2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)

Services Department per AMC 15.65. Coodinate with On -Site staff regarding preferred method.
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather

RE: Paradise Valley Block 10 Lot 4 – Septic System & Well Construction
A permit is requested to install an Advanced Wastewater Treatment System (AWWTS) septic tank and
conventional soil absorption system, as well as a private well, for the subject property. The proposed
system will serve a proposed 4-bedroom, single-family home. This design is for a 4-bedroom structure.
Wells
The subject property is not currently served by a water well. Neighboring parcels are served by private
water wells. There are no neighboring wells within 100’ of the designed septic system. The neighboring
parcels that share a property line with the subject property are vacant. The proposed on-lot private will is
within 100’ but maintains at least 50’ of separation from the proposed septic system.
Soils
AK Built Construction & Design, LLC (AK Built) performed test hole excavations and percolation testing on
October 9th, 2025. Three test holes were excavated. TH-01 was excavated to five feet below ground
surface; soils were visually classified as silt with a six-inch sand seam coincident with groundwater at two
feet below ground surface. TH-02 was excavated to ten feet below ground surface; excavated overburden
overlaid the test hole location from the driveway cut on the property, followed by silty sand and sandy
silt layers. No groundwater was encountered in TH-02. A percolation test was conducted at five and a half
feet to six feet below ground surface and resulted in a percolation rate of 10.2 minutes per inch.
Groundwater was monitored at least seven days later, and TH-02 was found to be dry. TH-03 was
excavated eight feet below ground surface; the organic soil layer was previously removed so no
overburden was present, the remainder of the test hole visually appeared to be primarily silt with a silty
sand seam collocated with the groundwater level. Groundwater in TH-03 was observed at four feet below
ground surface and monitored at least seven days later at 3.7 feet below ground surface. An application
rate of 5 gallons/day/square foot shall be used for a conventional pit, deep trench or wide trench
wastewater system utilizing Category III AWWTS.
Wastewater disposal systems
The designed wastewater disposal system is a wide trench system with 6” of sewer rock below the pipe
invert. The designed system has the following characteristics.

Replacement wastewater disposal systems
A replacement wastewater disposal system design is provided for the subject parcel. The replacement
wastewater disposal system is a wide trench system with 6” of sewer rock below the pipe invert. The
designed system has the following characteristics.

Existing disposal site
There is no existing disposal site located on the subject parcel. Neighboring parcels with existing disposal
sites do not impact the subject parcel and the proposed system will not negatively impact neighboring
parcels.
Drainage
The southwest property line parallels Potter Creek. The proposed septic system is at least 100 feet away
from Potter Creek surface waters. There is a drainage easement along the southeast property line. There
are no signs of visible surface water located along the drainage easement.
Topography
The designed system is in the north quadrant of the property. The surrounding property slopes up-
gradient to the northeast and slopes down-gradient to the southwest. Steep slopes exist at least 50 feet
from the designed system, as shown in the plans. The proposed wide trench system is installed on a slope
greater than 25% percent but less than 46%. The maximum excavation depth shall be measured from the
up-hill side of the trench. Maximum trench width is five feet. Natural vegetation within 50 feet down-
gradient of the disposal field shall remain undisturbed. Exposed slopes down-gradient of the disposal field
shall be stabilized with an erosion control matting or hydroseed incorporating a tackifier. The distribution
pipe invert is less than 36-inches below original ground as measured from the downhill side of the trench,

therefore a one foot filter sand layer shall be placed below the disposal field to ensure the filter sand is
placed at least 36-inches below the top of the native organic layer and ensure effluent distribution is
conveyed to the specified receiving soils. The disposal largely meets the requirement of being 100 feet
up-gradient from any slope exceeding 46%. Much of the down-gradient slope is less than 46% but a few
isolated sections of slope are steeper than 46% and within 100 feet of the proposed disposal field, as seen
in the site plan. It is not believed that the designed system will result in daylighting effluent.
The designed system does not have probable adverse impacts to adjacent properties.

3. TEST HOLES WERE EXCAVATED TO COLLECT SOIL, PERCOLATION, AND GROUND WATER DATA FOR THE
DESIGNED SYSTEM. SEE SOILS REPORTS.
4. SEPTIC TANK CAPACITY IS A MINIMUM OF 1,000-GALLONS PLUS 250-GALLONS FOR EACH BEDROOM OVER
THREE.
4.1. SEPTIC TANK SHALL BE A 1,500-GALLON ADVANTEX AX20 UNIT. A PUMP VAULT SHALL BE UTILIZED TO
ACCOMPLISH PRESSURE DISTRIBUTION.
5. DISPOSAL FIELDS SHALL BE DESIGNED TO ACCEPT 150-GALLONS OF WASTEWATER PER BEDROOM PER
DAY.
5.1. DESIGN FLOW = 150 GPD/BDRM x 4 BDRM = 600 GPD.
6. THE MINIMUM EFFECTIVE ABSORPTION AREA OF A DISPOSAL FIELD SHALL BE CALCULATED USING THE
WASTEWATER APPLICATION RATE CORRESPONDING TO THE PERCOLATION RATE IN ANCHORAGE
MUNICIPAL CODE (AMC) 15.65 TABLE 6.

ACCORDANCE WITH THE CURRENT EDITION OF MOA'S PUBLICATION "STANDARDS AND SPECIFICATIONS
FOR COMPONENT PARTS AND MATERIALS USED IN CONSTRUCTION OF ON-SITE WASTEWATER DISPOSAL
SYSTEMS".
2. COMPONENTS INCIDENTAL TO THE SEPTIC SYSTEM INCLUDING BUT NOT LIMITED TO PIPING, FITTINGS,
PUMPS AND VALVES THAT ARE LISTED OR LABELED BY A MOA APPROVED ACCREDITED THIRD PARTY
LISTING AGENCY FOR THE INTENDED USE DO NOT REQUIRE THE MOA SUBMITTAL/APPROVAL PROCESS.
3. SEPTIC TANKS SHALL BE CONSTRUCTED FROM DURABLE, CORROSION-RESISTANT MATERIALS, INCLUDING
CONCRETE, FIBERGLASS, OR PLASTIC. SEPTIC TANKS CONSTRUCTED FROM STEEL SHALL BE COATED ON
BOTH THE INTERIOR AND EXTERIOR WITH AN APPROVED POLYURETHANE LINING OR SUPERIOR MATERIAL.
4. ALL SEPTIC TANKS SHALL BE FITTED WITH WATERTIGHT COUPLINGS, APPROVED BY MOA, AT THE
PUMP-OUT ATTACHMENTS AND ON THE INLET AND OUTLET OF THE TANK.
5. A SEPTIC TANK SHALL HAVE A 20" MINIMUM DIAMETER MANWAY RISER SERVING THE FIRST
COMPARTMENT. THE RISER, INCLUDING THE COVER, SHALL BE INSULATED WITH 4" MINIMUM INSULATION
EXTENDING 48" BELOW GRADE OR TO THE TANK INSULATION. THE INSULATION SHALL BE OF AN APPROVED
TYPE SUITABLE FOR BELOW GRADE APPLICATIONS. EXPOSED INSULATION ABOVE GRADE SHALL BE
PROTECTED FROM UV DAMAGE. THE RISER LID SHALL BE SECURED TO PREVENT UNINTENDED ACCESS.

