Septic system design
Dear On -Site Services Engineer:
The owner of the above lot intends to build a 4-bedroom home on the property, so we are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home as well as the proposed well, septic location
and alternate site. No conflicts exist between this proposed system and any other well or septic
system, whether on this lot or adjacent lots.
The ground surface on the lot is very flat, with a slight slope away from the ROW to the north.
There are no slopes greater than 25% within 50 feet downslope of either the primary or alternate
site. Contours are shown on the site plan showing the grade and direction of flow. Stormwater
drainage will not impact this septic system. The new trench will be constructed parallel to the slope
as much as possible.
Due to high groundwater and surface saturation from snowmelt, we were unable to get a valid
percolation rate from the perkable soils under the clayish gravel. However, we took a soil sample
to the lab and found it was classified as "silty gravel with sand (GM)", containing less than 16%
fines. Initial perk results confirmed that the soils do drain, although we couldn't finish 3 tests
before the water rose. We have designed trenches with a conservative perk rate of 61-90 min/in,
and we request that these results be accepted in lieu of a standard percolation test.
Wells on this and adjacent lots are shown. The new system will be a minimum of 50' from all
wells and surface water and more than 5' away from the septic tank. The alternate site is located
more than 6' away. Please refer to the attached test hole logs, plan and profile pages for the septic
design. If this design is followed, there will be no adverse impacts to adjacent properties.

Anchorage, herein the "MUNICIPALITY in accordance with Anchorage Municipals
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties
Agreement agree as follows:
I - Advanced Lastewater Treatment Systems. The Municipality grants permissic
Owner to utilize and operate an Advanced Wastewater Treatment System (AWE
described as Advantex FAP 2.0

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement, the Owner shall enter into a service agre
with an AWWTS service and maintenance provider approved by the Municipalit
manufacturer's representative. The AWWTS shall be maintained in a satisfactor
condition capable of performing as designed and producing treated septic etflueri
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to p
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. `i
includes an annual maintenance fee (typically $400 to $600).
Owner agrees that only maintenance and repair personnel approved by the Muni
or the manufacturer's representative will inspect and make any necessary mai
ntc
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the pQtei
failure of the system, which could include sewage backup and costly repairs or
replacement.

Certificate of On -site Systems Approval.
OVvner'agrees that the AWWTS installation and maintenance requirements as provided
by tyre AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction. maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring ofthe AWWTS as required by the
AWWTS approval.
3, T errn. The term of t17is Agreement steal I begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4_ Nonwalver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

Dear Mr. Selyutin:
This is in response to your August 27, 2024, application for a Department of the
Army (DA) permit to discharge 223 cubic yards of 2-inch gravel to build a 3,000 square
foot-building pad for a single family home and 100 cubic yards of 1-inch gravel for a
2,700 square foot-backyard around the house, as well as 67 cubic yards of 1-inch
gravel to construct an 1,800 square-foot driveway for a total of 390 cubic yards of fill in
0.16-acre of palustrine wetlands. It has been assigned file number POA-2024-00286,
Little Rabbit Creek, which should be referred to in all future correspondence with this
office. The project site is located within Section 1, T. 11 N., R. 3 W., Seward Meridian;
USGS Quad Map Anchorage A-8; Latitude 61.07583º N., Longitude 149.72795º W.;
8200 Leo Circle, in Anchorage, Alaska. All work would be performed in accordance with
the enclosed plan (sheets 1), dated August 27, 2024.
DA authorization is necessary because your project will involve placement of fill
material into waters of the U.S. under our regulatory jurisdiction.
Based upon the information and plans you provided, we hereby verify that the work
described above, which will be performed in accordance with the enclosed plan (sheet 1
dated August 27, 2024) is authorized by Nationwide Permit (NWP) No. 29, Residential
Developments. Enclosed is a copy of the NWP No. 29, as well as the Regional and
General Conditions. These documents are also available on our website at
https://www.poa.usace.army.mil/Missions/Regulatory/Types-of-Permits/Nationwide-
Permits/. The following Regional Conditions apply to your project: C – Activities
Involving Trenching, D – Site Revegetation for Projects with Ground Disturbing
Activities, E – Delineation of Project Footprint, and F – Maintenance of Hydrology
Patterns. You must comply with all terms and conditions associated with NWP No. 29,
as well as with the special conditions listed below:

