Ref: Proposed Septic System for North Slope Subdivision #2; Lot 9B, Block 4,
To whom it may concern:
The 4 bedroom house is under construction and was previously permitted for a well and septic
system that was designed by S&S engineering. The well was drilled under this permit and the
owner has now retained G.E.G. to redesign the system using Advantex- technology. We are
proposing that a 1500 gallon Advantex Treatment System and a pressurized 5 -wide trench type
drainfield be installed in the vicinity of the S&S Engineering test holes. The drainfield will be
designed around the 30 foot radii of these test holes. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. We are proposing to use an average application rate of 1.25
gallons/day/ft2 should be used.
2. BED DESIGN:
a. Percolation Rate: 5.5 & 80 minutes/inch (soils vissualy rated to be 60 min./inch)
b. Proposed Application Rate: 1.25 gallons/day/R2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 480 ft2
f. Total Depth: 3 feet (maximum)
g. M.O.A. Approved Sand Filter: 2.0 feet
h. Effective Depth: 0.5 feet (minimum)
i. Width: 10 feet
j. Minimum Length: 50 feet long
k. Effective absorption area = 300 fiZ

inches on center(15 holes per lateral/30 holes total).
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached design, the average slope in the area is
where the proposed drainfield 5 to 10 percent slope running northeast to southwest.
Approximately 15 feet down hill from the proposed drainfield, the slope increases to a 15 to 20
percent. In short, there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

SEPTIC TANK / LIFT STATION /AIR COMPRESSOR
A. General
The work under this section consists of the performance of all operations pertaining to
furnishing and installing the septic tank/lift station, air compressor and piping to the sand
filter.
B. Materials
1. Tank
a. Septic tank shall be a two compartment tank of at least the minimum size
indicated on the drawings and MOA approved.
b. First compartment shall have a separate 4" pump -out riser. The second compartment
shall have a separate 4" pump -out riser.
2. Manhole Riser or Lift Station Tank
a. An additional second compartment manhole riser shall be capable of being equipped with
the following:
1. A junction (Nema 4x) box or equal, bonded or attached to
the pump basin.
2. UL listed electrical cord grips, installed in the J -box.
b. An additional tank (M.O.A. approved) may be provided for the
effluent pumping assembly and the items in (a.) above.
3. Effluent Pumping Assembly, Controls, Alarm & Piping
a. Effluent pumping assembly shall be of approved materials, including the following:
1. A 110 volt or equal U.L. Listed effluent pump.
2. High-density PVC cylinder housing the pump, with level
controls and screen, serving as a baffle to prevent the screen
from clogging.

3. All piping shall be PVC or other non -corroding material.
b. Pump controls and alarm system shall be an approved system with the following options
consisting of:
1. Event counter
2. Elapsed time meter
3. Program timer
4. 10 watt heater
5. Remote alarm panel option with a minimum of 80 DB
sound pressure at 24", operating temperature 30AC to
65AC, continuous sound to be located in the home.
6. Oil -tight visual alarm with push -to -silence feature.
Automatic audio -alarm reset.
7. 15 amp motor rated toggle switch, double pole, double
throw with three positions; manual (man), automatic (auto)
and center (off) (II.O.A.).
8. Nerna. 4x -rated, fiberglass, or equal, enclosure with hinged
cover.
9. All controls to be installed as per manufacturer's
recommendations.
10. Control panel to be installed on outside wall of home within
sight of septic tank/lift station.
c. Piping from the septic tank to the sand filter is to be 1 1/4" PVC schedule 40 solvent weld
piping properly bedded as described in Materials Specifications and sloped to drain back
to the lift station. Insulation board 2" thick and 2' wide centered over pipe is to be placed
where soil cover is less than 4' depth over pipe.
d. The air line from the house to the filter is to be HDPE arctic insulated pipe 3/4"-2" SDR -
11. The compressor will have a gauge (0-10 psi) and be installed in a heated area, crawl
space, or garage.

4. Air Compressor
a. Air compressor shall be a THOMAS LINEAR SERIES model number 5060-A or 507OV-A or
M.O.A. approved equal.
CONSTRUCTION SEQUENCE
In order to insure proper installation, the system is recommended to be installed in the following order:
I. Sewer line from house to tank
II. Septic tank/lift station
III. Sand filter
IV. Leachfield bed (if required)
V. Final grading
CONSTRUCTION
1. Construction Instruments
Contractor shall provide instruments such as transits, or levels, for establishing proper
elevations and levelness of pipe or materials.
2. Pipe Laying
All pipe shall be laid with material free of large or sharp rocks and organic matter.
Each section of pipe shall be handled carefully and placed accurately. The spigot end shall
be fully inserted pointing in the direction of flow and glued in place to make a water tight
seal.
Each section of pipe shall be properly supported to insure true alignment and an invert
which is smooth and free from roughness or irregularity.
At all times, when work is not in progress, open ends of pipe and fittings shall be closed
so that no undesirable substance will enter the end of the pipe or fittings.

3. Bedding of Pipe for Sewer Lines
The pipe should be bedded with materials free of large or sharp rocks and organic matter,
compacted. Native materials may be used as back fill material above the pipe, provided it is
free of large rocks and organic matter.
All lift stations shall be installed in accordance with the manufacturees recommendations
and standards. Electrical installation is to be in accordance with the National Electric Code
(latest adopted edition) by a licensed electrical contractor, with M.O.A. inspection where
required. If MOA inspection is not required, a licensed electrician is to provide a letter to the
Engineer stating that the system meets or exceeds all applicable codes.
4. Excavation
Excavate to proper depth to allow gravity flow, install tank level, tolerance 0.05 foot, on
undisturbed or compacted classified sand fill material.
Create a level sand surface over the tank and add 2" burial foam +35 PSI directly over
tank. Mound unclassified soil over tank with a minimum of 2 feet of cover and anti -flotation
weights that may be required in areas of shallow ground water.
5. Connections
All connections to be as per MOA requirements, PVC 3034 piping with Caulder or
equivalent couplings for inlet riser pipes.
All tank connections are to be completely water tight.
SAND FILTER
The work under this section consists of the performance of all operations pertaining to furnishing and
installing the sand filter.
A. Materials
1. Sand Filter
a. Liner (if required - not required for bottomless sand filter) - OSI Model LI or
equivalent. 30 mil PVC cut and fabricated to provide water tight enclosure of filter
sand volume. Seams to be permanent as per Manufacturer's recommendations. All
connections, such as boot flanges, through sand filter to be permanently bonded to the
liner.

b. The Contractor shall provide a washed sand to meet ASTM C-33 Concrete Sand
Specifications with the additional requirement that all but 1% of the materials
passing the #100 sieve be removed. All sand shall be approved for use by the
Municipality of Anchorage (M.O.A.). If the sample does not meet specifications, all
sand will be removed and replaced.
c. The Contractor shall be responsible for providing pea gravel that shall be a
washed material containing no fines (0% passing the #100 sieve) and a maximum
size of 1/4" diameter.
d. The Contractor shall provide the manifold assembly. The manifold assembly
shall be of approved materials with orifice shields and flushing assembly. All
drilled holes to be cleaned of buns and pipes flushed, prior to assembly. Flushing
valve to include riser to finish grade. Lateral distribution piping is to be laid level
(@ t 0.05').
e. Side walls and top of the filter box to be insulated with 2" direct burial polystyrene
insulation.
f. The Contractor shall be responsible for providing filter fabric.
g. Air coil in sand filter to be geoflow WF164-24 150 LF placed 6" above bottom
of sand.
Construction of Sand Filter
The sand filter construction is to follow procedures as outlined below, as applicable to this
design.
Pea gravel and filter sand are to be hand leveled to proper depth (tolerance within 0.05
feet) in filter, taking care not to allow mud or fine soil material into the filter or gravel layer.
This would include any soil material picked up by excavation machinery buckets while
loading specified material. The Contractor should estimate filter material volumes such that
allowance is made for wastage of material in contact with the ground at site.

The Engineer will reject any sand or pea gravel that does not meet specifications or is

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),

�Lr It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).

0 or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.

TOwner acknowledges that regular maintenance of an AW WTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 247 -hour notice.
` Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On -Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the goveming
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until tide is transferred,

Agreement shall in no way constitiiic a ,,f the provisions, nor in any way affect
the validity of the Agreement or any part hoof, or the right of the Municipality
thereafter to enforce every provision hereof.
S. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction. Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shalt govem the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

Revision to COSA Approval
Ladies and Gentlemen:
I am writing to request that the COSA approval for this property be revised from three bedrooms to four
bedrooms. The system was approved for four bedrooms in 2008. PES tested the system in August of 2012.
We introduced 484 gallons over a 110 minute period. The water levels in the field were dry when we
started the test and remained dry during the entire test. Using the procedure established by Lee Reid, P.E.
report "test of Insitu Septic Systems" one can plot the absorption rate verses time on a log scale to obtain
the daily absorption rate. When plotted on this scale one can easily see that the required 600 gallons will
be absorbed in less than 140 minutes, well below the required 1440 minutes. The actual absorption rate
was measured to be 4.4 gallons per minute (136 minutes to absorb 600 gallons). Therefore I request the
system be approved for four bedrooms.
By granting this approval, the health and safety of this lot or the surrounding lots will not be compromised. Please

THIS MEMORANDUM Or UNDERSTANDING made and entered into as of this

herein the "OWNER., and the Municipality of Anchorage, herein the
"MUNTCIPALITY" In consideration of the mutual covenants contained herein, the
panics to this Memorandum of 7 Tndetstanding agree as follows:
I. ADVANCED WASTEWATER TREATMENT SYSTBMS. Municipality
grants permission to Owner tm utilize and opcmte an Advanced Wastewater
Treatment System (AWWTS),described as_(3t 034111Tex locuted
at /L%M-CH SLvPE ZIU< tj,_ 4-4Y yQ , Anchorage, Alaska.
2. Definitions.
A. Alterution. Any change to rhe design or function of an AW WTS that
Includes the installation or removal of any pans, components or pieces not
included in the. original construction permit and design. Prior to
performing any alterations to an AW WTS the owner must obtain a
Wastewaicr Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
9. Certificate of Onsite Systems A,pprovaL An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and, title transfer in accordance with AMC 15.65.

These approvals certify that dte systems are adequate for the homes that
they support and meet the codes that were in place at die time. of system
construction.
C. Dalnuge. Any man-made or natural change in a system that would inhibit
the system from performing as designed,
D. Maintenance and Repair. The scheduled and as needed replacement or
existing paras, components and pieces of an AWWTS that werc included in
the original design which would allow the AW WTS to continue to
perform as designed.
B. Permit (Construction) An On -Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
P. Permit (Operating) AnAdvnnccdWastewater Treatment System
Operating permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AW WTS, upon meeting all the
tcquiromenls of this agreement, the conditions of Opurating Permit, the
requirements of the On -She Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
I Fee. Owner shall pay to Municipality an annual fee of"'2.tF4O—
($ D .00), payable on or before the issuance orthe operating permit and annually
dwreafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Term. The farm of this Memorandum of Undernnnding shall be for the life of tlic
AW WTS.

The term begins an the date of approval by the Municipatity orthc installed system and
shall continue while the A W WTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certiFcate of on -Site approval is issued to
the new owner or transferee of the property.
5. Allcrattvos.lnslatla(jon ylid Removal of Additions) Equipment, Owner agrees
not to make any alterations, removal of pans or additions to the AW WTS without a
Construction Permit from the Municipality.
6. Mainlennncc and Repairs.
A. Throughout the term orthis Memorandum of Understanding, the Owner shall
maintain A WWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memomudum of Understanding, and any extensions or
renewals thereof, at die owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) impecdan costs.
Further, Owner agrees to compiv will all applicable ordinance, laws, regulations,
rules and orders for the AW WTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AW WTS Operadng permit_ This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's A W WTS

C. Owner acknowledges that the fine schedule for failing to maintnin and repair an
AWWT5 are codified in AMC 14.60.

D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system..
F. Owner agrees to grant the Municipality reasonable access to test and inspucl the
AW WTS upon 24 hours written notice.
P. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification guidebook
for AW WTS is the guveming professional guidelines for the aonstmorion,
maintenance and repair of the Owner's A W WTS.
7. Nonwatver. 'lhc failure of tither parry at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such parry thareafierto enforce each and every provision
hereof.

A_ This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
rbnnality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. for the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized rcptesentatives of the parties arc:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized represemative or unauthorized menns shall be void.
9.3nrisdiction: Choice of Law Any civil action arising from this Memorandum of
Understanding shall he brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The taws of the Stoic of Alaska shall govern the rights and
ubligatioas of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding dccrpcd
invalid by a court ofcompetent jurisdiction shall not invalidate the rcmaining provisions
of the Mernoraodum of Understanding.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Oh -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates'are valid for one year for properties served by Class A or B Wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage riles and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

Engineers Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life ofall wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the solo benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

In conducting this evaluation, GEG, Ltd. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines d Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local softs condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report Is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party Is not authorized, nor will It confer any legal tight whatsoever.
