The proposed system will serve a 3 -bedroom single-family residence. The existing septic tank and drain field will be abandoned per MOA code requirements. System: A 1250 gallon STEP septic tank with a 15' X 30' absorption bed w/ 2' of sand. Soils: A test hole was excavated on August 4, 2008. Seethe attached soil logs. Generally speaking gravelly sand with cobbles and boulder up to and exceeding 24 inches in diameter were observed to a depth of 14 feet. Ground water was monitored for through August 12, 2008. Ground water was observed 12 feet below the ground surface. Surface Water: There are no surface waters within 100 feet of either the proposed septic system area or the reserve site. Topography: The average topography in the area of the septic system absorption field is generally level. Waivers: The home owner owns both Lot 1 Block 4 Seidler # 1 and Broadwater heights Block 2 Lot 1. A suitable site for the upgrade does not exist on Lot 1 Block 4 Seidler 41. We are proposing locating the septic system on Lot 1 Block 2 Broadwater Heights. Permission from right- of-way has been granted to cross the utility easement with a sewer service line. Box 196650 Anchorage,AK 99519-6650 SUBJECT: SEIDLER #1 Blk.4 Lot 1- COSA Inspection In our efforts to test,inspect,and receive COSA approval of the on-site well and wastewater systems on this property we encountered the uncompleted construction inspection report for permit SW080250. As a background,this septic upgrade was designed and permitted by Lars Spruckland, P.E. in October 2008. Bob Dean of Dean's Construction did construction of the project and per Bob the first inspection on the field was done by Lars in the morning of December 12,2008 with a second inspection scheduled for that afternoon. Unfortunately, Lars went into the hospital that day and passed away and no further inspections were done. Dean's Construction completed the installation and the property has not had any problems with the system. We tested and inspected the septic system on November 29,2018,and found the layout of the system to be substantially laid out as the original permit design shows (we found one monitoring tube not two as shown on the design). We have obtained an as-built survey from Fred Walatka and Associates dated October 5,2009 that also shows only one monitoring tube.The field measurements show the monitoring tube to be 64" with a 28" stickup thus 36" to the top of sand filter per design. We have attached the 2009 asbuilt survey,the MOA utility easement encroachment approval,and the lift station inspection report from Larry Betts as requested.Our inspection report,water sampling and the recorded Ingress/Egress Easement between the two properties affected have been submitted with our request for COSA approval.
