Box 196650 Anchorage, AK 99519 Re: Voyles Lot 8 Block 2 Narrative & Permit Application Dear Mr. Roth: Eagle River Engineering Services (EKES) was contracted to design and permit a septic system replacement at the above referenced property. ERES researched the proposed replacement site from the original septic permit, and found that,a shed was constructed over a large portion of the reserve site. Additionally, the only other available site would have taken down all the trees from the property. Based on these factors, along with poor soils and water table issues, the homeowner opted to utilize Advantex Technology. A test hole was excavated in the vicinity of the proposed original replacement site, near the test hole performed by S & S Engineering in 1994, and a new test hole in a different area on the lot. Percolation tests have yielded a percolation rate of approximately 10 minutes per inch in the upper soil layers in both test holes. S & S percolation test data yielded 53 minutes per inch. The water table was found to be 6.5' and dry in the two holes after 7 days of monitoring. The soil type appears adequate to support a new 3 bedroom leachfield if utilizing Advantex Technology. Advantex technology will prolong the life of the leachfield due to the slow percolation rate of the lower soil layer. Additionally, we will design the leachfields for the wastewater application rate corresponding to 30-60 minutes per inch. The proposed 3 bedroom septic system will have very limited impact on adjacent properties for the following reasons: 1. The surrounding lots are large, so there is room for wells, septic systems and alternate sites. 2. Immediate neighboring septic systems are all +20' distance, and no private (or community) wells within 100' of the proposed septic system. 3. GENERAL 1. The septic plan is for a 3 bedroom single family residence only. 2. The drawing and or site plan shall be apart of this specification. 3. All materials and workmanship shall meet the Anchorage Department of Ilealth and State Department of Environmental Conservation requirements. 4. All soil tests are advisory to the design and are to be verified or modified in the field by the engineer. 5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet Municipality of Anchorage, Department of Environmental Conservation requirements. 6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any adjacent multi -family wells. A licensed surveyor should locate any utility easements and exact 100' radius from the existing and neighboring on-site wells. 7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer approval. 8. Any remaining open test hole excavations shall be filled and monitor tube removed. B. ADVENTEX SYSTEM AND SEPTIC TANK 1. Septic tank shall be a fiberglass or steel 1500 -gallon tank capacity of MOA approved construction for use with Advantex system. Install as per current manufacturers recommendations for conditions in Alaska. 2. Install one AX -20 filter pod over septic tank capable of treating 600 gallons per day of waste flow. 3. Install Orenco 24" pump basin PB -2496 outside septic tank, with OSI pump model P30-05 and controls to allow 23 gallon dosing of septic leachfield. Set pump float to 13.5" above pump, timer for 1 minute dose time. Controls to be installed to MOA code by licensed electrician familiar with OSI applications. Controls to include a dedicated phone line and web based monitoring system. 4. A municipal inspection report or receipt from a licensed electrician shall be provided to the engineer verifying lift station wiring to all applicable codes. C. ' DRAINFIELD 1. The trench is to be located as shown on the site plan. 2. The total depth of the initial drainfield excavation is to be approximately 2.5' deep, relative to existing ground elevation at ground water monitor tube and end in native SM soil material. The excavation shall remove the topsoil layer, and just expose the free -draining soil beneath. The bottom of the trench shall be level, plus or minus 1.5". 3. Aporoximately two feet of Coarse Sand shall be utilized beneath the gravel layer to level the bottom of the drainfield. 4. A fifteen -inch layer of sewer rock is to be placed over the sand layer. The 1.0" effluent piping is to be buried within the gravel layer with 2" of gravel cover over the piping. 5. The completed trench gravel and piping is to be covered with typar fabric material. 6. Soil or combination of soil and extruded board insulation to a depth of 3' or equivalent is to be placed over the bed. Mounded side slopes on uphill side and ends are not to exceed 3:1. 7. In consideration of the mutual covenants contained herein, the parties to this Memorandum of Understanding agree as follows: 1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as Aoyw TEV located at NIWAIAS `�) L-7 Anchorage, Alaska. aI-7Aq avwiau�t (t 2. Definitions. A. Alteration. Any change to the design or function of an AWWTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Prior to performing any alterations to an AWW TS the owner must obtain a Wastewater Disposal System Construction Permit from the Municipality pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65. B. Certificate of On -Site S}'stems Approval. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with AMC 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. C. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. D. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AW WTS that were included in the original design which would allow the AWWTS to continue to perform as designed. E. Permit (Construction) An On -Site Wastewater Disposal System Construction Permit as defined by AMC 15.65. F. Permit (Operating) An Advanced Wastewater Treatment System Operating Permit. An annual permit, issued by the Municipality, that allows the Owner to operate an AW WTS, upon meeting all the requirements of this agreement, the conditions of Operating Permit, the requirements of the On -Site Wastewater System Construction Permit and all relevant provisions of AMC 15.65 3. Fee. Owner shall pay to Municipality an annual fee of ik D 1.00), payable on or before the issuance of the operating permit and annually thereafter. The annual fee is due on or before the anniversary date of the approval by the Municipality of installed system. 4. Term. The term of this Memorandum of Understanding shall be for the life of the AWWTS. The term begins on the date of approval by the Municipality of the installed system and shall continue while the AWWTS system is in use or operational or until the property is sold or title is transferred by owner and a new certificate of On -Site approval is issued to the new owner or transferee of the property. 5. Alterations Installation and Removal of Additional F,ouipment. Owner agrees not to make any alterations, removal of parts or additions to the AWWTS without a Construction Permit from the Municipality. 6. Maintenance and Repairs. A. Throughout the term of this Memorandum of Understanding, the Owner shall maintain ANVWTS in good repair. In addition, it shall be the responsibility of the Owner during the term of this Memorandum of Understanding, and any extensions or renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. Further, Owner agrees to comply will all applicable ordinance, laws, regulations, rules and orders for the AWWTS. B. Owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system pursuant to the terms and conditions contained in the Owner's AWWTS Operating Permit. This schedule shall be submitted to the Municipality annually upon the renewal of the permit. The schedule of maintenance and repair contained in the Owner's AWWTS Operating Permit is: C. Owner acknowledges that the fine schedule for failing to maintain and repair an AWWTS are codified in AMC 14.60. D. Owner agrees that only maintenance, repair personnel certified by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. E. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. F. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. G. Owner agrees that the relevant provisions of the standard specification guidebook for AWWTS is the governing professional guidelines for the construction, maintenance and repair of the Owner's AVrNArTS. 7. Nonwaiver. The failure of either party at anytime to enforce a provision of this Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in any way effect the validity of the Memorandum of Understanding or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. S. Amendment. A. This Memorandum of Understanding shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality of this Memorandum of Understanding was executed and such writing shall be attached to this Memorandum of Understanding as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this contract, the only authorized representatives of the parties are: Owner: Anchorage: Purchasing Officer C. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 9. Jurisdiction: Choice of Lay. Any civil action arising from this Memorandum of Understanding shall be brought in the Superior Court for the Third Judicial District of the State of Alaska at Anchorage. The scope of this project includes the installation of a 1000 gallon septic tank and two five foot wide drainfields to serve the proposed three bedroom residence for the referenced property. 2. Construction shall be in accordance with the approved site plan and design drawings, Municipal permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The contractor shall be responsible for obtaining any necessary underground utility locates. 4. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. On all leachfield mound systems, the property owner shall be responsible for ensuring a satisfactory vegetation growth over the mounded area. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. 1. A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall include two 4" cleanouts for pumping access. 2. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 4. Septic tanks installed with less than 4' of cover shall be insulated. Page Two Voyles Subdivision; Block 2; Lot 8 August 06, 1994 5. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10' from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. 6. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. 15ke 1140100 o- r: r • • 1. Excavate the proposed trench to the dimensions shown on the design. The bottom of the excavation shall be within 2 inches of level. If the sidewalls of the excavation become smeared, they must be raked or scratched (roughed -up) before gravel (sewer rock) placement. 2. Once the gravel is installed, the distribution pipe is to be installed level with the perforations faced downward. Gravel is then to be placed over the distribution pipe to provide a minimum of 2 inches of cover over the pipe. 3. A silt barrier must be installed between the final gravel layer and the native soil backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. 4. Monitor tubes shall be of four (4) inch diameter and installed approximately in the locations shown on the design. The portion of the monitoring tube extending through the gravel shall be perforated from the bottom of the trench to the invert of the distribution pipe. This is equivalent to the effective depth of the gravel as noted on the design. 5. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finish grade over the trench must be mounded to prevent the formation of a depression after settling. 1. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140/N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. 6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the #200 sieve. 7. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements. Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: 1. The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. 