjection will in no way preclude ENSTAR from full use and
enjoyment of its rights within any portion of its right-of-way.

To whom it may concern:
A revision to septic upgrade permit (permit# SW020238) was re -issued on October 3, 2002, for a
conventional type septic system. During the excavation of the confirmation test hole, the
monitoring tube for Steve Pannone's test hole was checked and found the groundwater to be at 8
feet. The confirmation percolation test found the rate to be 40 minutes/inch. Based upon this
information, a conventional type septic system will not fit in the area available. We are
proposing that a 1250 gallon steel septic tank and a 550 gallon "Fat Albert" pump vault with a
dosing timer and a bottomless intermittent sand filter (ISF) be installed. Comments regarding the
redesign of the septic system are as follows:
DRAINFIELD DESIGN: Bottomless Intermittent Sand Filter (ISF)
a. Percolation Rate: 20 & 40 minutes/inch
b. Allowable Application Rate for ISF: 2.0 gallons/day/R2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 300 ftZ
f. Effective Depth below pressure pipes: 2.5+ inches
g. Width: 12 feet
h. Length: 30 feet.
i. Effective absorption area = 360 ftZ
j. Air Supply: Thomas Industries, Model 5070, "Anchorage Tank".
k. Air Supply Line: "Wasteflow" emitterline, 1/2 inch I.D, "Anchorage Tank".
1. Sand Material: In accordance with M.O.A. latest standards
in. Pea Gravel: 100% passing 3/8" sieve, less than 20% passing the 1/4" sieve, and
less than 1% passing the #8 sieve.

We are proposing to excavate down to a depth of 4.0 feet (maximum), place a minimum of 6
inches of sand, install the air supply line, and cover it with 1.5 feet of sand. On top of the sand,
we will place 6 inches of 3/8 inch pea gravel, with the pressure laterals midway in the layer. We
will use a conventional S.T.E.P. (Anchorage Tank), equipped with a programmable timer so that
Dow can be intermittently dosed to the ISF.
SURFACE NVATERS: There are no surface waters within 100 feet of the proposed design.
TOPOGRAPHY: The area for the proposed septic upgrade is a 1 to 5 percent slope running
from south to north; in short, there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

To whom it may concern:
A septic upgrade permit (permit# SW020238) was issued on July 22, 2002, for an Advantex
treatment system. The Advantex design was prepared by Steve Pannone, P.E. The homeowner
contacted AKWWC, INC. requesting that we design a conventional septic system for the
property. Per the owner, the house is currently a three bedroom residence; but would like to
install a 4 bedroom septic system. The existing drainfields are sized for a 4 bedroom residence so
an alternate site will not needed to be provided. Comments regarding the re -design of the septic
system are as follows:
1. SOILS: A test hole was recently excavated on the property by Steve R. Pannone, P.E.
According to the percolation data, the rate was 24 minutes/inch, which equates to a 0.6
application rate. Also, a test hole was excavated on the property by Robert A. Shafer, P.E. in
1983. According to the soils log, the percolation rate was 60 minutes/inch, which equates to a
0.45 application rate. We are proposing that a 0.5 gallons/day/112 be used and that a confirmation
test hole with a percolation test/s be performed prior to the installation to confirm this application
rate. The monitoring tube for Pannone's test hole has been checked and found water to be at 11
feet below the ground surface.

Gentlemen:
My firm was contacted to a Health Authority Approval of the septic system serving this lot. The
existing trench was inundated with liquid when I arrived. The system is in failure. A single test
hole was excavated on June 14, 2002 on the lot. The soils report and a percolation test result is
attached. Ground water was monitored for seven days. No bedrock or ground water was
encountered in the test hole. however, past experience has shown that ground water is present
in this subdivision at a depth of approximately seven feet. This will be used for the ground water
design depth. An Advantex AX20 Septic System will be installed as the upgrade system.
The lot is approximately 9,200 square feet in size. Lot 1 slopes to the northwest at
approximately 1-2 percent in the area of the test holes. The ground around the test hole slopes
at approximately 10 percent south and west of the test hole. The existing septic tank will be
abandoned. The proposed installation will be located on the northern portion of the lot. The lot is
served by a community well (AWWU). The proposed location is greater than 200 feet away from
existing & neighboring wells and 25 feet from the water service lines. The surrounding wells are
located greater than 200 feet from the proposed installation. The proposed installation will not
affect the future development of the surrounding or existing lots. See the attached design.

RECERCULATING SEPTIC TANK
AND TRICKLE FILTER SPECIFICATIONS
Lot 1, Block 1 Spring Brook Vista Subdivision
1. GENERAL STATEMENT OF WORK
1.1 The work and plans covered by these specifications consist of furnishing all labor,
materials, tools, transportation, and equipment necessary to complete an
recalculating septic tank with trickle filter system as described herein and on the
attached Contract Drawing. All work under this project shall comply with current
Municipality of Anchorage (MOA) Regulations and shall conform to the following:
• Four (4) Bedroom Private House
1.2 The Contractor shall carefully examine the project documents, have full knowledge
thereof, have investigated the site and be satisfied with all conditions affecting the
work. The Contractor shall notify the Engineer IMEDIATELY if conditions are
encountered during the site investigations that affect the work. Any failure of the
Contractor to become acquainted with the available information will not relieve the
Contractor from the responsibility of performing the work as stated in these
specifications.
1.3 The Contractor shall call for locates of utilities. Damage to utilities by the
Contractor if locates are not called for is the responsibility of the Contractor.
2. MATERIAL
2_1 Tank: Septic: Tank shall be a two (2) compartment tank of at least the minimum
size indicated on the drawings and shall be MOA approved. The compartments
shall have a separate 4" clean-out riser.
2_2 Manhole Riser or Lift Station Tank: An additional second compartment manhole
riser shall be capable of being equipped with the following:
a. A junction box (Nems 4x) or equal, bonded or attached to the pump basin.
b. UL listed electrical cord grips, installed in the J -box.
C. An additional tank (MOA Approved) may be provided for the effluent
pumping assembly and the items above.
2_3 Effluent Pumping Assembly, Controls, Alarm & Piping:
a. Effluent pumping assembly shall be of approved materials, including: A 110
volt or equal UL listed effluent pump; High-density PVC cylinder housing the
pump, with level controls and screen, serving as a baffle to prevent the screen
from clogging; All piping shall be PVC or other non corroding material approved
by the MOA.

b. Pump controls and alarm system shall be an approved system with the
following options:
- Event Counter
- Elapsed Time Meter
- Program Timer
- 10 Watt Heater
- Remote alarm panel option with a minimum of 80 dB sound pressure at 24
inches, operating temperature 30 AC to 65 AC, and continuous sound to be
located in the house.
- Oil tight visual alarm with push -to -silence feature. Automatic audio alarm
reset.
- 15 amp motor rated toggle switch, double pole, double throw with three
positions; Manual (man), Automatic (auto), and off.
- Nema 4x rated fiberglass enclosure with hinged cover, or equal.
- All controls to be installed per manufactures instructions.
- Control panel to be installed on outside wall of home within sight of
septic/lift station.
c. Piping from septic tank to the trickle filter is to be 1 '/4" PVC schedule 40
solvent weld piping properly bedded and sloped to drain back to the lift station.
Insulation board shall be placed over the line where the cover is less than four
feet.
d. The Air Compressor shall be a Thomas Linear Series model No. 5060-A or
507OV-A or MOA Approved equal.
2.4 Trickle Filter
a. Trickle Filter Pod shall be AX20 manufactured by Orenco Systems Inc. and
distributed by Anchorage Tank & Welding.
b. Install all materials according to manufacturers instructions and in accordance
with MOA regulations.
2.5 Soil Absorption System
a) The drain field shall be constructed as shown on the drawings and in accordance
with all applicable regulations described in Municipality of Anchorage AMC Title
15.65, "Wastewater Disposal Regulations" (MOA).
b) Gravel to be used in the soil absorption system shall be well graded rocks and
cobbles with size ranging from '/Z" to 2 '/z" and approved for use in soil absorption
systems by MOA.
c) Perforated pipes shall be F810 LDPE or PVC 3034pipe, with two rows of
perforations and shall be approved for use in soil absorption systems by MOA.
d) Clean-out pipes and monitor tubes shall be 3034 PVC solid pipe.
e) Clean-out caps shall be rubber "Jim Caps" securable with adjustable metal bands.
Friction fit caps will not be accepted.

PART 3 EXECUTION
3.1 LOCATION AND GRADE
a) The septic tank shall be placed at the locations and elevations as shown on the
Drawings.
b) The drain field shall be placed in the location shown on the Drawing, with minor
adjustments allowed to minimize excavation and accommodate the existing terrain.
The drain field piping shall remain level over the length of the drain field.
c) Dewatering may be required for all excavations that penetrate the groundwater
surface. Dewatering costs shall be incidental to the excavation. If ground water is
encountered during the excavation for the septic system, notify the engineer
immediately.
3.2 EXCAVATION
a) General: The Contractor shall accomplish all excavation required by the drawings
through whatever substances encountered, to the depth as shown on the plans. All
excavation shall be accomplished in accordance with the State of Alaska safety code
and other applicable safety requirements.
b) Water Removal: Water is expected in the limits of excavation as evidenced by the soil
test pit information. If an apparent water table is observed in any of the excavations
less than 5 feet below existing grade, notify the engineer immediately.
3.3 INSTALLATION
A. Septic Tank
1. After excavation the tank shall be placed on undisturbed soil on a level grade
at a depth as indicated on Drawings. Backfill around the tank with
structural fill. Fill shall be installed in 12 inch lifts, compacted to 95%
maximum density. Any damage to the protective coating during installation
shall be repaired as recommended by the manufacturer.
2. The tank inlet shall be set at the elevation shown on the drawings. Care
shall be taken during installation to ensure the proper elevation drop
between the inlet and the outlet. Install flexible "furnco" rubber fittings at
connection from tank to inlet and outlet pipes as indicated on Drawings.
B. Joining Pipe Sections
1. Pipes shall be joined to one another, to the polyethylene fittings,
cleanouts, and electrofusion couplings.

2. Cleanouts:
(1) All earthwork and fill necessary to install the cleanouts shall be
considered incidental.
3. Jointing:
a. Fit the heat channel in place, ensuring that ends are deburred and
square to allow easy passage of electrocal or glycol heat trace.
b. Fit the insulation half -shells snugly in place. Band with strapping to
hold tightly in place.
c. Heat Shrink Coupling: Cover entire exposed joint plus an overlap of four
inches on pipe coated on either end. Heat in accordance with
manufacturer's instructions.
4. Installation:
a. Install insulation half -shells and heat -shrink wrap at pipe connections.
Maximum gap in half -shells shall be '/4 inch.
b. Lower assembled insulated pipe carefully into trench. Do not allow
joints to deflect greater than the elastic capacity of the heat shrink
sleeves. If sleeves are found to be damaged, reduce the deflection and
repair the damages sleeves.
c. Tolerances:
(1) Lay mains to indicated line and grade. Perform earthwork in
accordance with the Drawings.
(2) Minor Changes in Direction: Pipe deflection will be allowed for
small changes in direction, provided that the pipe bending radius
is less than 30 times the diameter of the outer jacket of the
insulated pipe system.
C. Drain Field
a) The drain field area shall be excavated to the elevation and dimensions as shown
on Drawing. Remove all organic and silt material to the top of the well -graded
sand layer. After excavation, the open bed shall be protected from surface runoff
to prevent the entrance of silt and debris. Under no circumstances shall the use
of heavy equipment be permitted on the drain field surface. All smeared or
compacted surfaces shall be raked to a depth of 1 inch, and loose material
removed, before any of the drain rock is placed.
b) After all organic and silt material is removed, sand bedding shall be placed to an
elevation shown on the drawings. Sand shall not be compacted.
c) The bottom of the bed should be as level as practical.
d) The gravel -less system and distribution piping shall be installed as shown on the
drawings.
e) The drain field shall then be backfilled with sand bedding to a depth of 1.0 above
gravel -less system. Filter fabric and insulation shall be placed over the sand bedding.

Backfill above insulation with material excavated from the building -pad to the
elevation shown. The material shall be mounded over the drain field as shown on
Drawings. Backfill of drain field shall not be compacted.
D. Close -Out
A. Contractor shall submit unqualified as -built drawings and certificates on MOA
approved forms to the Engineer, and MOA for approval to Operate.

