We are therefore requesting a permit be issued for the relocation of the existing absorption bed. It will also be increased in size to accommodate a six bedroom home. The attached Site Plan and backup documentation identify the location and configuration of the existing well and the proposed septic system on the lot. The existing absorption bed location will become the alternate site. Also identified on the plans are the existing wells on the lots surrounding this lot and the wetlands boundary. No conflicts exist between the proposed septic system and well or septic systems and wells on adjacent lots. Drainage arrows and ground surface contours are shown on the site plan showing the grade and direction of flow on the lot. The drainage pattern will not be changed as a result of this project. The test hole placed on the lot in the area of the absorption bed indicated silty gravel with sand (GM/GW). The percolation rate of the soil was determined to be 15 minutes per inch. Groundwater was not found but is assumed to be much closer to the surface during the spring. Groundwater monitoring from other test holes on the lot inidicate a depth of 5' below the surface may be more representative. An application rate of 2 gallons per day per square foot was used in the design of the absorption system. We are proposing to utilize the existing 1,500 -gallon STEP tank and construct a new 10' wide by 45' long absorption bed. The elevation of the distribution line in the absorption bed will be 2.5' above the existing ground surface. A minimum of 3' of fill or 2" of insulation and 2' of fill will be placed over the absorption trench and 4' over the septic tank. The existing Quanics Aerocell Unit will be reconstructed in the new absorption Tract A, Glacier View Heights Subdivision No. 4 August 5, 2013 Page Two bed. The ground surface on the lot in the area of the absorption bed slopes to the southwest at a shallow grade with some cross sloping to the north. The new bed will be constructed parallel with the slope as much as possible in conformance with Municipal requirements. The new septic system will be constructed a minimum of 100' from all wells and any surface water and 10' the proposed and existing water services in the area. If the system is constructed in accordance with our design the following statements apply: The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. The system, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. 3. The system, if constructed as designed, will have no adverse impact on reserve space, either surface or subsurface, on any lots located in the area. 4. Reference: Glacier View Heights Subdivision #4; Tract A - Septic System Evaluation and Upgrade Proposal for a Septic System Serving a Single Family Residence within the Municipality of Anchorage (MOA). Mr. Crewdson, The existing septic system does not meet MOA requirement and must be upgraded for the sale of the house. Per the request of the selling agent, we inspected the septic system which serves the referenced property for the purpose of obtaining an MOA Certificate of Onsite Systems Approval (COSA). Site visits were performed on 5/16/2012, 5/18/2012, 5/25/2012, and 5/29/2012. Our findings are summarized as follows: Prior to our site visit, we researched MOA records. Based upon our findings, the septic system had an approved permit based upon an engineer's design; however, no documentation of the installation could be found. It is our understanding that the engineer who documented the installation, Steve Eng, P.E. was unwilling to submit documentation of the installation to the MOA. Upon our initial inspection (5/16/2012), the septic tank and drainfield appeared to be installed near the location shown on the design; however, based upon review of the design, and a preliminary inspection report obtained by the buyer from the Mr. Eng, the drainfield should have been excavated to a depth of 1 foot and then filled with at least 2 feet of MOA approved sand filter, followed by 1 foot of drainrock. Given this information there should be at least a 2 to 3 foot mound over the drainfield area, which is not the case. Two (2) monitoring tubes were found in the drainfield and the liquid levels were checked. The west monitoring tube (MT1) was found to have 10 inches of liquid in it and the east monitoring tube (MT2) was found to be dry. Both MT1 and MT2 were found to have noncompliant pipe material and also have separation below the ground surface. It is our opinion that MT2 may not be a functional monitoring tube. It should be noted, that the house had been vacant for a long period of time prior to our initial site visit. Based upon this initial site visit and the information collected, it appeared that the drainfield was in a state of failure or encroaching (in) groundwater. We would also like to note that we 3701 E. In order to confirm our finding, we performed a second site visit on 5/18/2012 to perform elevation shots on the entire septic system. Elevation shots were taken and it was determined that the septic tank was within 2 inches of being level. The elevation of the monitoring tubes in the drainfield were also found to be within 2 inches of being level with MT2 being higher in elevation than MT1. MTI still had 10 inches of liquid in it. It is our suspicion that MT2 may not be functional. Based upon the elevation shots taken, it was our opinion that the drainfield was not installed per MOA code and that it may be encroaching upon groundwater. We contacted the selling agent on 5/22/2012 about our findings and proposed excavating test holes for the purpose of monitoring groundwater. He authorized us to have the test holes excavated and on 5/25/2012, we had an excavator dig a test hole above, and below, the drainfield to a depth of 10 feet. Groundwater monitoring tubes were