The owner of the above referenced lot is proposing to construct a 4 -bedroom single
family dwelling (see attached Plot Plan). The proposed dwelling is to be served by a
community water system and a private septic system. With this letter we are requesting
plan review and a permit to install an onsite septic system to serve the proposed
development.
DESIGN SUMMARY
Soils
A test hole was excavated on February 25, 2004, and a monitor tube installed. A
percolation test was conducted in the same location on the property on March 3, 2004.
The proposed drainfield will be designed around the 30 -feet radius of the test hole. The
attached Soils Log — Percolation Test form shows the approximate location of the test
hole, labeled TH #1. The soils were rated in the field as SM, silty sand.The percolation
test indicates a soil percolation rate of 40 minutes per inch which translates to a
wastewater application rate of 0.45 gpd/sgft.
Groundwater was not encountered during the initial excavation of the test hole. On
3/3/04, the monitor tube was probed and groundwater was not detected; however,
groundwater was detected on the adjacent lot (Lot 6) at approximately 7 feet below
ground surface. The elevation of the ground surface at the location where the
groundwater was probed is approximately the same as the elevation as the proposed
drainfield for Lot 5.

We are requesting a design width of 18 feet rather than 15 feet due to space restrictions
on the lot. There is a section line easement along the north property line that reduces
the useable area. In order to allow room for the initial and replacement systems it was
necessary to reduce the length of the bed and increase the width.
Topography
The topography of this lot is such that it slopes down gradually from the south to the
north. The bed will be installed parallel to the surface contours. There is adequate
usable area for an onsite septic system and replacement system.
Surface Water
There are no surface waters within 100 feet of the proposed septic system.
Surrounding Properties
This property and the parent subdivision will be served by community water system. We
are unaware of any adverse impacts this installation would have on adjacent wells or
septic systems. Development on this lot will not preclude development of wells and
septic systems on neighboring lots in the future.

The ground cover over the tank was found to be 2" short of 4'. Also the cover over one end of the leach field was
noted to be a couple inches short ft was verified that water was running through the clean -outs and the leach field.

property since December of 2004. 1 have never had a problem with the septic system freezing,
neither the lank nor the leach field, the entire time that 1 have lived here. There have been times
when my wife and I have been away for extended periods of time during both the winter and
summer seasons and everything was normal,
I think it is pertinent to know that a number of homes adjacent to ours and throughout the
subdivision had their systems built by the same contractor, and being a close-knit neighborhood,
I can say without question that there has been no reports or discussions of problems or issues of
