To whom it may concern:
The installation of the subject septic system was completed in December of 2001 and May of
2002 (Well). There were deficiencies identified (see attached list) that have yet to be corrected
because the builder has went out of business. The septic system was installed as per the MOA
permit and all applicable codes, except that there is inadequate soil cover over the septic tank.
The well location was selected by the builder and was drilled by MW Drilling. It is extremely
close to the property line and it is uncertain as to whether it is on the property. In the past we
requested an as -built survey that showed the well; however, the builder did not respond to our
requests.

• An as -built showing the well location must be done. We need confirmation that
the well was drilled on the property.
• There is a shortage of soil coverage over the tank. There needs to be 4 feet of
cover over the tank.

To whom it may concern:
The proposed 7 bedroom house will be served by a private well and septic system. Two test
holes weere excavated on the property and logged by Michael E. Anderson, P.E. An additional
test hole was excavated by our firm next to TH-53 to confirm the soils classification. We are
proposing that a 2000 gallon septic tank be installed outside the 100 foot well radii and also a
deep trench type drainfreld be installed around the 30 foot radii of TH-53 and TH#1. Comments
regarding the proposed design are summarized as follows:
1. SOILS: Attached are the logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that an application rate of 1.2 gallons/day/ft2
should be used and that the SM layer at 14.5 feet should act as a sand filter.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/ft2
c. Number of Bedrooms: 7
d. Design Flow: 1050 gallons per day
e. Minimum Absorption Area: 875 ft2
f. Total Depth: 10 feet (max.)
g. Effective Depth: 6 feet
h. Width: 2.5 feet
i. Reduction Factor: N/A
j. Minimum Length: 80 feet total length (2 @ 40 feet long each)
k. Effective absorption area = 960 flZ
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.

4. TOPOGRAPHY: As can be seen on the attached topography site plan, the average
topography in the area of the proposed septic system is a 5% to 15% percent running from
approximately north to south. The trenches are to be installed parallel to slope contours.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic
