To whom it may concern:
The existing 3 bedroom house is served by a community water system (Class "A") and a private septic
system. The existing septic tank is within 5 feet of the homes foundation; a waiver was informally
requested and then denied by your department. In response we are proposing to remove the existing tank
and replace it with a new 1000 gallon septic tank in a location which meets all separation distance
requirements.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/orseptic
system in accordance with the guidelines and regulations established by the Municipality of
Anchorage and industry practices. The reported results describe the condition of the systems on the
dates of the evaluation. Separation distances were measured to readily identifiable features. Hidden
defects or encroachments may exist that were not identified during the evaluation. The operational
life of all wells and septic systems depend on a variety of variables including, but not limited to, soil
conditions, groundwater levels (that may fluctuate during the year), quality of construction (materials
and workmanship), and the water usage of the family utilizing the systems. These conditions can
vary, and are outside the control of GEG. Satisfactory test results do not guarantee future
performance of the systems; therefore, GEG makes no warranty(express orimplied) regarding the
future performance of the well or septic system. GEG makes no representation whether an
alternative well or septic system can be installed on the property in the event eitherof the current
systems fail. The content of this report is for the sole benefit of the persor✓party who retained GEG.

To whom it may concern:
The existing drainfield currently serves the existing 3 bedroom house. The separation distance
between the drainfield and the building foundation is 5 feet which does not meet MOA
requirements. The effective depth of the drainfield is positioned lower than the elevation of the
foundation so there is no potential for effluent to adversely impact the structure. In short there is
minimal risk associated with the grating of this waiver.

'We received bid from Northern Excavation and Construction, LLC for $6,200 and
another from .1R's Septic Pumping for $6,250, Our company will escrow at Ioan closing
Sl 2,500, which is two (2) times the funds of the highest bid to perform the scope of the
work agreed to by the owner and contractor. The source of the escrowed fiords will be the
seller's proceeds from the house sale and/or cash the seller provides for the escrow at
time of closing.
We will not release the funds in the escrow wail we get municipal approval in the forn