Reference: Septic Adequacy Test Results and Drainfield to Foundation Separation Distance for
Spring Brook Vista S/D; Block 1, Lot 1 —12336 E. Prince of Peace Drive, Eagle River, AK 99577
Mr. Eklund:
We have recently performed a presoak and septic adequacy test on the drainfields for the referenced
property. The septic system consists of a 1250 gallon steel septic tank that was installed on 10/15/2002,
two (2) deep trenches that were installed in 1976, and another deep trench that was installed in 1983.
Based upon the Nome Owner Questionnaire (see attached), the house has been vacant since
11/19/2018. Given the house has been vacant almost 6 months, a presoak was required. On 3/28/2019,
we introduced 746 gallons to the 1976 south trench, 756 gallons to the 1976 north trench, and 706
gallons to the 1983 trench. In short, a total of 2,208 gallons was added to the drainfields. The following
day we returned to the property and performed the septic adequacy test. Per the inspection report 1976
inspection report, these drainfields have 5 feet of effective. Both the 1976 and 1983 trenches have 2"
steel (perforated) monitoring tubes that were driven near the ends of the drainfields into the drainrock in
the past. The 1983 trench also has a sump at the southwest end of the trench. Based upon our field
measurements, the monitoring tubes do not extend to the bottom of the drainfield, the following is our
findings:
• In the 1976 south trench, we measured 2.4' difference from the invert of the cleanout at the end
of the drainfield to the bottom of the 2" monitoring tube. We also found the invert of the
distribution line to be 6.4' below grade.
• In the 1976 north trench, we measured 2.3' difference from the invert of the cleanout at the end of
the drainfield to the bottom of the 2" monitoring tube. We also found the invert of the distribution
line to be 6.1' below grade.
• In the 1983 south trench, we measured 3.5' difference from the invert of the cleanouf at the end
of the drainfield to the bottom of the 4" sump and 2.2' of difference from the invert of the cleanout
at the end of the drainfield to the bottom of the 2" monitoring tube. The invert of the sump is 5.5'
below grade and the invert of the cleanout is 5.3' below grade. We would like to note that the
elevation difference between the distribution line invert of the sump and the cleanout is
approximately 1.7 feet, with the cleanout being at a lower elevation.

On 3/29/2019 (the day after the presoak), we returned to the property to perform the septic adequacy
test. The following is a summary of the testing procedure and our calculations of the absorption that
should take place in 24-hour period:
1976 — South Trench
Prior to adding water, there was 23.5 inches of liquid level in the monitoring tube for this trench. Over a
period of 60 minutes, 450 gallons were introduced to the cleanout at the end of this trench, which caused
the liquid level to rise to 30.5 inches (a rise of 7"). This brought the liquid level to approximately 2 inches
of water in the cleanout. We also checked the liquid level in the monitoring tubes for the other trenches
and there was no change in their liquid level. Water was stopped from being introduced and the trench
was allowed to recover. The monitoring tube was checked 1197 minutes later and the liquid level was 26
inches deep (2.25" below the invert). Based upon this data the following is the calculated absorption rate:
7" rise with 450 gallons = 64.29 gallons/inch. 4.5" drop X 64.29 gallon/inch = 289 gallons of
absorption in 1197 minutes. 289 gallons / 1197 minutes = 0.24 gpm X 1440 minutes in a day =
348 gpd (calculated absorption rate)
1976 — North Trench
Prior to adding water, there was 19.75 inches of liquid level in the monitoring tube for this trench. Over a
period of 67 minutes, 509 gallons were introduced to the cleanout at the end of this trench, which caused
the liquid level to rise to 27.75 inches (a rise of 8"). This brought the liquid level to approximately 0.5
inches in the cleanout. We also checked the liquid level in the monitoring tubes for the other trenches and
found there was no change in their liquid level in the 1976 south trench; but did find that the liquid level in
the 1983 sump and monitoring tube for the 1983 trench had a 1" rise. Water was stopped from being
introduced and the trench was allowed to recover. The monitoring tube was checked 1130 minutes later
and the liquid level was 26.25 inches (1.5" below the invert). Based upon this data the following is the
calculated absorption rate:
8" rise with 509 gallons = 63.63 gallons/inch. 1.5' drop X 63.63 gallon/inch = 95.45 gallons of
absorption in 1130 minutes. 95.45 gallons / 1130 minutes = 0.084 gpm X 1440 minutes in a day
= 122 gpd (calculated absorption rate)
1983 Trench
Prior to adding water directly to this trench, there was 12 inches of liquid in the sump and 15.75 inches of
liquid in the monitoring tube for this trench. Over a period of 90 minutes, 695 gallons were introduced into
the cleanout and sump at the end of this trench, which caused the liquid level to rise to 33 inches in the
sump and 36.75 inches in the monitoring tube (a rise of 21 inches in both pipes). This brought the liquid
level to approximately 9.25 inches below the invert of the sump; however, it did bring the liquid level in the
cleanout to approximately 9 inches over the invert of cleanout. We would like to note that the liquid level
dropped rapidly in the cleanout upon stopping the introduction of water. We also checked the liquid level
in the monitoring tubes for the other trenches and there was no change in their liquid level due to the
adding of this water. Water was stopped from being introduced and the trench was allowed to recover.
The sump and monitoring tube were checked 290 minutes later and there was 26.25 inches of liquid in
the sump (16" below the invert of the sump) and 24.75 inches in the monitoring tube (4 above the invert in
the cleanout). We also performed a 2"d recovery reading 1,100 minutes after water had stopped being
introduced and found the liquid level to be at 20.5 inches in the sump (21.75' below the invert of the
sump) and 24.75 inches in the monitoring tube (1" below the invert in the cleanout). We are assuming that
the sump for this drainfield is the most accurate method of monitoring the liquid level in this trench. Based
upon this data the following is the calculated absorption rate:

• Using 290 minute recovery reading — 1st recovery reading: 21" rise with 695 gallons = 33.1
gallons/inch. 6.75" drop X 33.1 gallon/inch = 223.43 gallons of absorption in 290 minutes. 223.43
gallons / 290 minutes = 0.77 gpm X 1440 minutes in a day = 1,109 gpd (calculated absorption
rate)
Using 1,100 minute recovery reading — 2nd recovery reading: 21" rise with 695 gallons = 33.1
gallons/inch. 12.5" drop X 33.1 gallon/inch = 413.75 gallons of absorption in 1,100 minutes.
413.75 gallons / 1,100 minutes = 0.376 gpm X 1440 minutes in a day = 541.6 gpd (calculated
absorption rate)
This concludes our absorption calculations for all three (3) trenches. Based upon these calculations, it
appears the combined absorption rate for all three (3) trench is over 600 gpd at the time of our test. We
would like to note that the 1976 trenches appear to be operating in the upper 90% of the absorption area.
Given the long-term vacancy of the property, the absorption rate observed may be artificially high. Also, it
is important to note that the 1983 trench was deemed failed in 2013. Given these facts, and that fact that
the drainfields are over 36 years old, it is reasonable to believe that the drainfields are approaching the
end of their useful life.
_Drainfield to Foundation Waiver Request
Upon review of the records for this property, we have found that a 12 -foot wide garage addition to this
house was added to the northeast end of the house. This was concluded by comparing a 2002 as -built
survey that performed by James Karl Dowling, RPLS which shows the house dimensions and measured
distance from the house to the north lot line; and a 2005 as -built survey (recertified survey in 2009)
performed by Fred Walatka, RPLS. It appears this addition took place between 2005 and 2009. In 2010
and 2013, another engineer submitted 2 separate COSA packages stating the drainfields were 10 feet
from the foundation and your department approved these COSA's. Based upon our findings, it appears
the drainfield encroaches upon the new portion of this foundation and may even possibly run underneath
the foundation. The garage foundation appears to be a slab on grade, but there is a possibility the
foundation has 4 foot deep footers. Based upon elevation shots we took, the top of the distribution line is
5.5 to 6 foot below grade. In short, the drainfield should be lower in elevation than the foundation footer
and should not affect the foundation. We cannot attest to the condition of the drainfield and distribution
pipe near or under the foundation. If required, the distribution line could be cameraed and located to
determine its condition and its relation to the foundation. We are requesting a 0 foot waiver for the
drainfield to the foundation.

