Subject: Upgrade Sewer Permit —Muirwood Park Block 4, Lot 3
Gentlemen:
The owner has requested we proceed forward to obtain a septic permit to
upgrade the failed leachfield on the subject lot. The general slope of this lot
is from east to west at a grade of approximately 3-8%.
From the Municipality of Anchorage (MOA) records, the 5 -bedroom system
was installed in July 2004. The system consists of a 1500 -gallon septic tank
and an absorption trench that is 64' long x 5' wide x 4' deep (effective
sewer rock). During our inspection it was discovered that the absorption
field monitoring tubes had 4.5' of standing fluid.
Afler review of the MOA documents, it appears that the absorption trench
was installed in close proximity to the soils test (TH-10). This testhole
consisted of silty sand with a percolation rate of greater than 60 minutes an
inch. However, the other two testholes (TH-20 & TH-11) showed sandy
soils with excellent percolating rates.
The existing system was designed and permitted from MOA records TH-20
& TH-I1 soils data. Prior to construction an additional testhole in the
central portion of the reserve field will be conducted to re -verify soils. With
this permit submittal, we propose to install the reserve field per permit
SW040089. See attached redlined design.

The property and adjacent lots are served by public water. There is no
surface water within 100' of the proposed system and there are no known
curtain drains within 50'. We do not expect there to be any adverse effect on
adjacent lots by the development of this system. If you have any questions,

CONSTRUCTION PR ACTICES AND MATERIAL SPECIFICATIONS

GENERAL:
1. The scope of this proje ct includes the installation of a septic tank and/or
drainfield to serve a si itgle-family residence on the referenced property as
delineated on the attache d Municipality of Anchorage (MOA) permit and site
plan. Construction shall be in accordance with the approved site plan and
design drawings; MOA pe' -mit with any special provisions or conditions, and all
applicable State and Munic ipal Wastewater Disposal Regulations.
2. ArcTerra Engineering & Sc .rveying, Inc. (ArcTerra) assumes no responsibility of
the accuracy of the as -t uilt survey provided by the owner or owner's
representative.
3. The owner or contractor is responsible to notify the engineer of any
discrepancies noted on the d esign plans or drawings prior to commencing work.
These discrepancies could be, . but not limited to, possible encroachments, surface
water, varying soils, unplat.'.ed easements, types or use of dwellings on or
adjacent to property, nearby p -iblic wells, etc.
4. The contractor shall be respi msible for locating any necessary underground
utilities, easements, property lines and surface water. The owner and/or
contractor shall ensure, if deet ned appropriate, that a registered land surveyor
prior to construction flags all s >tbacks. The contractor shall be responsible for
verifying all pertinent separatiot distances and component layout.
5. Unless specifically agreed othen rise, the property owner shall be responsible for
final grading areas subsequentl3 depressed from soil settling. On all leachfield
mound systems, the property owner shall be responsible for ensuring a
satisfactory vegetation growth ow -r the mounded area.
6. The MOA Development Service : Department On -Site Water & Wastewater
Program must certify contracto •s installing wastewater disposal systems.
Owners installing their own systet .is must also receive prior approval from said
MOA Department.
SEPTIC TANK/ HOLDING TANK/ LIFT STATION INSTALLATION:
1. A septic tank is to be constructec 1 by a certified septic tank manufacturer.
Construction shall include two 4" ele anouts for pumping access.
2. The septic tank shall be sufficie. ntly bedded with proper material and
compacting to prevent damaging, sett ling or shifting of the tank.
3. All standpipes on the septic tank shall I extend a minimum of 12 inches above the
final grade.

