system, will expire on 01 Nov 07. Permits are valid for 365 calendar days from date of issue.
If this was a -.yell permit and the well has been drilled, a well log and a pump install log must be sent
to this office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

ADVANTEXT"t TREATMENT SYSTEM
To whom it may concern:
1. GENERAL: The proposed 4 bedroom home will be served by a private well and septic
system. A total of four test holes have been excavated on the property. Two of the test holes were
logged by S & S Engineering and the other two test holes were logged by our firm. We are
proposing to design the septic system around the 30 foot radius of our TH# 1. Due to the shallow
bedrock, the limited amount of receiving soils, and the steep slopes, we are proposing to install a
1500 gallon Advantex treatment system and a 5 -wide trench type drainfield. The proposed well
is to be located in the area shown on the site plan and design drawing.
2. SOILS: See the attached log, which shows the soil classifications, groundwater monitoring,
and the percolation test results.
3. DRAINFIELD APPLICATION RATE: The intent is to install an Advantex Treatment
System that will allow the use of a small drainfield in the area around the 30 foot radii of the test
hole #1. Two percolation test where performed in this test hole, and the percolation rates ranged
from to be 9.6 to 26.7 minute/inch. We are going to use an application rate of 3.0 GPD/FT2.
4. TRENCH DESIGN:
a. Percolation Rate: 9.6 & 26.7 minutes/inch
b. Allowable Application Rate: 3.0 gallons/day/R2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 200112
f. Total Depth: 6.0 feet (max: at any point)
g. M.O.A. Approved Sand Filter Depth: 2 feet
h. Effective Depth: 0.5 feet

5. SURFACE WATER: There is a section of the stream that is not culverted in the northwest
corner of properly within 100 feet of the proposed septic system. We are proposing to have this
section culverted. A right of way permit will be obtained in order to perform this work and the
construction of the culvert and manhole shall be completed prior to the drilling of the well or the
installation of the septic system.
6. TOPOGRAPHY: As can be seen on the attached design, the average topography where the
proposed drainfield is to be installed is a 5 to 15 percent slope running from approximately
southeast to northwest.
7. CLOSING: I am open to any suggestions from your department that would be an
improvement to the proposed system. I am unaware of any negative impacts that this installation
would impose on adjacent wells, or septic systems. If you have any questions, please call us at

ADVANCED WASTEWATER TREATMENT SYSTEM MAINTENANCE AND
REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN 1 OF ANCIIORAGE
AND
THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this
day of 1VbVZfh&Y of 20N, by and between ALL-iq DA o:5
herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as ADVAM 7ER located
at Sel DLe— ?61-k J L� , Anchorage, Alaska.
2. Definitions.
A. Alteration. Any change to the design or function of an AW\VTS that
includes the installation or removal of any parts, components or pieces
not included in the original construction permit and design. Prior to
performing any alterations to an AW WTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
These approvals certify that the systems are adequate for the homes that
they support and meet the codes that Avere in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction) An On -Site Wastev.-ater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating) An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On -Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. Fee. Owner shall pay to Municipality an annual fee of _D — �Ve'CIO
($ f) .00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Term. The term of this Memorandum of Understandinga shall be for the life of the
AWWTS.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWNN7S system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On -Site approval is issued to
the new owner or transferee of the property.
5. Alterations, Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWVIrfS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWAVTS
Operating Permit is:
C. Owner acknowledges that the fine scXedule for failing to maintain and repair an
AWNVTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
G Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
+sithout a new Certificate of On -Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification guidebook
for AW WTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AW WTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any +say effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govem the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.

