Boyle Subdivision Separation Distance Waiver Absorption Bed to Lot Line Dear On Site Services Engineer: We hereby apply for a lot line waiver to allow the absorption bed on Lot 8B, Block 1, Thomas L. Boyle Subdivision to be constructed within 6' of the north property line as shown on the attached as -built drawing. The north line was not clearly identified prior to construction and the contractor inadvertently placed the bed closer than 10' from the line. Field investigations prior to construction found this area to have the best underlying soil for the construction of a new absorption bed. We also installed an Intermittent Bottomless Sand Filter System on the lot which normally treats septic effluent to much higher standards than a typical septic system. No conflicts were noted between this absorption bed and septic systems on adjacent lots. The lot to the north is currently vacant. We are therefore requesting a permit be issued for the construction of a new septic system to serve the existing three-bedroom home on the lot. The attached Site Plan and backup documentation identify the location and configuration of the new septic system and the parameters used in the design. Also identified on the plans are the locations of the existing well on the lot, test hole location and the locations of the wells and septic systems in the area. Drainage arrows are also shown and current drainage patterns will be maintained after construction. The lot is plagued by areas of surface water and some grading and drainage diversion will be required to assure a minimum separation distance of 100' between any surface water and the new septic system. We are proposing use of the Intermittent Bottomless Sand Filter System for this lot due to the shallow groundwater and the presence of surface water. The test hole placed near the location of the proposed absorption bed indicated clean sand with varying amounts of silt and percolation rates of between 1 and 5 minutes per inch. Groundwater was found during the placement of the test holes at approximately 8' below the surface. The level remained at 8' when measured on June 23, 2002. We are therefore proposing to use the Intermittent Sand Filter System with a 12' wide by 30' long absorption bed on the property. The bed will be constructed at the original ground surface with the sand filter constructed to a maximum depth of 2' below the surface to insure a separation distance of 4' to groundwater. The ground surface on the lot slopes as shown on the attached Site Plan with fairly shallow slopes from east to west. The new absorption bed will be constructed in conformance with Municipal requirements and parallel to ground contours where possible. All components of the new septic system will be constructed a minimum of 10' from the water service line on the lot and 100' from all wells in the area. The system will also be constructed a minimum of 100' from any surface water in the area. The scope of this project includes the pumping, removal and disposal in accordance with ADEC requirements of the existing septic tank on the property. It also includes the procurement and placement of a new 1,250 gallon combination septic tank and lift station and pressure distribution system. Work also includes the construction of a new 30' long x 12' wide Bottomless Intermittent Sand Filter at the location shown on the attached Site Plan. A minimum of 2' of coarse sand meeting the specification from Anchorage Tank must be imported and placed in the filter area. The 6" of pea gravel and distribution piping must then be constructed atop the imported filter sand material in conformance with specifications fumished by the supplier. The distribution piping for the filter must be placed at the original ground surface at the area shown on the Site Plan to provide a minimum of 4' separation to groundwater. All work must be completed in strict conformance with the manufacturer's requirements for installation of the intermittent sand filter. 2. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The Contractor shall be responsible for obtaining all underground utility locates and for the layout of the septic system and verification of the location of all lot lines. 4. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. Property owner shall be responsible for revegetation of affected areas unless specifically agreed otherwise. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Department of Health and Human Services for system installations. Owners installing their own systems must receive prior approval from D.H.H.S. before beginning system installation. SEPTIC TANK/LIFT STATION INSTALLATION The 1,250 gallon septic tank/lift station must be constructed by a certified tank manufacturer. Construction shall include an 18" manhole to provide access to the lift station. 2. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. Lot 1, Block 1, Boyle June 23, 2002 Page 2 of 3 4. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial insulation. 5. A foundation cleanout shall be installed one to four feet from the building foundation. Two cleanouts are required between the tank and the drainfield. 6. Final grading over the tank/lift station shall be such that a positive slope exists away from the septic tank. ABSORPTION BED CONSTRUCTION: 1. The absorption bed shall be constructed to the dimensions shown on the design. The bottom of the bed shall be within 2" of level. 2. Distribution piping must be placed level with perforations down atop a level bed of drainfield rock. Rock should then be placed over the pipe to provide a minimum of 2" of cover. 3. A silt barrier or geotextile fabric must be placed between the drainfield rock and the natural soil backfill. 