If this was a well permit and the well has been drilled, a well log must be sent to this office for documentation of the installation and to close the permit. If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to this office for review, approval and documentation. This as -built inspection report must be signed by the licensed Professional Engineer who inspected the installation of the system. As -built inspection reports are required to be submitted within 30 days of the completion of the system. A 6"x6" dia hole was prepared, and filled 3 times to soak. Two perc tests were performed, and in each case a 6" collum of water perced in .25 and .23 min respectively. A perc rate of .04 min/inch was calculated: Test hole was then dug to a depth of 9' where a sand w/ trace of silt was encountered. A 6"x6" test hole was dug and soaked three times. Perc test commenced at 8:51pm and was repeated fora total of three tests. the 6" collumn perced in 6.6, 6.8 and 6.5 min. respectively, and perc rates were calculated at 1.11, 1.13 and 1.08 min/inch. System Design Based on the perc rate and the soils log data, it was determined that the native soils offer excellent wastewater treatment potential. Because of the impracticality of a deep trench system in such loose soils, a bed type system was designed incorporating 6" sewer rock bedding under pipe, the 5' intermediate "GP" as pre-treatment medium, and the stratum of sand wl trace silt as final treatment medium. System Sizing To adequately provide wastewater treatment for the proposed 3 Bedroom house, a 1000 gal steel tank was specified, leaching effluent into a 384 sq. ft.(32' x 12') leaching bed at a rate of 1.179pd This bed system is to be comprised of one splitter box, and two 30' runs of 4" perforated PVC pipe with cleanouts at end of runs and each way at tank exit. to preclude freezing, 2" polystyrene insulation board is to be installed where earth cover is less than 3' over pipe and 4' over tank.
