Drain Field to Drain Field Separation Waiver Request
Ladies and Gentlemen:
We hereby submit a waiver request for the subject property. We are requesting a waiver to a distance of 10.0
feet between the drain fields installed on the subject property. The drain fields were laid out with the
appropriate separation distance prior to construction. During construction the eastern drain field drifted
toward the western drain field. This separation distance could not be verified during construction due to the
location of temporary soil storage piles, drain rock, and other construction equipment.
AMC 15.65 requires a separation of 6 feet or two times the effective depth which ever is greater between
adjacent drain fields. The subject drain fields were installed 10 feet apart, 4 feet closer than the 14 feet
required by 15.65.
The soils in this area are typically Hydrologic Soils Group A which contains organics underlain by silty sand
and gravel. The soil composition and percolation rate confirms the soil classification of HSG A. Due to the
nature of the drainage capacity of the soils found in the area. An infiltration line of 45% is used to provide an
assumed infiltration path for effluent in the system.
It can be seen on the attached section that the assumed infiltration line for the uphill, eastern most drain
field intersects the drain rock of the downhill, western most drain field. Approximately 60.7% of the west
drain fields effective depth is below the assumed infiltration line of the eastern drain field. It should be noted
that even if the drain fields were spaced as required by 15.65, approximately 53.1% of the west drain fields
effective depth would be below the assumed infiltration line of the eastern drain field.

Private well and Septic System Permit Request
This is a design narrative for a permit to install a well and septic system on the subject property. The proposed
upgrade will serve a proposed four (4) bedroom house. This lot and surrounding lots are served by private wells.
Currently there are no wells within 100 feet of the proposed installation.
1. Soils: A test hole was performed on this lot by Pannone Engineering Services, LLC on 6/27/2024. Groundwater
was not observed during the excavation of the test hole, and bedrock was not encountered. No groundwater
was observed in the test hole monitor tube after 7-days. Based on the results of the percolation test, sieve
analysis and overall soils appearance an application rate of 1.2 gpd/SF was used for the design of a conventional
wastewater treatment system in the area of the test hole.
2. Soil Absorption System Design: See Sheet 1 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field.
4. Topography: See attached site plan for area topography. The slope down gradient of the drain field is
approximately 26% based on MOA lidar. This installation conforms to the steep slope provisions of AMC 15.65.
5. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the
review is complete and no further comments are received from MoA On-Site Department, the note will be removed
and “For Construction” drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on
procedures outlined in the Certificate of On-Site Systems Approval Guidelines, indicates that the on-site water supply
and/or wastewater disposal system appears to comply with applicable Municipal and State codes, ordinances,
and regulations in effect at the time of installation, unless noted otherwise.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in
accordance with ADEC & MOA guidelines and regulations. The reported results describe the performance of the system
under the conditions encountered at the time of the test, and separation distances measured to readily identifiable
features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that
may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside
the control of the evaluator of this system. All systems eventually fail, and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore, we
cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this
report is for the sole benefit of the owner listed above.

A water sample revealed an arsenic concentration of 74.8 micrograms per liter
(ug/L). The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 ug/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. information on arsenic is available from the On -Site Water and
Wastewater Program website (www.muni.org/onside) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -Site
Systems Approval.