installed in each test hole and backfilled. On 5/29/2012, we returned to the property and found groundwater to be at 20 inches below grade in the uphill test hole (TH#2) and at 46 inches below grade in the downhill test hole (TH#1). Based upon the elevation of the groundwater levels, it is certain that the drainfield is installed in groundwater and that the liquid level in MT1 is actually groundwater. The following is a list of code violation and deficiencies • The drainfield is encroaching upon groundwater. MOA code requires a 4 foot separation from the bottom of the drainfield to the highest groundwater level. • The septic tank and drainfield are within the 100 foot setback of surface waters. The septic tank is 70 feet from running surface water west of the house and the drainfield is within 50 feet of the outfall of the curtain drain which also classified as a surface water. • There are no cleanouts in the drainfield. MOA requires there be cleanouts at the end of each lateral in the drainfield. • The type of pipe used for the standpipes in the drainfield does not meet MOA requirements. • The foundation cleanout is not within 5 feet of the foundation. • The double cleanouts after the septic tank are not sweeping towards each other. • The separation from the drainfield to the curtain drain could not be verified. The separation distance is required to be 20 feet on the uphill side of the drainfield and 50 feet on the downhill side. Given these code violations and deficiencies, it is our assessment that the septic system needs to be upgraded. On 5/30/2012, we met with you and based upon our discussion, you agreed that there was multiple code violations and dictated that an advanced wastewater system would be required. We proposed that a Category III Quanics Aerocell treatment system be used for this property and a new 10 foot wide by 30 foot long drainfield be installed over the existing drainfield area as far away from the surface waters as possible. We proposed to excavate down to the top of the existing septic system and install a sand filter in order to achieve separation to groundwater. We also proposed to submit for a waiver to surface waters from the proposed septic system and request a 3 foot separation distance from the new drainfield bottom to groundwater. You 3701 E. Comments regarding the design are summarized as follows: 1. SOILS: See the attached logs from Steve Eng, P.E. which are from the original approved design. The soil logs show the soil classifications, groundwater monitoring, and the percolation test results. 2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. 3. SURFACE WATERS AND WAIVER REQUEST: There are several surface waters within 100 feet of the existing septic system. As you can see on the attached site plan and design drawing, there is very limited room for a septic upgrade. We are proposing to maintain a 100 foot separation distance from the running water running along the west side of the property. It is unknown if this running water is seasonal; however it is believe to exist for more than 30 consecutive days in a year. The other surface water issue is the outfall of the curtain drain that is believed to be for the foundation. The exact route of the curtain drain is unknown. As part of the design, we are requiring that the contractor have the routing of the curtain drain documented in order to determine its location in relation to the existing drainfield. If the location is found within 20 feet of the existing east monitoring tube, a dirt block shall be installed to inhibit the flow of wastewater to towards the curtain drain. The outfall of the drain is currently running and It is unknown if this running water is seasonal; however it is believe to exist for more than 30 consecutive days in a year. We are requesting that you waive the distance from the proposed drainfield to the outfall of this drain to 75 feet. The following items are justification for this waiver: • The drainfield is in a highly visible area. If effluent were ever to overflow, it would be recognized quickly and corrected. The area between the drainfield and the surface water is highly vegetated. The actual path of travel to reach the surface water is likely over 100 feet. We are proposing that a Category III Quanics Aerocell treatment system be used for this installation. TOPOGRAPHY: As can be seen on the attached design drawing the topography around the drainfield is approximately 5 percent running north/northeast to south/southwest. The owner desired a 1500 gallon septic tank for possible future system expansion. This is a large 5 acre lot and no adverse impacts are expected from development. Down-slope of the site is uninhabited. A phreatic water table is present that is generally in the range of 5 to 7 feet below the surface of the ground. Monitoring the groundwater table after the record rainfall of 10/1/02 revealed a stabilized depth of 5 feet to groundwater which was used for design purposes. Water Well: 6 inch in diameter, surface grouted to 20 feet, ground sloped away from well, casing extending minimum of 18" above ground level, pitless adaptor at 10 foot depth, stainless steel well screen, all components/workmanship meeting State of Alaska Drinking Water Regulations. B. Certificate of On -Site Systems Approval. E. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an A W WTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 I Fee. Owner shall pay to Municipality an annual fee of 0 ($O .00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. Amendment. A. This Memorandum of Understanding shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this contract, the only authorized representatives of the parties are: --- Anchorage: Purchasing Officer C. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 9. Jarisdiekion: Choice of Law.
