Impacts to Adjacent Properties
Dear Onsite Services Engineer.
The owner of Lot 6, Block 5, Meadowridge Estates Subdivision North Addition intends
to construct a new three-bedroom home on the lot. The lot is currently vacant and is
served by a community water system. We are hereby applying for a permit to construct
a new septic system on the lot to serve the new home. The attached Site Plan and
backup documentation identify the location and configuration of the new septic system
and the parameters used in the design. It also shows the location of the alternate site
and the water service line. The lot is nearly 3.5 acres in surface area. Existing
drainage patterns are shown and will not be altered by the development of the lot.
Substantial grading has taken place in the past as site preparation for a new church.
Two test holes were placed at the proposed location for the new absorption trench and
revealed gravel with varying degrees of silt. Some angular rock was found, but did not
appear in segregated lenses. No groundwater was noted in the test holes during
excavation and none was noted during the monitoring period. We are therefore
proposing to place a 3' wide by 63' long absorption by 8' effective depth absorption
trench to treat the septic effluent. The pipe will be placed at 3' below the existing
ground surface and the total depth of the trench will bell'. A minimum of 3' of cover
will be placed over the bed to prevent frost penetration and possible freezing.
The ground surface on the lot slopes as shown on the attached Site Plan with steep
grades from east to west. The area for the new absorption trench, however, is fairly
flat. The new trench will be constructed parallel to the contours of the surface in
conformance with Municipal requirements. Grading will be accomplished to assure
surface drainage is away from the new trench. The existing drainage pattern on the lot
will not be affected during development.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future. The subdivision is currently

served by a community water system.

2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve

patterns in the area. The current drainage pattern will be maintained.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 6, BLOCK 5, MEADOWRIDGE ESTATES SUBDIVISION
GENERAL:
1. The scope of this project includes the procurement and placement of a
new 1,000 gallon septic tank at the location shown on the site plan.
Work also includes the construction of a new 63' long X 3' wide X 8'
effective depth absorption trench at the location shown. The
distribution line in the trench will be placed at 3' below the existing
ground surface. Total depth of the trench will be 11' below the existing
ground surface. All components of the septic syystem must be placed
a minimum of 10' from the water service line to be constructed on the
lot. I
2. Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
3. The Contractor shall be responsible for obtaining all u Iderground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for I revegetation of
affected areas unless specifically agreed otherwise. 1
5. Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system Installation.
SEPTIC TANK INSTALLATION
i
1. A new 1,000 gallon septic tank must be be procured from an approved
source and installed at the location shown.
2. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
i
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank. I
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.
6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.

7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the trench shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
6. Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trenches to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Fernco, or equal).

5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.

6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first' inspection must be
conducted after the excavation of trenches, beds or';pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.

The second inspection must be conducted after the placement of the
geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as -built of the completed system.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

As -Built Report
The inspection report for this lot was apparently never finally approved. I was unaware
of this until recently when we were requested to complete the Certificate of On Site
System Approval. My responses to Joe Goodall's comments made on May 9, 2005 are
as follows:
' I1' No exception fee is necessary as the final design is identical to the original
design. The actual location of the house changed from the initial permit
issuance. The file shows the permit was renewed at least twice before the septic
system was finally constructed. The original design and permit were completed
in 2001. Construction was completed in 2004.

)2 . Test Hole No. 3 was completed and submitted to the City. The entire alternate
site falls within the 30' radius of Test Hole No. 3.
�r1. The Site Plan has been drawn to a 1"=60' scale.
4 No slopes on the property exceed 20% and most are less than 10%. Substantial
grading and revegetation has taken place on the lot since the septic system was
constructed. It is not necessary to show the slopes on the Site Plan.
5. The storage tank belongs to the Dawn Water System and not to the property
owner. The abandoned well is also the responsibility of the Dawn Water
System. Water is provided to the lot by the Dawn Public Water System.
L) 6. The house has only three bedrooms. The Municipality unilaterally changed the
tax assessment in May of this year to 4 bedrooms. An appraisal verifying the
number of bedrooms is attached.

NOW, THEREFORE, BE IT RESOLVED by the Platting Authority that:
1, The Platting Board makes the following findings of fact.
a. The well radii were platted for a proposed community well that was to
serve the North Addition to Meadow Ridge Subdivision. The well was
drilled, but found not to produce sufficient. water. The well has never been
operational either as a community well or as a private well
b. The Dawn Development Corporation owns the public water system that.
serves the 21 residertia: lots in Meadow Ridge Subdivision, - North
Addition, The water distribution system is gravity -ted with a holding tank
located on lot 6. An easement allows access to the water well and storage
tank located on Lot 6. This easement for access to the water well and
storage tank; as well as for maintenance purposes, was granted to the
Meadow Ridge Estates Subdivision Homeowners Association and recorded
.April 2, 1991 at Book 2138, Page 188. The access easement will not be
affected by removing the well radii.
C. The Board found that there is no need for thr well radii and the plat note
referring to the well radii as the well is not and has never functioned as
either a community or a private well due to the low yields obtained during
the original pumping tests.
