We have completed our review of the submitted information and information found in this office's files on public water systems in Block 5 Sand Lake Subdivision. Based upon this review, we have the following comments: There is no information in this office's files that shows the existing Class C Public Water System and the Source Well located on Lot 21, Block 5, Sand Lake Subdivision has received operation approval from this Department. If you or your client has a copy of any type of approval issued by any agency, please submit a copy of such information to this office. If that is not possible, the highlighted items in the attached "Plan Approval Checklist for Engineering Plans, Class C Public Water System" will need to be submitted for review and, if appropriate, issuance of operation approval. Please note that an owner's statement and the date that the source well was installed will need to be included with the other items noted in the plan review checklist. From our review, it appears the lesser separation distance could provide the needed treatment of the wastewater, which is needed to protect public health and the environment. Alaska Testlab's standard procedure is in conformance with these standards, with the following descriptions: • The coarse fraction of non -extracted soils is not washed unless the coarse particles appear to be significantly coated with fines; • The fine fraction of the st;11 is cdla}s Washed; • The plus 3 -inch fraction is not routinely included in the test due to the large sample mass required for a representative sample; The estimated percentage of plus 3 inch material in the sample is shown on the test report; and • The mass of the coarse and fine test fractions are reported. The soil is classified in accordance with ASTM D 2487, "Classification of Soils for Engineering Purposes (Unified Soil Classification System)." The frost classification is identified in accordance with Corps of Engineers and Municipality of Anchorage (MOA) procedures. The test results are attached. The scope of this project includes the installation of a 1250 gallon septic tank and a five foot wide SEWER&WATER drainf ield to serve the existing four bedroom INSPECTION residence on the referenced property. The existing septic system is to be excavated, pumped, crushed and abandoned. ENGINEERING STUDIES 2. Construction shall be in accordance with the approved AND REPORTS site plan and design drawings, Municipal permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. WELL INSPECTION & FLOW TEST 3. The contractor shall be responsible for obtaining any necessary underground utility locates. SITE PLANS 4. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. On all leachfield mound systems, the property owner shall be ROADDESIGN responsible for ensuring a satisfactory vegetation growth over the mounded area. 5. Contractors installing wastewater disposal systems SOILTEST must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. PERCOLATION TEST EPTIC TANK INSTALLATION: 1, A septic tank is to be constructed by a certified STRUCTURAL& septic tank manufacturer. Construction shall include MECHANICAL two 4" cleanouts for pumping access. INSPECTIONS 2. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 4. Septic tanks installed with less than 4 ft. of cover shall be insulated. 5. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. 6. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. ABSORPTION TRENCH/DRAINFIELD INSTALLATION: 1. Excavate the proposed trench to the dimensions shown on the design. The bottom of the excavation shall be within 2 inches of level. If the sidewalls of the excavation become smeared, they must be raked or scratched (roughed -up) before gravel (sewer rock) placement. 2. Once the gravel is installed, the distribution pipe is to be installed level with the perforations faced downward. Gravel is then to be placed over the distribution pipe to provide a minimum of 2 inches of cover over the pipe. 3. A silt barrier must be installed between the final gravel layer and the native soil backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. 4. Monitor tubes shall be of four (4) inch diameter, installed approximately in the locations shown on the design, and extend a minimum of 12 inches above final grade. The portion of the monitoring tube extending through the gravel shall be perforated from the bottom of the trench to the invert of the distribution pipe. This is equivalent to the effective depth of the gravel as noted on the design. Page Three Lot 20 & 21, Block 5, Sand Lake S/D #2 September 17, 1996 5. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finish grade over the trench must be mounded to prevent the formation of a depression after settling. MINIMUM MATERIAL SPECIFICATIONS: 1. Any septic tank proposed for installation must be constructed by a Municipally approved septic tank manufacturer. 2. The following pipe materials are approved for use in septic system installations in the Municipality of Anchorage: Type of Pipe Perforated Solid Cast Iron Yes Yes ASTM D3034 (PVC) Yes Yes ASTM F810 (HDPE) Yes No ASTM D2662 (ABS) Yes Yes Use of a type of pipe other than listed above must be approved by the inspecting engineer. 3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam HI or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. 6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the #200 sieve. 7. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements, which ever applies. Page Four Lot 20 & 21, September 17, INSPECTIONS: Block 5, Sand Lake SID #2 1996 Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: 1. The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. 2. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. 