2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:

b. Covered, sealed, and heated to prevent freezing

Ref: Request for Re-Issuance for a Proposed Septic System Upgrade (with
Modification) for Seaview Heights; Block 2, Lot 5A
To whom it may concern:
The subject property received a permit to upgrade its septic system back in September
2017 (OSP171263). It expired back on 9/25/18 and our firm is requesting the permit be
re-issued. There has been one modification to the design for this re-issuance. The
modification is as follows;
• The proposed septic tank, AX-20 pod, and pump vault have all been moved
south, just outside the 50' radius of the private well located in the southeast
corner of the lot.
• The proposed steel tank is now proposed to be a poly coated steel tank.
• Due to changes in the MOA code, no horizontal separation distances waivers are
required for the proposed septic system since it is advanced treatment.
At this time, we request the permit be re-issued.

Ref: Proposed Septic System Upgrade for Seaview Heights; Block 2, Lot 5A
To whom it may concern:
The existing 2 bedroom house is served by a private well and holding tank. The
homeowner would like to get off of the holding tank and install a septic system. We are
proposing to install a new 1500 gallon STEP tank, one (1) Advantex AX-20 Pod, a
secondary pump vault and a deep trench type drainfield. Currently, there are three wells
on property, one that serves the house (well #3 on page 2/2), and two that are "non-
active" wells (well #1 and #2). Per conversations with Rebecca Carroll on 9/18/2017 the
following is a list of options that must be taken for the "in-active" wells (well #1 and 2 on
site plan):
• The wells are to be converted to agricultural wells with functioning pumps
• The wells are to be capped with a "welded cap" by a certified well driller
• The well are to be decommissioned per MOA Onsite Code
Per the MOA, any of the previously listed options are viable. However, well #2 must be
decommissioned if the homeowners do not wish to move the animal containment area
(minimum separation distance is 50 feet per MOA). Comments regarding the design are
summarized as follows:
1. SOILS: See the attached 2017 soil logs which show the soil classifications,
groundwater monitoring, and the percolation test results. As can be seen on the
attached site plan, there have been numerous testholes excavated on the subject
property, none of which indicated native material. We are proposing to install the
drainfield around G.E.G. testhole #3. Prior to the design submittal, GEG employee
(David Garness) met with MOA employees Deb Wokenfuss and Rebecca Carroll to
discuss the testholes that were performed by Cindy Ellis, P.E., Tobben Spurkland, P.E.
and GEG. The only suitable soils (imported fill) were found in the vicinity of GEG test
hole #3. It was agreed that a drainfeild would be permitted if consistent soils were found
within 5 feet of the proposed drainfield area (around GEG TH#3). Per the directive of

Rebecca Carroll, on 8/21/2017 confirmation testholes were to be excavated to a
minimum of the total depth of the proposed system (11 feet). Garness Engineering and
Wilco Contractors met onsite on 8/21/2017 and excavated several "exploratory"
testholes to a depth of 12 feet. Testholes were excavated 25 feet south and 10 feet
north of GEG TH#3 to meet the requirement set forth by the MOA. These excavations
confirmed imported suitable fill throughout the length of the proposed trench.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: As can be seen on the attached topography drawing, the average
topography around the proposed drainfield is 0-15% running southeast to northwest.
5. WAIVERS: We are requesting that your department issue a variance from the
required separation distance of 100 feet from the proposed drainfield to the existing well
down to 50 feet. We are also requesting a variance from 100 feet down to 90 feet from
existing well to proposed septic tank and AX-20 pod. Justifications for granting of these
waivers are as follows:
• We will be using CAT III advanced treatment.
• The house is between the well and the proposed drainfield
• The well is uphill from the proposed drainfield. If effluent were to overflow from
the drainfield, it would not travel directly towards the well head.
• The proposed system will be remotely monitored.
• We are not aware of any naturally occurring or insitu soils, so the use of ERS is
necessary for a drainfield to be installed.
• Water samples were pulled on 8/15/2011 by B.Hippe for the 2011 COSA per
MOA records. The nitrate levels were listed as non-detectable.
We are unaware of any adverse impacts this installation or waivers would have on
adjacent wells or septic systems. If you have any questions, please contact us at 337-

