4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions: SW020027 has not been closed out. This is still to be closed out. Assuming that a 5
bedroom septic field was constructed, then this new tank permit is also for a 5 bedroom system.

Subject: Septic Tank Replacement Design and Permit Application
Dear Plan Reviewer,
The owners of the above 5-bedroom home septic tank was found to be corroded and possibly leaking
and needs to be replaced.
SEPTIC TANK REQUIREMENTS
We propose to remove the existing tank after it has been pumped. The tank will be decommissioned
and buried onsite.
A new 1,500 plastic septic tank will be installed in roughly the same location as the old one adjusting
for the deck. The deck is less than thirty inches above the surrounding ground proposed is a five foot
separation
The tank shall be covered with a minimum of 2" MOA approved insulation and 3' of cover or a
minimum of 4' of cover without insulation.
DRAIN FIELD CONNECTION
We are proposing to connect the existing building service line to the new septic tank and to the
existing outlet line as determined in the field due to the lack of record drawings.
SURFACE WATERS
There is no surface water within 100 feet of the proposed tank. Drainage arrows on the site plan
indicate the slope and direction of stormwater runoff. (See attached site plan)
WELLS & DRINKING WATER
There are no water wells within 100 feet of the proposed tank. The property is served by a private
well. The Surrounding properties are served by Valli Vue public water.

We do not anticipate any adverse effects on the neighboring septic systems, reserve areas or drainage
patterns by the installation of the proposed septic tank replacement.
Upon completion of the installation, a record drawing will be submitted showing the location of the
new tank, leach field, well, and other applicable features.

Dear Rex Thomas:
An Ori -Site Water/Wastewater Permit, number SW020027, issued by this office for a single-family
system, will expire on February 25, 2003. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400.00 for a wastewater permit and $150.00 for a well
permit.

To Whom it may concern:
This is a request for a septic permit at the above referenced location. The lot is surrounded by the tree
streets, Valli View Subdivision, and is serviced by community water. However this lot has an existing
well, see plan, that will be tested and certified by Alpine Drilling in the spring. The test holes were very
consistent, with silty sand, SM, for the entire depth of 19 feet. The perc was 22 minutes per inch with
no water table observed during the monitoring period. A simple duel trench has been designed with 52
foot length each side of the test holes, typical. The creek is 100 feet away as the plan shows and will
not be a problem.
This new system will not affect the future development of the neighboring properties due to the location
of the septic systems and public water around the lot. The general drainage is to the north and west at
approximately five or ten percent but greater at the driveway area see plan