3. The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre -construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractors activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractors activities. Final acceptance of the contractors work rests with the owner and the M.O.A. S & S Engineering's inspecting engineer will not be responsible for the construction means, methods, techniques, sequencet procedures or the safety precautions incident to this project. CONTRACTOR/INSTALLER EASEMENT AGREEMENT The undersigned, in consideration of mutual agreement set forth below,) grant, convey and affirm the following easement, effective this 21 day November 1996, and agree to the conditions and restrictions as follows: We, Donald M. Hennessey and Joan M. Hennessey, hereinafter referred to grantors, owners of certain real property herein after referred to as "Lot 21; Block 5; Sand Lake Subdivision #2". According to the official plat #P176 filed in the records of ANCHORAGE RECORDING DISTRICT, Third Judicial District, State of Alaska, do hereby grant to Donald M. Hennessey and Joan M. Hennessey, hereinafter referred to as the grantees, as owners of certain real property hereinafter referred to as "Lot 20; Block 5; Sand Lake Subdivision #2" acrordinry tto the nfficial nl8t #P176 filed in the records of ANCHORAGE RECORDINC DTSTRICT, Third Judicial District, State of Alaska, an easement for an on-site wastewater disposal system to be placed partially onl, Lot 21; Block 5; Sand Lake Subdivision #5 , together with right to maintain the on-site wastewater disposal system to include reasonable egress and ingress for maintenance of the absorption area. It is understood and agreed that sewer service to Lot 20; Block 5; Sand Lake Subdivision #2 is to be provided by connection beginning on Lot 20 and extending across Lot 21; Block 5; Sand Lake Subdivision #2 . It is further agreed that the cost of maintenance of the in place sewer lines shall be born by the grantees. 2. There shall be no charge for the use of the on-site wastewater disposal facilities so long as this easement is in affect unless a charge is arranged between the parties of their heirs or assigns or successors in interest by separate written agreement. 3. Grantors makes no warranty as to the adequacy of the disposal system and shall not be liable for any damages caused by system failinq to work adequately. 4. The grantors further covenant and agree that the grantees shall full, free and uninterrupted use of said easement for domestic use as long as such use does not create an unreasonable adverse impact upon the grantors. 5. Failure to make use of the on-site wastewater disposal system described shall not be deemed abandonment of the said easement. 6. The grantees further agree to bear all cost, if any, for the operation, maintenance and/or abandonment of the on-site wastewater d system and/or easement. 7. This easement from grantors to grantees shall be binding upon the to the benefit of the parties hereto, their heirs, representative, successors and assigns, and shall run with the land. Health & Human Services Eagle River, Alaska 99577 SUBJECT: Lots 20 and 21, Block 5, Sand Lake Subdivision; Separation Distance Waiver, Anchorage, Alaska, AWPSA Project Number 9721 -WV -269-020; Review Dear Mr. Cowan: This letter is in response to the submittal received on September 25, 1996 in which you requested that the Department grant a lesser horizontal separation distance between a Class C Public Water System Source Well located on Lot 21, Block 5 and a wastewater disposal system located on Lots 20 and 21, Block 5, which is serving a single family home. We have completed our review of the submitted information and information found in this office's files on public water systems in Block 5 Sand Lake Subdivision. Based upon this review, we have the following comments: There is no information in this office's files that shows the existing Class C Public Water System and the Source Well located on Lot 21, Block 5, Sand Lake Subdivision has received operation approval from this Department. If you or your client has a copy of any type of approval issued by any agency, please submit a copy of such information to this office. If that is not possible, the highlighted items in the attached "Plan Approval Checklist for Engineering Plans, Class C Public Water System" will need to be submitted for review and, if appropriate, issuance of operation approval. Please note that an owner's statement and the date that the source well was installed will need to be included with the other items noted in the plan review checklist. From our review, it appears the lesser separation distance could provide the needed treatment of the wastewater, which is needed to protect public health and the environment. Therefore, in accordance with the provisions of 18 AAC 80.030, the following separation distances are granted: Between the existing Class C Public Water System Source Well located on Lot 21, Block 5, Sand Lake Subdivision #2 and the wastewater disposal system (septic tank and soil absorption system) located on Lots 20 and 21, Block 5, Sand Mr. Cowan 2 October 31, 1996 Lake Subdivision #2, from 150 to 100 feet. The following are requirements of the above granted waiver: The existing Class C Public Water System will need to receive final operation approval within one year after the issuance the requested waivers. Verification that the vertical separation distance of four (4)feet between the lowest part of the soil absorption system and the seasonal high ground water has been met. In addition, verification of the in-situ soils will need to be submitted to this office. As -built drawings, under the stamp of a Professional Engineer (P.E.), need to be submitted to this office within 90 days of completion of the project. The as -built drawings will need to include a cross section of the soil absorption system. This waiver will remain valid until the wastewater disposal system on Lots 20 and 21, Block 5 again requires upgrading or if it is determined that the new wastewater disposal system is the source of contamination in the existing source well or any other in the Sand Lake Subdivision. If upgrading or replacement of the new wastewater disposal system due to the above noted problems is necessary, due to the small lot size and the high density of wastewater disposal systems, additional treatment to the waste stream will be a requirement of any future waiver requests for this property. This approval to construct does not imply the granting of additional authorizations, nor obligate any federal, state, or local regulatory agency to grant the required authorizations.