To whom it may concern:
The existing 2 bedroom house is served by a private well and holding tank. The
homeowner would like to get off of the holding tank and install a septic system. We are
proposing to install a new 1500 gallon STEP tank, one (1) Advantex AX-20 Pod, a
secondary pump vault and a deep trench type drainfield. Currently, there are three wells
on property, one that serves the house (well #3 on page 2/2), and two that are "non-
active" wells (well #1 and #2). Per conversations with Rebecca Carroll on 9/18/2017 the
following is a list of options that must be taken for the "in-active" wells (well #1 and 2 on
site plan):
• The wells are to be converted to agricultural wells with functioning pumps
• The wells are to be capped with a "welded cap" by a certified well driller
• The well are to be decommissioned per MOA Onsite Code
Per the MOA, any of the previously listed options are viable. However, well #2 must be
decommissioned if the homeowners do not wish to move the animal containment area
(minimum separation distance is 50 feet per MOA). Comments regarding the design are
summarized as follows:
1. SOILS: See the attached 2017 soil logs which show the soil classifications,
groundwater monitoring, and the percolation test results. As can be seen on the
attached site plan, there have been numerous testholes excavated on the subject
property, none of which indicated native material. We are proposing to install the
drainfield around G.E.G. testhole #3. Prior to the design submittal, GEG employee
(David Garness) met with MOA employees Deb Wokenfuss and Rebecca Carroll to
discuss the testholes that were performed by Cindy Ellis, P.E., Tobben Spurkland, P.E.
and GEG. The only suitable soils (imported fill) were found in the vicinity of GEG test
hole #3. It was agreed that a drainfeild would be permitted if consistent soils were found
within 5 feet of the proposed drainfield area (around GEG TH#3). Per the directive of

Rebecca Carroll, on 8/21/2017 confirmation testholes were to be excavated to a
minimum of the total depth of the proposed system (11 feet). Garness Engineering and
Wilco Contractors met onsite on 8/21/2017 and excavated several "exploratory"
testholes to a depth of 12 feet. Testholes were excavated 25 feet south and 10 feet
north of GEG TH#3 to meet the requirement set forth by the MOA. These excavations
confirmed imported suitable fill throughout the length of the proposed trench.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: As can be seen on the attached topography drawing, the average
topography around the proposed drainfield is 0-15% running southeast to northwest.
5. WAIVERS: We are requesting that your department issue a variance from the
required separation distance of 100 feet from the proposed drainfield to the existing well
down to 50 feet. We are also requesting a variance from 100 feet down to 90 feet from
existing well to proposed septic tank and AX-20 pod. Justifications for granting of these
waivers are as follows:
• We will be using CAT III advanced treatment.
• The house is between the well and the proposed drainfield
• The well is uphill from the proposed drainfield. If effluent were to overflow from
the drainfield, it would not travel directly towards the well head.
• The proposed system will be remotely monitored.
• We are not aware of any naturally occurring or insitu soils, so the use of ERS is
necessary for a drainfield to be installed.
• Water samples were pulled on 8/15/2011 by B.Hippe for the 2011 COSA per
MOA records. The nitrate levels were listed as non-detectable.
We are unaware of any adverse impacts this installation or waivers would have on

Alteration. Any change to the design or function of an AWWTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On-Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
Permit. An On-Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA,and shall continue while the AWWTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations, Installation and Removal of Additional Equipment. Prior to
performing any alterations to an AWWTS, the owner agrees to obtain an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AWWTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AWWTS
service and maintenance provider approved by the municipality and the
manufacturer of the AWWTS for the entire term of the AWWTS. 1 n addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof,at the owner's sole expense,to pay
for any and all: (1) repair(s), (2) maintenance, (3)adjustment(s), (4)replacement
costs,and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality,the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On-Site Systems Approval.
H. Owner agrees that the AWWTS installation and maintenance requirements as
provided by the AWWTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AWWTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof,or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

C. Any attempt to amend, modify,or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severability. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

application were completed by Watkins Engineering of Anchorage. We have reviewed
the design and permit application and have verified the site conditions are the same as
presented in the design. We are in agreement the design as submitted meets the
requirements of the house and is in compliance with Municipal Code. We therefore

in 2007.
If the tank is constructed in accordance with the submitted design the following
statements apply:

