I spoke with Ms. Debbie Wockenfuss of the On -Site Water & Wastewater Department on June 4'" to find out what the DIIHS issues were with my septic system. She informed me that I was missing the distance to surface water on the Engineer's Inspection form and 1 was missing the Owner Installation Letter. I have spoken to the Engineer that did the inspection and he corrected his form for the missing information and the form is attached. 1 would like to submit the remainder of this letter as the Owner Installation Letter. 1 am a registered Civil Engineer with the State of Alaska (CE9694) and I was trained in the Army to operate numerous pieces of construction equipment including backhoes (dual MOS qualified 62E & 62J). 1 observed the perk test for this site and I also double checked the calculations and conferred with the design engineer for my system prior to having it submitted for the initial permit. 1 rented a trackhoe from a family friend to do the excavating for the septic system. I borrowed a level and rod from work to check elevation prior to calling for the inspections. 1 had sewer rock delivered since 1 do not have a commercial driver's license. 1 had a friend and his son assist me with the installation. It was backfilled with the trackhoe and rough graded. The home excavation contractor later graded the entire site to maintain proper drainage away from the house. 1 finished landscaping the backyard (to include the leach field and septic tank) in 2005 using a D3 dozer from my brother-in-law and a Bobcat skid loader rented from K2 Equipment. The landscaping material was hauled in by Popeye's Trucking, T&.J, and Quam Industries (1 believe). The septic system has worked well since it was put into commission in 2001. JR Septic Pumping has pumped the tank each summer since it was installed. The attached Site Plan and backup documentation identify the location and configuration of the new septic system. The lot is fairly flat with little slope. The test hole was placed as indicated on the site plan. No groundwater or bedrock was encountered in the test hole. The test hole was uniformly compact silty gravel. A water monitor tube was placed to a depth of 16.5 feet and observed for 10 days. No water was observed. Trench System Data: a. Percolation Rate: 40 minutes/inch b.
