Dear Owners:
Our records show that the home located at 12521 Lake Otis Pkwy was recently purchased without the benefit of
a Certificate of On-Site Systems Approval (COSA). This means the systems involved are not approved to
operate as required by Anchorage Municipal Code (AMC). The use of an unapproved well or septic system is
subject to fines under AMC 14.60.030.
Please contact our office within 30 days to provide a status update on getting this permit closed. The Onsite
Water and Wastewater Section will do what we can to issue a COSA and approve the well and septic system to
operate.