Septic System Upgrade: Holding Tank
To Whom It May Concern:
Attached please find the application and supporting documentation to install a
4000 gallon holding tank for the referenced 2 bedroom house. The existing septic
system has failed and there is no suitable location for a soil absorption system on
the lot.
The house was built in the 1960s, and there are no records on the existing septic
system. The system was partially excavated on October 16, 2006, and found to
be in failure and consisting of substandard construction. The 1000 gallon steel
septic tank had numerous holes in it. The drainfield consisted of perforated pipe
overlaying native gravels, with no filter fabric protecting the gravel. The water level
in the drainfield was above the pipe.
An asbuilt survey was done on October 23, 2006 by a registered land surveyor.
This showed the septic tank to be partially over the east lot line. The portion over
the lot line will be removed and buried on site.
Three test holes were dug on October 16, 2006, and none were acceptable for a
soil absorption system. The first hole found peat over blue clay, with water on top
of the blue clay at 4 ft. The second and third holes found only fill to 12 ft. Six test
holes were dug in 1995 and did not find acceptable soils. A permit was issued for
a holding tank installation in 1995, but the tank was never installed. A previous
owner of the property informed us that the entire area was filled with construction
debris and also a few old cars. Therefore, a holding tank is the only viable option
for wastewater disposal for this property. The owner understands that a 4000

gallon tank is larger than the minimum requirement of 2000 gallons for a 2
bedroom house.
I am not aware of any adverse effect that this upgrade would have on any adjacent
properties' water and wastewater treatment needs.

Gentlemen;
Per your request we excavated three additional testhole on this lot on March 21, 1995. The excavation was performed
by "Cbuck's Backhoe".
The enclosed "Soil Logs" describe the conditions found. The approximate location of the testholes are shown on the
attached siteplan.
These testholes confirm my original opinion that a conventional septic system can not be installed on this lot. If you
would allow an absorption trench to be installed in the fill material, the new owner would not object to that. May be
a 2000 gal. Septic tank with a bighwater alann and a drainfield in the peat. Otherwise a holding tank is the only
solution.

Your application for a holding tank permit for the subject lot has been denied. This office
does not feel that all the possible avenues to construct an on-site wastewater disposal
system have been considered. The Anchorage Municipal Code 15.65.070, A, 1.,2.,3 does
not allow for the construction of a holding tank until an engineer has conclusively shown
this office that an on-site wastewater disposal system cannot be constructed.
According to our records, the soil conditions beneath the fill Ol1 this lot are capable of
receiving and treating domestic wastewater. Also, the i[mnediate adjoining lot (lot 5B)
has a functioning on-site wastewater disposal system and the conditions that exist on this
lot are the same as the sub, jeer lot.

I am applying for a holding tank permit for this property.
We excavated three testhole on the property on February 21, 1995. The approximate locations are shown on the
attached siteplan. We found shnilar conditions at all three locations. With the possible exception of the house location,
it seems that the whole lot is overlaid with fill material to depths exceeding 14 feet. We found peat, brush, and
construction debris at all three locations. The testholes varied in depth fi'om 10 feet to 14 feet.

The water analysis on Lot 5, Seaview Subdivision, well is
satisfactory. The construction of the well meets State and
Borough codes.
The on-site sewage system was inspected and the protective
distances for well protection were adequate. The distance of
the seepage pit to the neighboring lake is questionable as
the sheerness of the terrain makes accurate measurement impossible.
We therefore would grant a temporary approval with the under-
standing that any test run on the sewage disposal system (such
as dye or bacteriological) showing indication of contaminating
the lake will result in this Department disapproving the system.
Upon disapproval, a holding tank installation would be mandatory.
If you agree to this arrangement, please sign this letter and
return it to this office, at which time I will be glad to grant

Approval (COSA) based only upon the representations given in paragraPh 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up' to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Certificate of On-Site Systems Approval # 111436
A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 2, Lot 5A
of Seaview Heights Subdivision. This inspection revealed an arsenic
concentration of 13 micrograms per liter (ug/L) for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On-Site Water and Wastewater Program
website (www.muni.org/onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay, that portion of
Chugach's total costs for such modification which is reasonably attributable to accommodating or
preserving thc Encroachment. If such payment is not m~ie within thirty O0) days of written demand
for same, Chultach may terminate this permit in accordance with paragraph 9, below.

additional and extraordinary costs incurred during any future required construction, repair