Mr. Eklund:
We have recently performed a presoak and septic adequacy test on the drainfields for the referenced
property. The septic system consists of a 1250 gallon steel septic tank that was installed on 10/15/2002,
two (2) deep trenches that were installed in 1976, and another deep trench that was installed in 1983.
Based upon the Home Owner Questionnaire (see attached), the house has been vacant since
11/19/2018. Given the house has been vacant almost 6 months, a presoak was required. On 3/28/2019,
we introduced 746 gallons to the 1976 south trench, 756 gallons to the 1976 north trench, and 706
gallons to the 1983 trench. In short, a total of 2,208 gallons was added to the drainfields. The following
day we returned to the property and performed the septic adequacy test. Per the inspection report 1976
inspection report, these drainfields have 5 feet of effective. Both the 1976 and 1983 trenches have 2"
steel (perforated) monitoring tubes that were driven near the ends of the drainfields into the drainrock in
the past. The 1983 trench also has a sump at the southwest end of the trench. Based upon our field
measurements, the monitoring tubes do not extend to the bottom of the drainfield, the following is our
findings:
• In the 1976 south trench, we measured 2.4' difference from the invert of the cleanout at the end
of the drainfield to the bottom of the 2" monitoring tube. We also found the invert of the
distribution line to be 6.4' below grade.
• In the 1976 north trench, we measured 2.3' difference from the invert of the cleanout at the end of
the drainfield to the bottom of the 2" monitoring tube. We also found the invert of the distribution
line to be 6.1' below grade.
• In the 1983 south trench, we measured 3.5' difference from the invert of the cleanout at the end
of the drainfield to the bottom of the 4" sump and 2.2' of difference from the invert of the cleanout
at the end of the drainfield to the bottom of the 2" monitoring tube. The invert of the sump is 5.5'
below grade and the invert of the cleanout is 5.3' below grade. We would like to note that the
elevation difference between the distribution line invert of the sump and the cleanout is
approximately 1.7 feet, with the cleanout being at a lower elevation.

On 3/29/2019 (the day after the presoak), we returned to the property to perform the septic adequacy
test. The following is a summary of the testing procedure and our calculations of the absorption that
should take place in 24-hour period:
1976—South Trench
Prior to adding water, there was 23.5 inches of liquid level in the monitoring tube for this trench. Over a
period of 60 minutes, 450 gallons were introduced to the cleanout at the end of this trench, which caused
the liquid level to rise to 30.5 inches (a rise of 7"). This brought the liquid level to approximately 2 inches
of water in the cleanout. We also checked the liquid level in the monitoring tubes for the other trenches
and there was no change in their liquid level. Water was stopped from being introduced and the trench
was allowed to recover. The monitoring tube was checked 1197 minutes later and the liquid level was 26
inches deep (2.25" below the invert). Based upon this data the following is the calculated absorption rate:
• 7" rise with 450 gallons = 64.29 gallons/inch. 4.5" drop X 64.29 gallon/inch = 289 gallons of
absorption in 1197 minutes. 289 gallons / 1197 minutes = 0.24 gpm X 1440 minutes in a day =
348 gpd (calculated absorption rate)
1976— North Trench
Prior to adding water, there was 19.75 inches of liquid level in the monitoring tube for this trench. Over a
period of 67 minutes, 509 gallons were introduced to the cleanout at the end of this trench, which caused
the liquid level to rise to 27.75 inches (a rise of 8"). This brought the liquid level to approximately 0.5
inches in the cleanout. We also checked the liquid level in the monitoring tubes for the other trenches and
found there was no change in their liquid level in the 1976 south trench; but did find that the liquid level in
the 1983 sump and monitoring tube for the 1983 trench had a 1" rise. Water was stopped from being
introduced and the trench was allowed to recover. The monitoring tube was checked 1130 minutes later
and the liquid level was 26.25 inches (1.5" below the invert). Based upon this data the following is the
calculated absorption rate:
• 8" rise with 509 gallons = 63.63 gallons/inch. 1.5" drop X 63.63 gallon/inch = 95.45 gallons of
absorption in 1130 minutes. 95.45 gallons / 1130 minutes = 0.084 gpm X 1440 minutes in a day
= 122 gpd (calculated absorption rate)
1983 Trench
Prior to adding water directly to this trench, there was 12 inches of liquid in the sump and 15.75 inches of
liquid in the monitoring tube for this trench. Over a period of 90 minutes, 695 gallons were introduced into
the cleanout and sump at the end of this trench, which caused the liquid level to rise to 33 inches in the
sump and 36.75 inches in the monitoring tube (a rise of 21 inches in both pipes). This brought the liquid
level to approximately 9.25 inches below the invert of the sump; however, it did bring the liquid level in the
cleanout to approximately 9 inches over the invert of cleanout. We would like to note that the liquid level
dropped rapidly in the cleanout upon stopping the introduction of water. We also checked the liquid level
in the monitoring tubes for the other trenches and there was no change in their liquid level due to the
adding of this water. Water was stopped from being introduced and the trench was allowed to recover.
The sump and monitoring tube were checked 290 minutes later and there was 26.25 inches of liquid in
the sump (16" below the invert of the sump) and 24.75 inches in the monitoring tube (4 above the invert in
the cleanout). We also performed a 2nd recovery reading 1,100 minutes after water had stopped being
introduced and found the liquid level to be at 20.5 inches in the sump (21.75" below the invert of the
sump) and 24.75 inches in the monitoring tube (1" below the invert in the cleanout).We are assuming that
the sump for this drainfield is the most accurate method of monitoring the liquid level in this trench. Based
upon this data the following is the calculated absorption rate:

