To whom it may concern:
Prior to the involvement of Gayness Engineering Group, Ltd. (GEG), this was a vacant piece of property.
GEG completed test holes and a design. A well and sewer permit was issued by the MOA on 7/25/02, and
the system was installed by the fall of 2002. We were waiting to be provided the well log, pump log, as -
built survey and for the final grades to be completed over the septic area before completing an as -built
package. Your department contacted us in 2013 asking about the status of the septic system as -built
drawings. To our surprise, we discovered that a house had been built, and that a certificate of occupancy
had been issued by the MOA for the property. We then requested an as -built survey from the property
owners. This spring we received an as -built survey, and discovered that the septic is sitting under what
appears to be a driving surface. Furthermore, the septic pipes are not exposed at grade or visible.
It should also be noted that, according to the as -built survey, the well was placed in a section line
easement, and the house appears to be sitting in a 10' telecom & electrical easement. If the final grade
elevations are to be established on the property, the septic system cleanouts and monitoring tubes will
need to be exposed and inspected for any possible damage. Attached are partial as -built drawings.
GEG will be available in the future to resolve issues associated with documentation of this septic system
on a hourly rate fee basis. It is our understanding that provisions are now in place at the building
department to require final approval of septic systems, prior to issuing a certificate of occupancy to avoid

Dear Terri White:
An On -Site Water/Wastewater Permit, number SW020245, issued by this office for a single-family
system, will expire on July 25, 2003. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400.00 for a wastewater permit and S 150.00 for a well
permit.

Ref: Proposed and Well Septic System Design for Lot 6, Block 1, Tower Estates Subdivision
To whom it may concern:
The proposed 3 bedroom house will be served by a private well and septic system. Two test
holes were excavated in the area of the proposed septic system. The septic system will be
designed around the 30 foot radii of these test holes. We are proposing that a 1000 gallon septic
tank and a five foot wide drainfield be installed. Comments regarding the design are summarized
as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that an application rate of 0.6 gallons/day/ftz
should be used.
2. TRENCH DESIGN:
a. Percolation Rate: 12.9 & 17.7 minutes/inch
b. Allowable Application Rate: 0.6 gallons/day/ft
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 750 ftZ

4. TOPOGRAPHY: As can be seen on the attached design, the average topography where the
proposed drainfield is to be installed is a 40f percent slope running from approximately north to
south. A plywood/visqueen barrier is to be installed around the perimeter of the trench to insure
that the effluent will be below the organic layer. Once below the organic layer, the effluent
should move downward rather rapidly with minimal lateral migration. Therefore, we are
proposing to fill below the trench so as to create a slope of less than a 25 percent for a distance of
35 feet downhill from the trench.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic
