Filter Pod to Well Encroachment Waiver
Dear Onsite Services Engineer:
The existing AX -20 Filter Pod serving the Advantex Treatment System on Lot 1, Block
1, Valhalla Subdivision was apparently relocated at some time in the past after it was
found floating in the groundwater. Unfortunately it was relocated only 86' from the well
on Lot 1 and 88' from the well on Lot 2. A waiver is therefore required to allow the pod
to remain in its current location.
Advantex Treatment Systems have been proven to reduce the nitrate content in the
septic effluent prior to discharge into the surrounding environment. A recent water
sample taken from water in the well indicates only 3.27 milligrams per liter of
nitrate/nitrite content. No contamination of the well can be attributed to the current
location of the pod.
Underlying soils on both lots are dense tight silt (ML) with extremely slow percolation
rates. Recent tests indicate impermeable layers of clay are present between the
aquifer and the surface soil layers. The well on Lot 1 is 58' deep with casing the entire
depth. The aquifer is at 47' below the surface. The well on Lot 2 is also 58' deep.
We are confident based on the long term performance of the existing septic system and
the water quality from the well that contamination from the existing filter pod is not likely.
We therefore recommend that the waiver allowing the pod to remain in its current
location at 86' from the well on Lot 1 and 88' from the well on Lot 2 be granted.

Dear On Site Services Engineer:
The only available area for a septic system upgrade on Lot 1, Block 1, Valhalla
Subdivision is within 6' of the north lot line. A waiver is therefore necessary to construct
the absorption bed at this location. This system has an Advantex Treatment System
and placing the new bed within 6' of the north lot line will not present conflicts to the lot
to the north. A lot line waiver has been in existence for the previous bed for several
years. We therefore recommend a waiver be issued allowing the bed to be within 6' of
the north lot line.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer
The owner of Lot 1, Block 1, Valhalla Subdivision intends to reconstruct the existing
absorption bed to insure a 4' separation from groundwater on a year round basis. The
bed currently is influenced by runoff during the spring and fall of the year. Drainage
patterns in the area were modified drastically with the upgrade of Huffman Road
several years ago. We are therefore requesting a permit be issued for a septic system
upgrade to reconstruct the bed in its current location. The existing Advantex Treatment
System will remain in use and be connected to the new bed. The location of the
absorption bed is shown on the attached Site Plan. Drainage patterns are also shown
on the plan and will be modified somewhat as a result of the proposed construction.
The total height new bed will be approximately 4.5' above the existing ground.
The test holes placed on this lot revealed silt with some sand and gravel. The
percolation rate in the materials encountered was between 60 and 80 minutes per inch.
Groundwater was found at 3' below the surface at its highest elevation. Our design
provides for the bottom of the absorption bed to be at lest 5' above the highest recorded
groundwater elevation. The absorption bed will be 2 - 35' long x 13' wide beds for a
total of 900 square feet of absorption area. The distribution line in the beds will be
placed at 2' above the existing ground. 2' of fill and 2" of insulation will be placed
above the beds to meet Municipal Code and prevent freezing. The side slopes of the
mound will be at 3:1 at the north and south ends and 3:1 on the east and west sides.
Landscaping and fill will be required to insure positive drainage away from the finished
mound. The existing bed will remain in place as no organic mat is present and it is dry
for much of the year. All organic material above the bed and approximately 2' of cover
over the bed will be removed and replaced with coarse sand and gravel.
The ground surface of the lot slopes to the west at a slight grade. The bed will be

constructed parallel with the contours of the slope as much as possible. All
components of the new septic system will be constructed a minimum of 100' from any
surface water and 100' from the well proposed for this lot. No conflicts exist between
the proposed well and septic system and those in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

Septic System As -Built
Certificate of Health Authority Approval
In early November we checked the water level in Test Hole No. 4 and found it to be 9'
below the surface. The sub drain placed earlier in the summer has been very effective
in lowering the ground water level on the lot. I then called Dan Roth and advised him
of the water level and requested that we be allowed to lower the absorption bed to 3.5'
below the ground surface. This elevation provides a minimum of 5.5' separation
distance between the bottom of the bed and the water level. Dan agreed this would be
acceptable and the bed was placed at that level.
The configuration of the bed was changed to 20' wide by 45' long to provide a minimum
of 900 square feet of absorption area. The design called for 30' wide by 30' long. The
configuration was changed because the developer was required to change the location
of the driveway to satisfy Building Safety. During construction the bed was widened to
22'.

