The attached site plan identifies the location of the home as well as the addition septic location. The existing 4-bedroom system includes two trenches, each designed at 46’ x 2.5’ x 5.5 ED. The new third trench is designed at 25’ x 3’ x 5.5’ ED. Effluent will be distributed using the 5 -hole distribution box as follows: two outlets will be combined with a Y-connection to feed the two existing trenches, one pair per trench, and the final outlet will connect to the new trench. This configuration will provide appropriate distribution of effluent across all three trenches. The ground surface on the lot are very mild slopes. There are no slopes greater than 25% within 50 feet downslope of the primary site. Contours are shown on the site plan showing the grade and direction of flow. Stormwater drainage will not impact this septic system. The new trench will be constructed parallel to the slope as much as possible. Although the new test hole that we dug perked at 15.0 min/in, the older test holes perked at 30 min/in. For that reason, we used the slightly conservative rate of 0.6 GPD/SF in our design. Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all wells and surface water, and more than 5’ away from the septic tank. The addition will be a minimum of 11’ away from existing system. Please refer to the attached test hole log, plan, and profile pages for the septic design. If this design is followed, there will be no adverse impacts to adjacent properties. Box 196650 Anchorage, Alaska 99519-6650 Ref: Septic System Upgrade for Lot 55, T12N, R3w, Section 23, S180', less 110' To whom it may concern: The existing 4 bedroom house is served by a private well and septic system. The existing septic system is in a state of failure and needs to be upgraded. A test hole was dug on the property. The drainfield will be designed around the 30 foot radius of this test hole. We are proposing that a new 1250 gallon septic tank and also deep trench type drainfield be installed. Comments regarding the design are summarized as follows: 1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and the percolation test results. It is our opinion that due to the overall appearance of the soils, an application rate of 0.6 gallons/day/ft2 should be used. 2. TRENCH DESIGN: a. Percolation Rate: 30 minutes/inch b. Proposed Application Rate: 0.6 gallons/day/ft2 c. Number of Bedrooms: 4 d. Design Flow: 600 gallons per day e. Minimum Absorption Area: 1000 ft2 f. Total Depth: 10 feet (max.) g. Effective Depth: 6 feet h. Width: 2.5 feet i. Reduction Factor: N/A j. Minimum Length: 84 feet long k. Effective absorption area =1008 ft 3. SURFACE RATERS: There is a stream along the west end of the property. The septic system is to be installed 100+ feet from the stream. 3701 E. Tudor Road, Suite 101 • Anchorage, AK 99507 Ph: (907) 337-6179 • Fax: (907) 338-3246 • Website: akwwc.com 4. TOPOGRAPHY: As can be seen on the attached design drawing, the average topography in the area of the proposed drainfield is fairly flat. In short, there are no slope concerns. I am unaware of any adverse impacts this installation would have on adjacent wells or septic systems. If you have any questions, please contact us at 337-6179. Thank you for your assistance. P.E., M.S. NOTE. Attached is a site plan drawing, a design drawing, a soils log, and a 7 page construction specification letter which are all part of the design package for this septic system. 3701 E. GARNESS, CERTIFY THAT THIS W ERFORMED IN ACCORDANCE WITH ALL STATE AND MUNICIPAL GUIDELINES IN EFFECT ON THIS DATE: WALoi To Whom It May Concern: 21 August 2000 This is concerning the leach field at 3231 E. 144'" Ave. Anchorage, Alaska. To the best of my recollection my father installed it in the early 70s soon after we moved from 3401 E. 144h. The question that this letter is to address is the manufacture date stamp on a visible pipe that is part of the leach field. If it has a newer pipe it would only have been placed to fix an existing pipe. II L . #C'e'ra Jerry S. My commission expires g S -c') a 1 r • To Whom It May Concern, I, Marcel Warmilee, purchased 3231 E 144' , S 180 Lot 55, Section 33, T12N, R3 W, S.M. except W. 110 on October of 1997. The summer of 1999, I was excavating the roadside on the south side of the property where I noticed a large boulder. I was planning to move the boulder in my yard for decoration. Under the boulder, I noticed a pipe. I started digging around the pipe and found it was the end of the leech field. STATEMENT OF INSPECTION BY ENGINEER As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation. In conducting an adequacy test,I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty for future performance, nor can we estimate remaining life of the system.