4. Monitor tubes must be 4" in diameter and installed at the locations shown on the design. The portion below ground must be perforated. 5. Contractor shall verify the septic tank and drainfield are a minimum 100' away from any private water wells in the area, 150' from a Class "C" Well or 200' from any community well. 6. Direct bury insulation must be placed over the distribution system if less than 3' of backfill depth is available. Finish grade over the trench must be mounded to prevent settlement or depressions. 7. Grade area surrounding the absorption trenches to drain away. 8. A minimum 2' of accepting soil is required below the drainfield rock for a 5' wide trench. Contractor shall verify this condition prior to placement of the rock. All pockets of unacceptable materials must be removed and replaced. MATERIAL SPECIFICATIONS: 1. The lift station must be constructed by a Municipally approved septic tank manufacturer. An Orenco 20 OSI 05 HHF-5 pump is recommended. Anchorage Tank is a recommended supplier for the materials required for this project. 2. The following pipe materials are approved for use in septic system installations in the Municipality of Anchorage: Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662 or A.B.S. (perforated and solid). 2. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco, or equal). 4. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed between the final drain rock layer and the native soil layer. 5. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the #200 sieve. EQUIPMENT SPECIFICATIONS: 1. A pressure gauge calibrated from 0 to 10 PSI shall be furnished and installed in line with the air line. The gauge shall be mounted in an area which provides easy viewing and is seen on a regular basis such as the entrance to the home. 4. Material for Filter Sand must be classified as a masonry sand and meet the following gradation: 50% or More Passing the No. 4 Sieve 2% or Less Passing the No. 100 Sieve 1% or Less Passing the No. 200 Sieve INSPECTIONS: A minimum of two inspections are required by Municipal Ordinance. These inspections must be conducted under the supervision of a professional engineer registered in the State of Alaska. The first inspection must be conducted after the excavation of trenches, beds or pits and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations, Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this agreement, the only authorized representatives of the parties are: a. Owner: 0 b. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance and Repair Agreement shall be brought in the Superior Court for the Third Judicial District of the State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Maintenance and Repair Agreement. 9. Severability. Any provisions of this Maintenance and Repair Agreement decreed invalid by a court of competent jurisdiction shall not invalidate the remaining provisions of the Maintenance and Repair Agreement. Subject: Lot 8B, Block 1, Boyle Subdivision Conditional Certificate of Health Authority Approval Dear Jim: The owner of Lot 8B, Block 1, Boyle Subdivision passed away more than 2 years ago. The house on the property has been vacant since that time and is currently being sold by the executors of the estate. No money is available in the estate to test, upgrade or certify the existing well and septic system on the lot prior to the sale. We are therefore requesting a Conditional Certificate of Health Authority Approval be issued for the lot with the condition that the well and septic system on the lot be brought into compliance by the new buyers within 6 months of the sale date. Funds will be escrowed to cover the costs of any work required to upgrade the well and septic system to comply with Municipal requirements. The current well and septic system in place on the lot were constructed in October of 1972. A recent inspection indicated a large excavated low area over the existing absorption trench. No monitor tubes or cleanouts could be found to assess the general condition of the trench. It appeared, however, that the system may have failed at some time in the past and an attempt had been made to surface drain effluent from the tank to surrounding low areas. It was obvious from the inspection that substantial work would be required to bring the existing system into compliance and that a new system would probably have to be constructed. The well is only 50' from the existing septic tank. The water service line to the house exits the top of the wellhead indicating there is no pitless adapter. Wiring is not in conduit and a sanitary cap must be placed. The only soils log available for the lot indicates silty gravel material with groundwater seeps at 8'. A notation was made on the soils log that the water seeps do not appear to be from groundwater. Our inspection on May 11, noted several areas to the south of the property with running surface water probably from snowmelt. It is logical to assume the groundwater table on this lot is fairly shallow, but will probably not prevent construction of a new septic system. Substantial surface area exists to the east of the existing septic system for a new system. This area is higher in elevation and better drained than the location of the current system. A lift station may be required to properly transport the effluent from the septic tank to the new absorption area. The buyer has expressed a willingness to escrow 1.5 times this amount until the actual work can be completed. We therefore request a Conditional Certificate of Health Authority Approval be issued for this property so the transaction can be completed and the title to the property be transferred.