4. Septic tanks installed with less than 4 foot of cover shall be insulated. The
contractor will ensure that the fill over the tank does not exceed the burial depth
rating.
5. A foundation cleanout shall be installed one to four feet from the building
foundation. In the line between the tank and the leachfield there shall be two
adjacent cleanouts (unless an effluent pumping system exists within the septic
tank). These cleanouts shall be located on undisturbed soil not more than 10 feet
from the tank. The second cleanout, in line, shall be to clean toward the septic
tank.
6. Final grading over the septic tank shall be such that a positive slope exists away
from the septic tank.
7. All H.D.P.E. tanks shall have a vertical length of 4 -inch perforated drainpipe
besides the tank extending to the bottom of the tank to above grade to monitor
ground water.
8. All tanks shall be anchored when there is not enough adequate soil cover to
resist buoyancy forces in high -groundwater areas.
9. All holding tanks and lift stations shall have an audible and visual alarm inside
the residence.
10. All lift stations shall be wired in compliance with the Uniform Electrical Code
and all MOA guidelines.
11. The contractor shall remove the old tank, or abandon it in compliance with the
Uniform Plumbing Code. The contractor is solely responsible for the
abandonment procedure.
ABSORPTION TRENCII/DRAINFIELD INSTALLATION:
1. Excavate the proposed trench to the dimensions shown on the design. The
bottom of the excavation shall be within 2 inches of level. If the sidewalls of the
excavaction become smeared, they must be raked or scratched (roughed -up)
before gravel (sewer rock) placement.
2. Once the sewer rock is installed, the distribution pipe is installed level with the
perforations facing downward. Gravel is then to be placed over the distribution
pipe to provide a minimum of 2 inches of cover over the pipe.
3. A silt barrier must be installed between the final gravel layer and the native soil
backfill. Ensure the silt barrier covers the entire gravel surface before placing
backfill.
4. Monitoring tubes shall be four- (4) inch diameter, installed approximately in the
locations shown on the design, and extend a minimum of 12 inches above final
grade. The portion of the monitoring tube extending through the gravel shall be
perforated from the bottom of the trench to the invert of the distribution pipe.
This is equivalent to the effective depth of the gravel as noted on the design.
5. Backfill over the final gravel layer must not be less than twenty-four inches.
Insulation must be installed when the backfill depth is less than thirty-six (36)
inches. The finish grade over the trench must be mounded to prevent the
formation of a depression after settling.
6. All advanced technology waste water disposal systems (Advantex, Biocycle,
Intermittent Dosing Sand Filter, etc...) shall be constructed, installed, maintained
and monitored per manufacturer's specifications and MOA requirements.

MINIMUM MATERIAL SPECIFICATIONS:
1. A MOA approved septic tank manufacturer must construct any septic tank
proposed for installation.
2. The following pipe materials are MOA approved for use in septic system

4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder,
Fernco, or equal).
5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be
installed between the final leachfield gravel layer and the native soil backfill.
6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less
than 3% passing the #200 sieve. If deemed necessary, a sieve analysis should be
performed.
7. When sand is being used as a filter material, its gradation specification must
conform to current MOA or ADEC requirements.
INSPECTIONS:
Typically there will be three (3) inspections required during the installation of the
wastewater disposal system. These inspections will occur as follows:
1. The first inspection must be conducted after the excavation of ditches, pits,
trenches, or beds and before the installation of any gravel. A septic tank may be
set in place, but may not be backfilled before this inspection.
2. The second inspection must be conducted after the placement of the sewer rock,
distribution lines, standpipes, and cleanouts, but before the placement of the silt
barrier, insulation, or any other backfill.
3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required, especially with the
installation of multiple trenches, sand filters, pressurized distribution systems, etc. The
inspecting engineer is to be contacted at least 24 hours prior to the start of construction.
If necessary, a pre -construction meeting will take place on-site. The inspecting engineer
will not coordinate, direct or control in any way the contractor's activities.
The owner shall contract with the contractor to perform the work outlined in these
specifications and attached plans and in accordance with the attached MOA permit. All
materials, components, and construction practices shall comply with the MCA's and the
State of Alaska's latest ordinances, regulations, or policies regarding wells and septic
systems. The failure of the inspecting engineer to notice a material or construction non-
compliance does not relieve the contractor of his/her responsibility to correct such
deficiency. There will be no contractual arrangement existing between the contractor

