Ref: Proposed Septic System for Hidden Hills Subdivision, Lot 41, Block 1,
To whom it may concern:
The proposed 5 bedroom house is to be served by a private well and septic system. One test hole
was excavated on the property. The drainfield will be designed around the 30 foot radius of the
test hole. We are proposing to install a grinder pump inside the foundation, a 1500 gallon septic
tank, and a deep trench type drainfieldplot
. The proposed well is to be drilled in the area shown on the attached site plan and design
drawing. Comments regarding the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 1.2 gallons/day/ftZ should be used, and that the insitu sandy soils will act as a
sand filter.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Proposed Application Rate: 1.2 gallons/day/ftZ
c. Number of Bedrooms: 5
d. Design Flow: 750 gallons per day
e. Minimum Absorption Area: 625 ftZ
f. Total Depth: 13 feet (maximum.)
g. Effective Depth: 8 feet
h. Width: 2.5 feet
i. Minimum Length: 40 feet long
j. Effective absorption area = 640 ftZ

3. SURFACE WATERS: There are no surface waters within 100+ feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached design drawing, the average topography in
the area of the proposed drainfield is approximately 20-25% running north/northwest to
south/southeast. In short, there are no slope concerns.
5. LOT LINE WAIVER: We request A 2 feet lot line waiver from the east/northeast lot line
to the proposed primary and alternate drainfields. We are unaware of any adverse effects on the
neighboring property with the granting of this waiver.

Dear Mr. Gamess:
Your request for a waiver of the required 10 feet horizontal separation from the Lot Line
to the primary and alternate Drainfields has been approved. The approved separation
distance is 2 feet.
This waiver approval applies to the existing Lot Line to the primary and alternate
Drainfields only. Any future upgrade to the on-site wastewater disposal system will
require all separation distances be met or another approval from this department.

Dear Mr. Gamess:
On 6/2/2004 this o0icc received your submittal requesting a waiver of the required separation distance between
a "Class C' well located on Lot 19 Block I IIidden Hill Subdivision, and a proposed septic located on Lot 41
Block 1 Hidden Hills Subdivision.
The well was granted final operation approval on March 9, 1985 by this office. Waivers were included which
addressed the separation distance to septic systems in existence at that time. Due to the comparatively small
lots and surrounding development, it is not possible to construct a septic system on Lot 41 that meets current
minimum separation distance requirements to the Class C well on Lot 19.
Based on the information provided, a minimum separation distance of 100 feet is approved between the Class C
well on Lot 19 and the septic tank or absorption field on Lot 41.
Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC
15.195. 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15185,
Informal review requests must be delivered to the Division Director, 555 Cordova St., Anchorage, Alaska
99501, within 15 days of the permit decision. Adjudicatory hearing requests must be delivered to the
Commissioner of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, Juneau,
Alaska 99801, within 30 days of the permit decision. If a hearing is not requested within 30 days, the right to
appeal is waived.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.

investigation, based on procedures outlined in the Health Authority Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is(are) safe, functional and adequate

information obtained from the Municipality of Anchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG, Ltd. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.
