The attached approved design. 2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80) 3. The wastewater code requires inspections during the installation. The engineer shall notify the Development Services Department per AMC 15.65. Coodinate with On-Site staff regarding preferred method. 4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather shall be either: a. Opened and Closed on the same day, or b. The installation will serve a proposed five (5) bedroom house. This lot and surrounding lots are served by private wells. Currently there are no wells within 100ft of the proposed installation. 1. Soils: A test hole was performed on this lot by Garness Engineering Group, Ltd on 04/01/2011. The approximate location is shown on the attached site plan. This test hole (TH#2) was used for the basis of design for the soil absorption system on the subject property. Groundwater was observed during the excavation of this test hole at a depth of 6.0’ below ground surface (BGS), and bedrock was not encountered. Groundwater was observed in the test hole monitor tube after 7-days at a maximum depth of 5.0’ BGS. Based on the results of the percolation test and overall soils description of this test hole; an application rate of 1.0 gpd/SF was used for the design of a conventional wastewater treatment system in the area of the existing test hole. Several test holes were performed by Alaska Water & Wastewater Consultants, Inc, in the area as a part of subdivision approval: all of which depict the surrounding lots exhibiting similar soil strata and characteristics and to the subject property. Due to the majority of the proposed system lying outside of the 30.0’ test hole radius of (TH#2); another test hole (TH (P)) shall be conducted by PES in the location as depicted on the attached site plan –at the time of the construction of the proposed wastewater system –with subsequent change order applied for if the proposed installation is found to be unsuitable given the test hole results. 2. Soil Absorption System Design: See design package. 3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. 4. Topography: See attached site plan for area topography. 5. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the review is complete and no further comments are received from MoA On-Site Department, the note will be removed and “For Construction” drawings will be issued.