Dear On Site Services Engineer:
The underlying soils on Lot 1, Block 1, Valhalla Subdivision percolate at rates in excess
of 80 minutes per inch. The absorption bed must be designed to absorb only .5 gallons
of water per day. The only area on the lot available for the bed encroaches to within 4'
of the lot line to the north. We are therefore requesting a waiver be issued allowing the
bed to be constructed at this location.
All required separation distances can be met between the new bed and all wells and
septic systems in the area. We therefore recommend a waiver be issued allowing the
new bed to be located 4' from the north property line.

Septic System Design and Permit. Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 1, Block 1, Valhalla Subdivision intends to construct a new three-
bedroom home on the lot. We are therefore requesting a permit be issued for the
construction of a new septic system to serve the home. The attached Site Plan and
backup documentation identify the location and configuration of the new septic system
and alternate site and the parameters used in the design. Also identified on the plans
are the location of the existing well on the lot, test holes and the wells and septic
systems on the surrounding lots. Drainage arrows and spot elevations are shown
indicating the current drainage patterns. The drainage patterns will be maintained after
construction.
Substantial soils exploration was completed on this lot last year. A well permit was
issued for the lot at that time and the well was subsequently placed on the lot. We have
placed additional test holes on the lot this year and monitored the water level over the
past three months. Recently a sub drain was placed along the eastern lot line and
extended along the southern lot line to daylight. Ditching will be developed along the
western property line when the lot is further developed to direct the drainage to a
culvert crossing Elmore Street. It appears the ground water level has dropped between
2' and 3' since the placement of the sub drain. Most of the organic and original ground
layer has been removed in the area of the proposed absorption bed and the water level
was recently noted at 3' to 5' below the surface.
All of the test holes placed on this and the adjacent Lot 2 indicate tight silty soils with
shallow groundwater. The percolation rate varies between 60 and 80 minutes per inch
with the water table now at 3' to 5' below the surface. We are proposing to place a
1,500 -gallon Advantex Treatment System with an add on lift station on the lot. Effluent
will be pumped to a 20' wide by 24' long raised mound at the northeast corner of the lot
for disposal. The bottom of the absorption bed will be constructed 2' above original
ground to insure 4' separation from groundwater. This system will be nearly identical to
the system placed last year on the adjacent lot, but slightly larger and higher above
Lot 1, Block 1, Valhalla

existing ground.
Test Hole No. 4, placed last year is nearest the location of the proposed absorption
bed. This hole is now showing 3' of unsaturated soil after placement of the sub drain.
The percolation rate of the underlying material was determined at 80 minutes per inch.
We have designed the absorption bed based on this data for an application rate of 1
gallon per day per square foot. Coarse sand will be utilized to level the original ground
and raise the bed to provide 4' of separation from the groundwater level.
The ground surface on the lot slopes as shown on the site plan from east to west at
gradual slopes. The absorption bed will be constructed at parallel with the slope as
much at the front of the property in conformance with Municipal requirements. All
components of the new septic system will be constructed a minimum of 100' from any
surface water and 100' from the well on this and adjacent lots.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.,
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 1, BLOCK 1, VALHALLA SUBDIVISION
GENERAL:
1. The scope of this project includes the procurement and placement of a
new 1,500 Gallon Advantex Treatment System with an AX -20 -Mode
la 40x94 Pod to serve a new three-bedroom home. System will be
placed at the location shown on the Site Plan. The new septic system
also includes the construction of a new 24' long X 20' wide absorption
bed at the location also shown on the Site Plan and at the
configuration shown. The distribution line in the bed will be placed at
2.5' above the existing ground surface. Clean coarse sand meeting
the gradation specified below must be placed atop the existing ground
to level the bed. The new system must be placed a minimum of 100'
from the existing well on the lot and those on adjacent' lots and 100'
from the sub drain outlet located near the southwest corner of the lot.
2. Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
The Advantex Treatment System must be installed in conformance
with specifications and regulations furnished by the manufacturer,
Anchorage Tank and Orenco Systems Incorporated. No variances
from the manufacturer's specifications will be allowed without
permission from the manufacturer and Municipal representatives.
2. A septic tank. is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.

5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.
6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the bed shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
6. Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).

4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Fernco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
SAND GRADATION:
1. Sand used to level the new absorption bed must have the following
gradation:
50% Passed the No. 4 Sieve
2% Passed the No. 100 Sieve
I% Passed the No. 200 Sieve
2. The sand source must be approved prior to use.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as -built of the completed system.

5. Amendment. This Agreement shall only be amended by authorized repre;:cmatives of r

6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall iYc
brought in the Superior Court for the Third Judicial District of the State of Alaslai at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Anyprovisions of this Agreement :z.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a .single family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days.from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results less than 30 days old. (Certificates may be reissued for a period 'of up to, i one year with
valid water samples.) Certificates are valid for one year for properties served by Class A or B wells or public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.
