Subject: The Tablelands, Block 8 Lot 7 – 22622 Sambar Loop
Septic Tank Replacement
Dear On-Site Services Engineer:
The owner of the above lot has a septic tank that has reached its end of useful life, so we are
submitting this permit application for its replacement. The attached site plan identifies the location
of the home as well as the existing trenches and new tank location. No conflicts exist between this
proposed system and any other wells or septic system, whether on this lot or adjacent lots. We are
replacing the septic tank with the same size designed for 4 bedrooms.
The new septic tank will be a minimum of 100’ from surface water. This property and the adjacent
properties are all served by a community water system, so there are no wells that could be affected
by this tank placement. Please refer to the attached plan for the design. If this design is followed,
there will be no adverse impacts to adjacent properties.

Request for Separation Distance Waiver for Onsite Disposal System'
AK Rim Engineering Ref. No. 04-00129
Dear Ms. Makela:
This letter is to request a waiver on the required 10 feet lot line setback for the onsite wastewater
disposal system on the above referenced lot, as per MOA Code 15.65.140. li Included with this
letter is the required fee of $175.
The nearest edge of the wastewater disposal field is approximately 6 feet from the lot line,
according to the as built survey (see attached). The property comers and portions of the lot
lines were staked prior to installation of the drainfield. However, stakes are often knocked down
or inadvertently removed. It can be seen on the as built drawing that the west lot line is not a
straight line from the northwest comer to the southwest comer and so the excavator appears to
have believed he was well inside the required setback.
There will be no negative impacts by this slight intrusion into the setback. The neighboring lot to
the west is owned by Troy Davis Homes. _.

The owner of the above referenced lot is proposing to construct a 4 -bedroom single
family dwelling (see attached Plot Plan). The proposed dwelling is to be served by a
community water system and a private septic system. With this letter we are requesting
plan review and a permit to install an onsite septic system to serve the proposed
development.
DESIGN SUMMARY
Soils
A test hole for ground water and percolation test was excavated on February 26, 2004,
and the soils logged by our firm. A monitor tube installed at that time. A percolation test
was conducted on March 3, 2004. The proposed drainfield will be designed within the
30 -feet radius of the test hole. The attached Soils Log — Percolation Test form shows the
approximate location of the test hole, labeled AK TH #1. The soils were rated in the field
as SP -SM, poorly graded sand with silt and gravel. The percolation test indicates a soil
percolation rate of 40 minutes per inch which translates to a wastewater application rate
of 0.45 gpd/sgft.
Groundwater was not encountered during the initial excavation of the test hole. On
3/3/04 and 4/1/04, the monitor tube was probed and no groundwater was detected.
On 4/2/04, a second test hole was dug for a percolation test. The test hole, labeled AK
TH #2, was dug to 5 feet where the perc test was conducted. The percolation rate was
60 minutes per inch which translates to a wastewater application rate of 0.45 gpd/sqft.

lot.
Surface Water
There are no surface waters within 100 feet of the proposed septic system.
Surrounding Properties
This property and the parent subdivision will be served by community water system. We
are unaware of any adverse impacts this installation would have on adjacent wells or
septic systems. Development on this lot will not preclude development of wells and
septic systems on neighboring lots in the future.
