Per the homeowners request we are revising the design for 4 bedrooms. Due to a rise in groundwater an Advantex treatment system will be used. The drainfield will be designed mound the 30 foot radius of test hole #2. We are proposing to install a 1500 gallon Advantex tank with a pump vault and a 5-wide type drainfield. TOPOGRAPItY: As can be seen on the attached topography site plan the average slope down hill from the drainfield is approximately 5 to 10 percent running southeast to northwest. That letter requested that you respond to these concerns by December 1, 1997. No response has yet been received by this office. As a result we are issuing to you a Notice of Violation listing the sections of the Anchorage Municipal Code (AMC) with which your wastewater disposal system is not in compliance. We would like to work with you and your engineer on a plan of action for the wastewater disposal system currently in place. Please respond to this request by Friday, January 9, 1998. We will require a wastewater disposal system that complies with AMC Chapter 15.65 Wastewater Disposal Regulations. This department's approval must be complete by June 15, 1998 or we may issue a citation. Citations may carry a fine of up to $300 per day, for each day the wastewater disposal system is not in compliance. We appreciate your cooperation in resolving this situation. Some of our concerns have been addressed, however there are some areas which still need to be clarified. 1) The 2 foot separation distance between the bottom of the field and bedrock. The existing condition of a sand filter surrounded by an impermeable membrane which extends to bedrock is not an acceptable situation and has not been suitably addressed. It appears that this has thus far been addressed with the highlighted comment from the Typical Nide Drainfield Requirements and Specifications sheet from our department and the statement from your October 9 report that there is native soil under the sand filter. The comment highlighted from the `Sample Wide Drainfield Calculations' may have been taken out of context. The statement reinforces that the given example meets the 2 feet of native soil requirement. (3'/2 feet cover + 6" from bottom of pipe to bottom of cover + 3 feet sewer rock = 7 feet total depth. That leaves 2 feet of native soil below the bottom of the trench remaining from the 9 foot depth of the accepting stratum.) The Anchorage Municipal Code (AMC) classifies fractured bedrock the same as bedrock. Therefore the "native soil consisting of sandy silt and shattered rock" is still classified as bedrock and either the separation distance must be met, or justification for waiving this separation distance must be furnished. The AMC 15.65.060-H provides that "Tire Department may approve an on-site wastewater disposal system that does not conform to this Chapter if the Department finds, after consideration of relevant test results, engineering data, publications and other materials, that the system will Anction as effectively as a system that conforms to this Chapter. The applicant shall be responsible for ftrnishing proof that the system will function as effectively as a system provided for in this Chapter. " Please supply us with a proposal for modification of the disposal field which addresses our main concerns of oxygen flow to the effluent in the sand filter, effluent treatment and lateral movement of the effluent before reaching bedrock in addition to data or publications which would justify a variation from specific code requirements. 2) The separation distance from the septic tank to the well must beat least 100 feet. While drilling a new well is an acceptable solution to meeting the separation distance to the septic tank, relocating the tank outside the 100' well radius may be more cost effective for you. Either solution is satisfactory to this Department. The other necessary changes to the disposal system which you have already agreed to, include: 1) Install new central lateral on the bed at a separation distance of S feet on center. 2) Install sweep cleanouts at the far ends of all distribution laterals. 3) Grade mound so that side slopes are no steeper than 33% and plant vegetation on mound. A new As -Built report must be submitted with the completion of the changes to the system. The requirements for this report are included on the second page of the memo given to you at our October 8'h meeting. Please submit to us a strategy and justification for correcting the above requirements by Monday, December V. We appreciate your cooperation in resolving this situation. That distance is 100 horizontal feet. This is also addressed in AMC Chapter 15.55 Water Well Regulations in 15.55.060.13 which lists the minimum separation distance between a well and a septic tank as 100 feet. 15.65.060.E.12 "A subsurface disposal field must have cleanouts.... The cleanouts must be at least four inches in diameter for gravity systems and located near each end of the field" 15.65.060.H.1 "... Design a) Installed system differs significantly from approved design. 15.65.150-F-6 i) Installed system is abed, approved system was a double trench. ii) Installed sand filter is 8 feet deep, approved filter was 4 feet deep. iii) Installed effective depth is 1.5 feet, approved effective depth was 6 inches. b) The absorption area calculations were based on the native soil percolation rate of 5.3 and 2 minutes/inch. The percolation rate of the sand used is unknown. 15.65.060-1-3 c) The distance between the distribution lines is listed as 10 feet and the maximum allowed for a bed is 6 feet. 15.65.060-11-5 2. Installation a) The system is owner installed. Permission was not given by DHHS for this. He was neither information submitted attesting to his qualifications, nor is he an approved excavator. 15.65.035-A. From site visit, owner states he was a project manager for many years and he has photos and receipts of the installation. b) Native soil was excavated to bedrock, the soil layer under the absorption area must be 2 feet thick. 15.65.060-H-1. The accepting stratum must be native soil. 15.65.060-1-2 c) The sand used in the filter layer is not stated as coming from an approved source, nor is a sieve analysis given as per permit special provisions. d) The side slope of the mound is steeper than 33% 15.65.060-1-4 e) The 4 standpipes in the bed are all monitoring tubes. The 2 pipes at the far end of the field from the tank need to be cleanouts. 15.65.060-E-11 3. well a) The well on the site is only 93.4 feet from the first cleanout of the septic tank. b) Nowell log was submitted with the As -Built. 4. As -Built a) Separation distances are not completely filled in b) Well information is missing c) Soil rating information is missing d) Total absorption area is missing e) Bench mark location is missing, however there is no evidence that a survey was even completed. f) Total depth of system from original grade is listed as 10 feet, however 2 soils logs recorded bedrock at 4 and 6 feet from grade. g) Depth to pipe bottom from original grade is listed as 7 feet. This does not agree with the submitted drawing & stated excavation depth. With an excavation of 10 feet and a depth to pipe bottom from original grade of 7 feet, there would only be a 3 foot separation between the bottom of the pipe and the bottom of the system. 5. Septic tanks installed with less than 4 ft. of cover shall be insulated. 5. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. 6. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. ABSORPTION TRENCH/DRAINFIELD INSTALLATION: 1. Excavate the proposed trench to the dimensions shown on the design. The bottom of the excavation shall be within 2 inches of level. If the sidewalls of the excavation become smeared, they must be raked or scratched (ruffed -up) before gravel (sewer rock) placement. 2. Once the gravel is installed, the distribution pipe is to be installed level with the perforations faced downward. Gravel is then to be placed over the distribution pipe to provide a minimum of 2 inches of cover over the pipe. 3. A silt barrier must be installed between the final gravel layer and the native soil backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. 4. Monitor tubes shall be of four (4) inch diameter, installed approximately in the locations shown on the design, and extend a minimum of 12 inches above final grade. The portion of the monitoring tube extending through the gravel shall be perforated from the bottom of the trench to the invert of the distribution pipe. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finish grade over the trench must be mounded to prevent the formation of a.depression after settling. MINIMUM MATERIAL SPECIFICATIONS: 1. Any septic tank proposed for installation must be constructed by a Municipally approved septic tank manufacturer. 2. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam HI or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. 6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the 8200 sieve. 7. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements. These inspections will occur as follows: 1. The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. 2. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. 3. The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre -construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractors activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractors activities. Final acceptance of the contractors work rests with the owner and the M.O.A. Upon excavation to find constricted spot and damage found it was decided replacing the entire line in a shorter run with internal heat trace would be more cast efficient than tracing back the old line to reveal other damaged areas. A new water line was installed from the well head at a depth of 3 feet from the eastern side of the well head to the south west corner of the building making entry to the utility room. Insulated board was added as another layer of protection. An internal heat trace was installed and runs the duration from the entry point to 10 feet down hole to prevent freezing of the line. 10 watts @ 120 -volt heat arctic trace was used. Manufacturer specs state that 2 inches of insulation or 2 feet of earth is a required insulator needed. The line was buried at or slightly beyond the 3 -foot depth depending on the shallow bedrock formation found shallow being the reason the line was originally buried and ultimately sustained damage. The heat trace has a G.F.C.I weatherproof adapter that is currently inside the home. The water line ultimately comes up out of the ground at the well casing and then makes its way down through a sanitary seal into the well and pump line. All exposed line is insulated. The exposed areas at the well head have 3 unions in place so that future servicing or replacement can be done without modification. The old pitless was plugged and sealed to prevent any entry points for contaminants. A flow test was also conducted on 10/29/2015 resulting in a well yield of 1.9 g.p.m. Total depth is 105 feet. S.W.L was found to be 24 feet 8 inches. Well has a new sanitary seal and the wires are encased in liqui tite conduit to allow flexibility for servicing. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The teim of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations, Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (S) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a Written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. I. Owner agrees to provide and maintain a telephone connection to the AWWTS as required by the AWWTS approval. 6. Nonwaiver. The failure of either parry at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any,part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A.
