If this was a well permit and the well has been drilled, a well log must be sent to this office for documentation of the installation and to close the permit. If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to this office for review, approval and documentation. This as -built inspection report must be signed by the licensed Professional Engineer who inspected the installation of the system. As -built inspection reports are required to be submitted within 30 days of the completion of the system. Box 196650 Anchorage, Alaska 99519-6650 Ref: Proposed Septic Well and Septic System for Lot 2; Stoneview Subdivision To whom it may concern: The proposed 3 bedroom house will be served by a private well and septic system. Two test holes were excavated on the property, one by AKWWC, Inc and another by Eugene Fisher P.E. The drainlield will be designed around the 30 foot radii of these test holes. We are proposing that a 1000 gallon septic tank and a 5 -wide type drainfield be installed. Comments regarding the design are summarized as follows: 1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring, and the percolation test results. It is our opinion that an application rate of 0.8 gallons/day/fl2 should be used. It is our opinion that the insitu sandy soils will act as a sand filter. 2. TRENCH DESIGN: a. Percolation Rate: <1 & 3.3 minutes/inch b. Proposed Application Rate: 0.8 gallons/day/f12 c. Number of Bedrooms: 3 d. Design Flow: 450 gallons per day e. Minimum Absorption Area: 563 ft2 f. Total Depth: 6 feet (max.) g. Effective Depth: 3 h. Width: 5 feet i. Reduction Factor: 0.58 j. Minimum Length: 66 feet long k Effective absorption area = 568 ft2 3. SURFACE WATERS: There is a creek running along the south end of the property. The proposed septic system is to be installed 100+ feet from the creek. TOPOGRAPHY: The area for the proposed drainfields is relatively flat. In short, there are no slope concerns. 5. LOT LINE WAIVER: We request a 1 foot lot line waiver to the west lot line and a 2 foot lot line waiver to the north lot line.