CONSTRUCTION SPECIFICATIONS:
1. SEPARATION DISTANCES AS IDENTIFIED IN THE "SEPARATION DISTANCES" TABLE SHALL BE MAINTAINED.
NOTIFY THE ENGINEER OF DISCOVERIES MADE DURING CONSTRUCTION THAT IMPACT SEPARATION
DISTANCE REQUIREMENTS.
2. FLAG AND SURVEY ALL PROPERTY LINES, WELL RADII, AND EXISTING FIELD SEPARATIONS PRIOR TO START
OF CONSTRUCTION.
3. LAYOUT ALL FIELD COMPONENTS AND LOCATIONS PRIOR TO START OF CONSTRUCTION. NOTIFY THE
ENGINEER IF CONFLICTS EXIST.
4. GROUNDWATER MONITOR TUBES SHALL REMAIN IN PLACE UNTIL CONSTRUCTION BEGINS. ABANDON
GROUNDWATER MONITOR TUBES BY CUTTING ONE FOOT BELOW GRADE AND ABANDON IN PLACE. IF THE
GROUNDWATER MONITOR TUBE IS WITHIN FIVE FEET OF THE DISPOSAL FIELD, REMOVE THE
GROUNDWATER MONITOR TUBE IN IT'S ENTIRETY.
5. TANKS WITH TWO TO FOUR FEET OF COVER SHALL BE INSULATED WITH A MINIMUM OF 2" OF APPROVED
INSULATION PLACED IMMEDIATELY ABOVE THE TOP OF THE TANK. TANKS SHALL NOT HAVE LESS THAN
TWO FEET OF COVER. INSULATION SHALL EXTEND 12" BEYOND THE EDGE OF TANK FOR EVERY 1" OF
INSULATION PROVIDED.
6. A SEPTIC TANK SUBJECT TO BUOYANCY FORCES SHALL BE ANCHORED OR BALLASTED AS REQUIRED TO
PREVENT FLOTATION REGARDLESS OF THE LIQUID LEVEL IN THE TANK.
7. A SEPTIC TANK SHALL HAVE A 4" MINIMUM STAND PIPE PROVIDING ACCESS TO OTHER COMPARTMENTS
NOT SERVED BY THE MANWAY RISER.
8. PROVIDE AIR TIGHT CAPS ON ALL CLEAN OUTS AND MONITORING TUBES.
9. PROVIDE A BUILDING CLEAN OUT INSTALLED ONE TO FOUR FEET FROM THE BUILDING FOUNDATION. IF NOT
PRACTICAL, A DOUBLE CLEAN OUT SHALL BE INSTALLED ON THE UPSTREAM SIDE OF THE SEPTIC TANK
WITHIN TEN FEET OF THE INLET.
10. A DOUBLE CLEAN OUT SHALL BE PROVIDED WITHIN TEN FEET OF THE SEPTIC TANK OUTLET. THE
OPPOSING CLEAN OUTS SHALL BE ORIENTED SUCH THAT THE CLEAN OUT CLOSEST TO THE TANK SHALL
BE TO CLEAN THE LINE AWAY FROM THE SEPTIC TANK, AND THE CLEAN OUT FURTHEST FROM THE TANK
SHALL BE ORIENTED TO ALLOW CLEANING TOWARD THE SEPTIC TANK. THE CLEANOUTS SHALL BE
LOCATED ON UNDISTURBED SOIL. A PRESSURE SYSTEM DOES NOT REQUIRE A DOUBLE CLEAN OUT.
11. SINGLE SEGMENTS OF SUBSURFACE DISPOSAL FIELDS SHALL NOT EXCEED 100 FEET IN LENGTH.
12. THE LONG AXIS OF A DISPOSAL FIELD SHALL FOLLOW THE CONTOURS OF THE ORIGINAL GROUND.
13. THE HORIZONTAL SEPARATION DISTANCE BETWEEN THE CLOSEST EDGES OF WORKING OR
DECOMMISSIONED SUBSURFACE DISPOSAL FIELDS, OR SEGMENTS OF SUBSURFACE DISPOSAL FIELDS,
SHALL BE AT LEAST TWICE THE DEPTH OF THE DRAIN ROCK BELOW THE LEVEL OF THE DRAINPIPE, BUT
NOT LESS THAN SIX FEET.
14. THE BOTTOM OF THE TRENCH SHALL BE LEVEL WITHIN 4". THE BOTTOM OF A WIDE TRENCH OR BED
DISPOSAL FIELD SHALL BE LEVEL WITHIN 2" BEFORE PLACEMENT OF DRAIN ROCK.
15. AFTER EXCAVATION HAS EXPOSED THE DESIGNATED INFILTRATIVE SURFACE, ANY NATIVE SOIL THAT IS
SMEARED SHALL BE SCARIFIED TO IMPROVE ITS POROSITY BEFORE PLACEMENT OF DRAIN ROCK.
16. PERFORATED DISTRIBUTION PIPE USED IN A SUBSURFACE DISPOSAL FIELD SHALL BE LAID LEVEL AND