Special Conditions:
1. Credit Purchase Notification: Prior to commencing the work authorized by this
permit, the Permittee shall purchase 0.20-credit palustrine credits from an approved
credit provider for the loss of 0.10-acre of palustrine wetlands. You must email the
signed credit transaction form to mitigationmanager@usace.army.mil and to
Estrella.f.campellone@usace.army.mil upon completion of credit transaction (see
form attached). If you are unable to complete this transaction, you are required to
obtain a permit modification prior to commencing the work authorized by this permit
for approval of an alternate mitigation method.
2. Fill Material: The Permittee shall use only clean fill material for this project. The fill
material shall be free from items such as trash, debris, automotive parts, asphalt,
construction materials, concrete blocks with exposed reinforcement bars, and soils
contaminated with any toxic substance, in toxic amounts in accordance with Section
307 of the Clean Water Act.
3. Erosion Control: The Permittee shall install erosion control measures along the
perimeter of all work areas to prevent the displacement of fill material outside the
authorized work area. The erosion control measures shall remain in place and be
maintained until all authorized work is completed and the work areas are stabilized.
Immediately after completion of the final grading of the land surface, all slopes, land
surfaces, and filled areas shall be stabilized using sod, degradable mats, barriers, or
a combination of similar stabilizing materials to prevent erosion.
4. Erosion control material: To the maximum extent practicable, plastic-free erosion
and sediment control products such as netting manufactured from 100-percent
biodegradable materials like jute, sisal, or coir fiber shall be used for erosion control.
5. Upland Staging/Disposal: All excavated or dredged material being displaced shall
be disposed of at an upland location. No stockpiling or side casting of fill materials
shall occur in wetlands or other waters of the U.S. including temporary stock piling or
side casting of less than three months, that does not have Department of Army
authorization.
6. Protection of Water Circulation Patterns: The permittee shall ensure that the
project minimizes alterations to water circulation patterns to the extent practicable. If
it is determined by the Corps that the project negatively impacts the hydrology within
Waters of the U.S., as evidenced by ponding water, or drying of soils and vegetation,
change in deposition patterns or other negative secondary impacts adjacent to the
project footprint, the permittee may be required to take additional measures (i.e.
install additional depressed road beds, culvert(s), or a similar water conduit) in the
authorized project to re-establish the hydrology of the area to that of pre-construction
conditions.

7. Archeological Discoveries: If human remains, historic resources (which includes
resources 50 years or older), or archaeological resources are encountered during
construction, all ground disturbing activities shall cease in the immediate area and
you shall immediately (within one business day of discovery) notify the U.S. Army
Corps of Engineers (Corps), Alaska District, Regulatory Office at (907) 753-2712 or
Estrella Campellone at (907) 753-2518 and Estrella.f.campellone@usace.army.mil.
Upon notification the Corps shall notify the State Historic Preservation Office
(SHPO) and appropriate Tribe/s. Based on the circumstances of the discovery,
equity to all parties, and consideration of the public interest, the Corps may modify,
suspend or revoke the permit in accordance with 33 CFR Part 325.7. After such
notification, project activities shall not resume without written authorization from the
Corps.
Further, please note General Condition 30 requires that you submit a signed
certification to us once any work and required mitigation are completed. Enclosed is the
form for you to complete and return to our office.
Unless this NWP is modified or revoked, it expires on March 14, 2026. If you
commence or are under contract to commence this activity before the date that the
NWPs are modified or revoked, you will have twelve (12) months from the date of the
modification or revocation of the NWPs to complete the activity under the present terms
and conditions of these nationwide permits. It is incumbent upon you to remain informed
of the changes to the NWPs.
Nothing in this letter excuses you from compliance with other Federal, state, or local
statutes, ordinances, or regulations.

29. Residential Developments
Discharges of dredged or fill material into non-tidal waters of the United States for the
construction or expansion of a single residence, a multiple unit residential development,
or a residential subdivision. This NWP authorizes the construction of building
foundations and building pads and attendant features that are necessary for the use of
the residence or residential development. Attendant features may include but are not
limited to roads, parking lots, garages, yards, utility lines, storm water management
facilities, septic fields, and recreation facilities such as playgrounds, playing fields, and
golf courses (provided the golf course is an integral part of the residential development).
The discharge must not cause the loss of greater than 1/2-acre of non-tidal waters of
the United States. This NWP does not authorize discharges of dredged or fill material
into non-tidal wetlands adjacent to tidal waters.
Subdivisions: For residential subdivisions, the aggregate total loss of waters of United
States authorized by this NWP cannot exceed 1/2-acre. This includes any loss of waters
of the United States associated with development of individual subdivision lots.
Notification: The permittee must submit a pre-construction notification to the district
engineer prior to commencing the activity. (See general condition 32.) (Authorities:
