We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the subject site. This
investigation was performed in accordance with your request of
April 16, 1976 and those procedures outlined in a letter dated
July 15, 1975, by Mr. Rolf Strickland of the Municipality of Anchorage,
Department of Environmental Quality.
A single test hole was put down within the Lot 16 area for the purpose
of defining general subsurface soil conditions for the proposed
sanitary system. Excavation was accomplished with a truck mounted
auger type drilling rig and the test hole was extended to a total
depth of 15 feet below ground surface. The final log prepared for
the test hole has been included in Drawing A-01.
Ground water was not encountered in the test hole.
We appreciate being given this opportunity to be of service to you.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an Independent professional civil
engineer registered In the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also Issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage Is not responsible for errors or omissions in the professional engineers work.

In conducting this evaluation, GEG, Ltd. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life crag wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report Is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party Is not authorized, nor will it confer any legal right whatsoever.

Septic System Upgrade Proposal
Dear Ronda,

Thank you for calling me regarding the well and septic tests for your house, In preparation for
the sale. We ran a flow test on the well and a septic adequacy test on July 13, 2004. The well
flow was adequate for a 4 bedroom house, but the septic system will require an upgrade.
Details of the tests and expected costs of a septic upgrade are provided below. Please call me
at 349-1851 if you have any questions.
Well Test
The well now test was conducted on July 13, 2004. According to a well log acquired from the
Municipality of Anchorage (MOA) Onsite Water and Wastewater Dept., the well is 144 ft deep
and was drilled in May 1976. During our test, the initial static fluid level was at 96 ft. It dropped
to 114 ft and stayed there throughout the test. Total production from the well was 536.6 gallons
over 158 minutes, which yields a rate .of 3.4 gallons per minute (gpm).. This exceeds the
requirement of 0.42 gpm for a 4 bedroom house.
Septic Adequacy Test
The septic adequacy test was conducted concurrently with the well flow test. According to the
MOA records, the septic system consists of a 1250 gallon fiberglass septic tank and a 40 ft long,
3 ft wide, 6 ft effective depth trench installed In 1976. At the beginning of the test, there was 56
inches of water in the drainfield. After introducing 536 gallons of water, the system backed up at ..., ,...,
the house, and the test was curtailed. • The required absorption rate'for a 4 bedroom house is
600 gallons per day. Another attempt was made to introduce water into the field 4 hours later in
the day. Water could only be introduced at a rate of 1 gpm without backing the system up. My
conclusion is that the dralnfield is technically failed, with the operating level at the lateral pipe.
Septic Upgrade Process & Costs
In order to design a new dralnfield, the soils and groundwater level must be evaluated by
digging test holes. After a design is complete, it is submitted to the MOA with a permit
application. When the permit is approved, 1 can assist you In obtaining bids from qualified
excavators to install the new drainfield. Since your tank is fiberglass, it should still be in good
condition and can be used again. It may be possible to retain the old drainfield, to be used
again in the future. After a period of drying out (about 3 years), it should be naturally
rejuvenated.

Since the available space on the lot Is limited, It will be necessary to have a land surveyor mark
the lot lines. A new as -built survey will be needed to obtain the Health Authority Approval (HAA)
from the MOA. I can assist you In contacting a qualified surveyor.
To obtain an HAA, we will need to test the water quality. This is usually done when you have a
contract on the house, because the water test must be performed within the 30 days prior to the
HAA application.
The cost of the new septic system cannot be determined until the design is complete. A
conventional system would cost between $10,000 and $15,000, and an advanced system would
be around $20,000. We will look for the most cost-effective alternative. A cost estimate follows:

General
1. Total coliform or other bacteria was not detected for the water sample collected from the well.
2. The nitrate sample taken from the subject well is 0.151 mglL.
3. The well was installed in 1976 at a total depth of 144 feet. The static water level at the time of
Installation was 89 feet, and is currently 98 feet. The well is cased to a depth of 144 feet.
4. The absorption field was also Installed in 1976. The absorption field Is 40 feet in length, with gravel
depth below the pipe of 6 feet, and a total depth of 9 feet from original grade. A single family dwelling
exists on the property.

Waiver Request and Health Authority Approval for Sunset Hills Subdivision; Lot 16, Block C.
The existing 4 bedroom house is served by a private well and septic system. We request you
grant a 96 feet separation distance waiver from the well to the septic tank on the referenced
property. The measurement from the center of the well to the center of the tank cleanout is
100.10 feet. We are assuming that the separation to the edge of the tank from the well is
approximately 98 feet. We are conservatively asking for a 96 feet separation distance waiver. The
following items are justification for the waivers:
The referenced lot is fairly flat. The house is between the well head and septic system so if
the septic system was to overflow, it appears that the effluent would not travel toward the
well head.
The location of the septic system is in a very visible area so that if any effluent was to
surface, it would be noticed and the problem corrected.
The other path of contamination is subsurface migration wastewater should the tank begin to
leak. As can be seen on the attached well logs, the aquifer is relatively deep, with approximately
64-100 feet of silt, clay, and hardpan soils that have served to inhibit the migration of untreated
wastewater into the aquifer. Recent water sample results indicated nitrate levels of 0.151 mg/L
and coliform bacteria were undetected. Based upon the aforementioned facts, it appears that
there is minimal risk associated with granting the 96 feet separation distance waiver.
We also request a 0' lot line waiver from the existing drainfield to the northeast lot line. There
would be no adverse impacts with the granting of this waiver.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single family on-site wastewater disposal and/or
water supply system. DSD also Issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of Issue for properties served by a private or Class C well and may
be reissued with new water sample results less than 30 days old. (Certificates may be reissued for a period of
up to one year with valid water samples.) Certificates are valid for one year for properties served by Class A or B
wells or a public water system. The Municipality of Anchorage is not responsible for errors or omissions in the
professional engineer's work.
Note: Alaska Waterand Wastewater Consultants, Inc. shall be paid $1,250.00 at, or prior
to closing for the engineering services provided.

In conducting this evaluation, AYWVC, Ina attempted to provide a thorough,
conscientious engineering analysis of the system to accordance with ADEC and MOA
DSD Guidelines A Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable fee tures. The operational file of all wells and
septic systems depend on the local soils condition, groundwater levels the may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee hrture performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AWWC, Inc. can therefore not provide

Approval for the individual sewer and water facilities cannot
be granted until the following items have been completed:
• The water analysis report needs to be submitted to this
office from the Chem Lab,,5633 B Street, for our review.
• The septic tank pumped with a receipt submitted to this
department.
• An adequacy test needs to be performed on the existing
/ leaching area. This test will determine if the system is
adequate according to National Standards. A listing of
private firms performing the test is enclosed. This report
needs to be submitted to this office for our review.
Please notify this Department for a reinspection when the
noted discrepancies have been corrected. If there are any
further questions, please call this office at 264-4720.