18. AT LEAST ONE 4" DIAMETER MONITORING TUBE SHALL BE INSTALLED IN EACH SEPARATE DISPOSAL FIELD
SEGMENT TO ALLOW MEASUREMENT OF THE FLUID LEVEL IN THE DISPOSAL FIELD. MONITOR TUBES SHALL
NOT BE CONNECTED TO THE DISTRIBUTION PIPE NETWORK. THE PORTION OF A MONITOR TUBE
EXTENDING DOWN FROM THE LEVEL OF THE HORIZONTAL DISTRIBUTION PIPES TO THE BOTTOM OF THE
DRAIN ROCK SHALL BE PERFORATED. THE PORTION OF THE MONITOR TUBE ABOVE THE HORIZONTAL
DISTRIBUTION PIPES SHALL BE NON-PERFORATED. MONITOR TUBES SHALL BE LOCATED AT ALL ANGLE
POINTS OF DISPOSAL FIELDS TO MARK THE LOCATION OF THE ABSORPTIONS SYSTEM. PRESSURIZED
TRENCH DISPOSAL FIELDS THAT DO NOT HAVE CLEAN OUT PIPES SHALL HAVE A MONITOR TUBE WITHIN
ONE FOOT OF EACH END OF INDIVIDUAL TRENCH SEGMENTS. PRESSURIZED BED DISPOSAL FIELDS THAT

19. AN APPROVED PERMEABLE GEOTEXTILE SILT BARRIER SHALL BE INSTALLED COVERING THE ENTIRE TOP
SURFACE OF THE DRAIN ROCK PRIOR TO BACKFILL.
20. THE DISPOSAL FIELD COVER SHALL BE A MINIMUM OF TWO FEET DEEP, OVER THE TOP OF THE DRAIN
ROCK. IF THE DISPOSAL FIELD COVER IS LESS THAN THREE FEET, THE DISPOSAL FIELD SHALL BE
INSULATED WITH 2" OF APPROVED RIGID BOARD INSULATION. MINIMUM SOIL COVER OVER THE INSULATION
SHALL BE ONE FOOT. INSULATION SHALL EXTEND 12" BEYOND THE EDGE OF TANK FOR EVERY 1" OF
INSULATION PROVIDED.
21. THE FINISH GRADE OVER A SUBSURFACE DISPOSAL FIELD SHALL BE MOUNDED A MINIMUM OF 6" ABOVE
ADJACENT GROUND TO PREVENT THE FORMATION OF A DEPRESSION AFTER THE BACKFILL HAS SETTLED.

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this 27 Day of
October
of 2025 , by and between
herein the "OWNER," and the Municipality of
Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as
located at (legal description) Paradise Valley Block 10 Lot 4
2. Maintenance, Repairs and Alterations.
,(Owner is required to read, understand and initial each section)
" jTh-oughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
---accordance with the equipment's approval for operation in the Municipality.
z : It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).

Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

f Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be
assessed in accordance with AMC 14.60.030.
'Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
-Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On -Site Systems Approval.
.i Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
'Owner agrees to maintain remote monitoring of the AWWTS as required by the

AWWTS approval.
3. Term. The term of this Agreement shall begin oil the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