and ArcTerra. ArcTerra shall be the owner's representative and will inspect the work as
stated above to document the contractor's activities. Final acceptance of the contractor's
work rests with the owner and the MOA.
ArcTerra shall have no liability to the owner or to others for acts or omissions of the
contractor or any other persons performing work on this project or the failure of the
contractor to carry out the work in accordance with these construction documents.
ArcTerra's inspecting engineer will not be responsible for the construction means,
methods, and techniques, sequence, procedures or the safety precautions incident to this
project.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 3, Block 4, Muirwood Park Subdivision intends to construct a five -
bedroom home on the lot. We are therefore requesting a permit be issued for the
construction of a new septic system to serve the new home. The subdivision is served
by the Municipal water system and no conflicts exist with adjacent septic systems or
wells. The attached Site Plan and backup documentation identify the location and
configuration of the proposed septic system and alternate site. Existing spot elevations
and the current drainage pattern are identified on the Site Plan. The drainage pattern
will be maintained after development of this lot is complete.
Test holes placed on the lot during the subdivision phase indicated silty sand with some
gravel in the area of the absorption trenches. The accepting soils were tested and
found to have percolation rates of less than 1 minute per inch. No groundwater was
found during excavation and none was noted during the monitoring period. Our design
includes the placement of a new 1,500 -gallon septic tank along with a 63' long by 5'
wide by 4' effective depth absorption trenches to collect and treat the sewage
generated in the house. The bottom of the trench will be 6' below the surface. The
distribution pipe will be 2' below the surface. A minimum of 3' of cover will be placed
atop the trenches and a minimum of 4' of cover will be placed atop the new septic tank.
The ground surface on the lot slopes at shallow grades from west to east as shown on
the Site Plan. The absorption trench will be constructed parallel with the slope of the
ground surface as much as possible in conformance with Municipal requirements. The
new septic system will be constructed a minimum of 10' from the water service
proposed for the lot and on adjacent lots. It will also be constructed a minimum of 100'

from any surface water in the area
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 3, BLOCK 4, MUIRWOOD PARK SUBDIVISION
GENERAL:
The scope of this project includes the procurement and placement of a new
1,500 -gallon septic tank at the location shown on the Site Plan. Work also
includes the construction of a new 63' long X 5' wide X 4' effective depth
absorption trench at the location also shown on the Site Plan. The
distribution line in the trench will be placed at 2' below the existing ground
surface. Total depth of the trench will be 6' below the existing ground
surface. The new septic tank and absorption trench must be placed a
minimum of 10' from the water service on this and adjacent lots. In addition,
a separation distance of 100' must be maintained between the septic tank
and absorption trench and any surface water.
Construction shall be in accordance with the approved site plan, design
drawings, Municipal Permit with any special provisions or conditions, and all
applicable State and Municipal Wastewater Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the location
of all lot lines. The Contractor must also verify the location of wells in the
area and assure that the separation distances are met.
4. Unless specifically agreed otherwise, the contractor shall be responsible for
final grading areas subsequently depressed from soil settling. Property
owner shall be responsible for , revegetation of affected areas unless
specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified by the
Municipal Department of Health and Human Services for system installations.
Owners installing their own systems must receive prior approval from
D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. A new 1,500 -gallon septic tank must be procured from an approved source
and installed at the location shown.
2. A septic tank is to be constructed by a certified septic tank manufacturer.
Construction shall include two 4" cleanouts for pumping access.
3. The septic tank shall be sufficiently bedded to prevent settling or shifting of
the tank.

4. All standpipes on the septic tank shall extend a minimum of 12 inches above
final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial
insulation.
6. A foundation cleanout shall be installed one to four feet from the building
foundation. Two cleanouts are required between the tank and the drainfield.
7. Final grading over the tank shall be such that a positive slope exists away
from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the design.
The bottom of the trench shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a level
bed of drainfield rock. Rock should then be placed over the pipe to provide a
minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield rock
and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations shown on
the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield area minimum 100' away
from any private water wells in the area, 150' from a Class "C" Well or 200'
from any community well.
6. Direct bury insulation must be placed over the distribution system if less than
3' of backfill depth is available. Finish grade over the trench must be
mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for a 5'
wide trench. Contractor shall verify this condition prior to placement of the
rock. All pockets of unacceptable materials must be removed and replaced.

MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a Municipally approved septic tank
manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and
solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662
or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow
Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the
#200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These inspections
must be conducted under the supervision of a professional engineer registered
in the State of Alaska. The first inspection must be conducted after the
excavation of trenches, beds or pits and before the installation of any gravel. A
septic tank may be set in place, but may not be backfilled.
The second inspection must be conducted after the placement of the geotextile
fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No
backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction notes
for use in preparing the certified as -built of the completed system.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system in
accordance with the guidelines and regulations established by the Municipality of Anchorage and industry
practices. The reported results describe the condition of the system/s on the date/s of the evaluation.
Separation distances were measured to readily identifiable features. Hidden defects or encroachments may
exist that were not identified during the evaluation. The operational life of all wells and septic systems depend
upon a variety of variables, including (but not limited to) soil conditions, groundwater levels (that may fluctuate
during the year), quality of construction (materials and workmanship), and the water usage of the family utilizing
the system/s. These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the systems; therefore, GEG makes no warranty (express or implied) regarding