To whom it may concern:
1. GENERAL: The proposed 4 bedroom home will be served by a private well and septic
system. A total of four test holes have been excavated on the property. Two of the test holes were
logged by S & S Engineering and the other two test holes were logged by our firm. We are
proposing to design the septic system around the 30 foot radii of our TH#2 and the north test
hole by S & S Engineering. Due to the shallow bedrock, the limited amount of receiving soils,
and the steep slopes, we are proposing to install a 1500 gallon Advantex treatment system and a
5 -wide trench type drainfield. The proposed well is to be located in the area shown on the site
plan and design drawing.
2. SOILS: See the attached log, which shows the soil classifications, groundwater monitoring,
and the percolation test results.
3. DRAINFIELD APPLICATION RATE: The intent is to install an Advantex Treatment
System that will allow the use of a small drainfield in the area around the 30 foot radii of the test
holes. A percolation test was performed in three of the test holes and the percolation rates ranged
from to be <1 to 26.7 minute/inch. We are going to use an application rate of 4.0 GPD/FT2.
4. TRENCH DESIGN:
a. Percolation Rate: <1 & 26.7 minutes/inch
b. Allowable Application Rate: 4.0 gallons/day/It2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 150 A2
f. Total Depth: 3.0 feet (max: at any point)

5. SURFACE WATER: There is some surface water to the northwest of the proposed septic
system. The 100 foot setback to these surface waters will be flagged by a registered land
surveyor in order to insure the 100 foot setback requirement is met.
6. TOPOGRAPHY: As can be seen on the attached design, the average topography where the
proposed drainfield is to be installed is a 5 to 15 percent slope running from approximately
southeast to northwest. There is a cut bank running along the east property line. We are
requesting a 35' waiver from the cut bank to the proposed alternate site. We are unaware of any
adverse impacts on the adjacent lots with the granting of this waiver.
7. CLOSING: I am open to any suggestions from your department that would be an
improvement to the proposed system. I am unaware of any negative impacts that this installation
would impose on adjacent wells, or septic systems. If you have any questions, please call us at

Subject: Soil Investigation for Sanitary Sewer System Lot 4, Block 5,
Seidler Subdivision, Eagle River, Alaska
Dear Mr. Fischer:
At your request of August 17, 1977, we conducted a subsurface soils
investigation at the proposed location of the sanitary sewer system on
the subject lot. The investgation complied with those procedures required
by the Municipality of Anchorage Department of Health and Environmental
Protection.
This investigation, which was accomplished on August 18, 1977, was
conducted within a test hole excavated to a depth of 13 feet below the
existing ground surface. Excavation was accomplished with a backhoe and
the test hole location is shown in attached Drawing A-01. Material
exposed in the hole was monitored by an experienced engineering geologist.
The topography at the excavation site is generally steeply -sloping to
the north. At the time of the investigaton the site had original vegeta-
tion consisting of high grass and shrubs. The top of the test hole was
located at original ground surface.
The soils encountered in the excavation are shown in the test hole log
in Drawing A-01. The symbols used in Drawing A-01 are explained in
Drawings B-01, & B-02. This log displays specific conditions encountered
at the test location. However, subsurface conditions may vary in other
parts of the lot without any apparent surficial evidence of the change.
It is our understanding that at the time the trench was excavated,
bedrock was encountered at a depth of 131, and groundwater was encountered
'immediately "above the -bedrock. When our geologist arrived on the site,
there was water standing in the hole to a depth of 8.5' below the ground
surface.

Based on the visual classification of the soil and the requirements set
forth by the Municipality of Anchorage, aaercolation test was not
necessary within the test hole on the subject lot.

CLASSIFICATION, CONSISTENCY AND SYMBOLS
CLASSIFICATION: ldentifi.cati.on and classi.f1caLion of the soil is accomplished in
accordance v,,ith the Unified Soil Classification System. Normally, the grain size
distribution determines classification of the soil. The soil is defined according to
major and minor constituents with the minor elements serving as modifiers of the
major elements. For cohesive soils, the clay becomes the principal noun with the
other major soil constituents used as modifier; i,e, silty clay, when the clay particles
are such that the clay dominates soil properties. Minor soil constituents may be
added to the classification breakdown in accordance with the particle size proportion
listed below; i.e, sandy silt w/some gravel, trace clay.

SOIL CONSISTENCY - CRITERIA: Soil consistency as defined below and determined
by normal field and laboratory methods applies only to non -frozen material. For

in making any correlation with the consistency values listed below. In permafrost
zones, the consistency and strength of frozen soils may vary significantly and