• Using 290 minute recovery reading — 1st recovery reading: 21" rise with 695 gallons = 33.1

gallons / 290 minutes = 0.77 gpm X 1440 minutes in a day = 1,109 gpd (calculated absorption
rate)
• Using 1,100 minute recovery reading — 2nd recovery reading: 21" rise with 695 gallons = 33.1
gallons/inch. 12.5" drop X 33.1 gallon/inch = 413.75 gallons of absorption in 1,100 minutes.
413.75 gallons / 1,100 minutes = 0.376 gpm X 1440 minutes in a day = 541.6 gpd (calculated
absorption rate)
This concludes our absorption calculations for all three (3) trenches. Based upon these calculations, it

Adequacy Test and Health Authority Approval
Dear Mr. Cross:
Eagle River Engineering Services (ERES) was contracted by Jerry Lewis to perform an adequacy
test and secure a Health Authority Approval (HAA), for the above referenced property. During
the records search portion of the process, it was noticed that several other engineers had done
work for the client over the past year and a half.
ERES arrived on site, and after a brief site survey, verified that there were 2 separate existing
permitted leachfields, each with new cleanouts and monitor tubes that were installed within the
last year. Additionally, a new larger septic tank had been installed just a year prior to our
involvement with this client.
The oldest existing leachfield was the original leachfield. This system was designed, built, and
approved for 3 bedrooms in 1976. This system consisted of a 1,000 gallon septic tank with 2
leachfield lines, 56 feet long, 4 feet wide, and 6 feet of sewer rock in each line. Monitor tubes
and cleanouts were added in 2002.
The second leachfield was added in 1983, and was designed and permitted for 1 additional
bedroom. The leachfield is 60' long, 3 feet wide, and 4 feet deep. A monitor tube and cleanout
were added in 2002.
The combined existing leachfields are designed and permitted for a total of 4 bedrooms. The
newly added 1250 gallon septic tank is also of adequate size for a 4 bedroom house. The
leachfield was therefore tested with a minimum of 600 gallons to determine if its absorption
capacity is sufficient for a 4 -bedroom house.
ERES decided that the water should be applied at one of the after tank cleanouts, to allow it to
flow to each separate leachfield as it would under normal usage. The appropriate amount of
water was added for a 4 bedroom single family residence. Water levels were measured to rise in
each leachfield, and then recover to at or below the water level readings prior to the addition of
the test water. The leachfields appeared to be functioning adequately, and capable of absorbing

the required amount of liquid. We can only speculate on why the system was previously failed
by another engineer.
ERES is submitting as -built documentation for the new 1250 gallon septic tank as part of this
HAA application. Additionally, a current survey as -built showing the locations of the newly
installed monitor tubes and cleanouts is also being provided.

A sewer system adequacy test was performed on the system located
on the referenced property as you requested. The septic tank was
"pumped and verified to have a capacity of _1000 gL-dons. The
absorption trench was tested by a continuous flow of water over
a period of 48 hours. It was noted during this time that the
water backed into the septic tank in the first 24 hour period.
T e flow was tYien reduced and during the second 24—hour period
the system appears to accept approximately 300 gallons of water.
It can be concluded from this test that thought the septic tank
is adequate for three bedrooms the absorption trench appears
to only be adpn>>?te for approximately two bedrooms. It will be
necessary for you to navethe sys em u it can
be considered adequate for the three bedroom residence located
on this property.