2. The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. Often there will be more than these 3 inspections required, especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre -construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractor's activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractor's activities. Final acceptance of the contractor's work rests with the owner and the M.O.A. S & S Engineering shall have no liability to the owner or to others for acts or omissions of the contractor or any other persons performing work on this project or the failure of the contractor to carry out the work in accordance with these construction documents. It was determined that there were inadequacies in the as -built as to the effective depth of the gravel. The as -built drawings showed the gravel depth to be 24" while monitor tube measurements taken in the field indicated a gravel depth greater than 24" (by 3" to 4') in some locations and less than 24" (by 3" to 4") in others. Bragaw• Street Anchorage, Alaska Re: Lot 8, Block 2, Voyles Subdivision, Chugisk, Alaska Dusty Tinley, Seller (lentlemen: This letter serves as a report of the inspection and adequacy teat performed on the wastewater disposal system at the above referenced property. As -built documents on file with the Municipality of Anchorage (M.O.A.) show the system, constructed in 1994, consists of a 1,000 gallon, two-comparhnmt, steel tank and two 5 -feet wide by 70 -feet long trenches with an effective depth of 24 -inches of gravel. During the course of our inspection and testing, four visits were trade to the property. The first visit was on February 2, 2002, at which time some of the clean -outs and monitoring tubes were not exposed. The test was postponed until the owner of the property could have the monitor tubes located and exposed, but a reading was taken at the tank and monitor tube 01 on the line closest to the road. The water levels were measured at 48 -inches in the tank and 13 -inches in the monitor tube. (See attached drawing) A copy of the as -built drawing was left with the owner to aid in locating the balance of the monitor tubes and clean -outs. A second visit was made on February 8, at which time the clean -outs and mtmitu6ng tubes were still not completely uncovered. Measurements taken at this time indicated the line farthest from the road had 25 -inches of water in monitor tube #3. A phone call was made to Dan Roth at the MOA to see if the test could be conducted with one line being essentially full. We were advised that the test could be conducted, on one trench, as long as it met the three-bedroom absorption requirement of 450 gallons in a 24-hour period. Thereafter 509 -gallons of water were introduced into the leach field. The water level in monitor tube #1 was 17 -inches at the end of the test. During the course of the test, it was discovered that the owner had uncovered monitoring tube W2, but an initial reading had not been taken prior to beginning the test procedure. The reading at the end of the test was 0.112 -inches. A 24-hour reading was taken on February 9 and the absorption requirement was not met. Because the water level in the trench was only raised to 7 -inches below the perforated pipe invert, it was decided to try one more time to see if absorption could be met by taking the water right up to the bottom of the invert pipe and fully utilizing the upper portion of the leach field. A third visit was conducted on February 10, at which time all available pipes were uncovered and elevation shots were taken to confirm if they met with the as -built (locurnent5. There were some discrepancies between the effective depth of the system shown on the as -built drawings and what we treasured in the field. This was brought to Jeff Poet's attention at your office. February 25, 2002 test, 990 -gallons were introduced, bringing the water level up to the bottom of the invert pipe in the line closest to the toad. A 24-hour reading was taken on February 11, which showed the total absorption of 416 -gallons. Mr. Finley rc2li7ed that his automatic purge had taken place on his filtering system that would have released water and impacted the test procedure. It was decided at that time M try another test offer having the tank and leach field pumped so that incidental water usage in the house would not impact the test. Readings were made from time to time during the week after the tank was pumped to monitor the level of all four monitor tubes. The fourth visit was made on February 17, with water level readings in monitor tube #1 at 13 - inches, monitor tube #2 at 6 -1/4 -inches, monitor tube #3 at 20 -3/8 -inches, and monitor tube #4 at 12 -112 -inches. A total of 1,314 -gallons of water was introduced into the leach field closest to the road, containing, monitor tubes #I and #2. A total of 627- gallons were introduced in the leach field farthest from the ruad, containing monitor tubes #3 and #4, for a total of 1,941 -gallons of water into the whole system. it was decided to use both lines after discovering through the field elevation measurements that there were approximately 3 -inches left from the original measurement to the bottom of the invert pipe. A reading was taken 24 -hours later, showing a total absorption or 326 -gallons in the line closest to the road and a total of 266 -gallons in the line farthest from the road, for a total absorption of 592 gallons in a 24-hour period. Our testing and observations in the field indicate that the trench farthest from the road functions in approximately the top 3 -incites of its effective depth. The trench closest to the road functions in approximately the top 16 -inches of its effective depth. However, there is no guarantee as to the remaining life of the system due to unforeseen acts of God such as earthquakes or fractures, or the improper use of the system by future owners. The system passed at the time: the final test was performed. It is important that a probTct live buyer realize the poor absorption quality of the soil that was determined by percolation tests at the time of the original design of the system. The percolation rate of the on-site soils was measured at 53 -minutes per inch with groundwater found at 9 -fat in Test Hole C. Test Hole #2, which was used for the design of the system, had a percolation rate of 50 -minutes per inch with groundwater found at 10 -feet The maximum allowable percolation rate is 60-minute3 per inch.
