The well is actually located 19' south of the north property line and 35' east of the west property One. Tobben Spurkland in several drawings Indicates the well is located much further south. The actual location will not allow placement of the new.septic system on Lot 3 as originally planned. We have therefore revised the location of the new septic system as shown on the attached Site Plan. A new .Area Map along with test hole logs and percolation information are also attached. A formal submittal will be made in the near future. Please revise the septic system construction permit for Lot 3, Block 1, Campbell Point Subdivision to allow placement of the new septic system as shown on the revised Site Plan. BOX 240773 ANCHORAGE, AK 99524 April 7, 2005 Municipality of Anchorage Development Services Department On -Site Water and Wastewater Program 4700 South Bragaw Street Anchorage, AK 99519-6650 Subject: Lot 3, Block 1, Campbell Point Subdivision Septic System Design and Permit Application Impacts to Adjacent Properties Dear Onsite Services Engineer: The owner of Lot 3, Block 1, Campbell Point Subdivision intends to construct a new four-bedroom home on the property. We are therefore requesting a permit be issued for the construction of a new septic system to serve the home. Water will be furnished from the Class C Well on the adjacent lot. A waiver has been issued from ADEC allowing the placement of this well with the protective radius waivered to 100'. A copy of the waiver approval is attached for your review. No conflicts exist between the proposed septic system and the wells or septic systems on adjacent properties. The Site Plan and backup documentation identify the location and configuration of the proposed septic system. Existing drainage arrows and the current drainage pattern are also identified on the Site Plan. The drainage pattern will be maintained after development of the new system is complete. Test holes placed near the location of the new absorption trench and alternate site revealed silty sand and gravel to depths of 16' to 17'. The accepting soils were tested and found to have a percolation rate of between 1 and 18 minutes per inch. The average application rate for design purposes was set at .8 gallons per day per square foot. No groundwater was found during excavation and none was noted during the monitoring period. Our design includes the placement of a new 1,250 -gallon septic tank along with a new 55' long by T wide absorption trench with a 7' effective depth to collect and treat the sewage generated in the house. The bottom of the trench will be 11' below the ground surface. The distribution pipe will be 4' below the surface. A minimum of T of cover will be placed atop the bed and a minimum of 4' of cover will be placed atop the new septic tank. The ground surface on the lot slopes from the middle of the lot both east and west at Lot 3, Block 1, Campbell Point Subdivision April 7, 2005 Page Two grades approximating 2% to 8%. The new absorption trench will be constructed parallel with the slope of the ground surface as much as possible in conformance with Municipal requirements. The new septic system will be constructed 100' from all wells and 100' from any surface water. It will also be constructed a minimum of 10' from any water service in the area. If the system is constructed in accordance with our design the following statements apply: 1. The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. DATE 44 ONSITE WASTEWATER DISPOSAL SYSTEM CONSTRUCTION AND MATERIAL SPECIFICATIONS SUBJECT: LOT 3, BLOCK 1, CAMPBELL POINT SUBDIVISION GENERAL: 1. The scope of this project includes the procurement and placement of a new 1,250 -gallon septic tank at the location shown on the Site Plan. Work also includes the construction of a new 55' long X 3' wide X 7' effective depth absorption trench at the location also shown on the Site Plan. The distribution line in the trench will be placed at 4' below the existing ground surface. Total depth of the trench will be 11' below the existing ground surface. The new absorption trench and septic tank must be placed a minimum of 100' from the wells in the area. A minimum of 100' must separate any surface water in the area and the septic tank and absorption trench. The trench must be a minimum of 10' from the lot line and both the well and septic tank must be at least 10' from the water service line. 2. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The Contractor shall be responsible for obtaining all underground utility locates and for the layout of the septic system and verification of the location of all lot lines. The Contractor must also verify the location of wells in the area and assure that the separation distances are met. 4. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. Property owner shall be responsible for revegetation of affected areas unless specifically agreed otherwise. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Department of Health and Human Services for system installations. Owners installing their own systems must receive prior approval from D.H.H.S. before beginning system installation. SEPTIC TANK INSTALLATION A new 1,250 -gallon septic tank must be procured from an approved source and installed at the location shown. 2. A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall include two 4" cleanouts for pumping access. Lot 3, Block 1, Campbell Point Subdivision April 8, 2005 Page 2 of 3 3. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 4. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade. 5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial insulation. 6. A foundation cleanout shall be installed one to four feet from the building foundation. Two cleanouts are required between the tank and the drainfield. 7. Final grading over the tank shall be such that a positive slope exists away from the septic tank. DRAINFIELD CONSTRUCTION: 1. The drainfield shall be constructed to the dimensions shown on the design. The bottom of the trench shall be within 2" of level. 2. Distribution piping must be placed level with perforations down atop a level bed of drainfield rock. Rock should then be placed over the pipe to provide a minimum of 2" of cover. 3. A silt barrier or geotextile fabric must be placed between the drainfield rock and the natural soil backfill. 4. Monitor tubes must be 4" in diameter and installed at the locations shown on the design. The portion below ground must be perforated. 5. Contractor shall verify the septic tank and drainfield area minimum 100' away from any private water wells in the area, 150' from a Class "C" Well or 200' from any community well. 6. Direct bury insulation must be placed over the distribution system if less than 3' of backfill depth is available. Finish grade over the trench must be mounded to prevent settlement or depressions. 7. Grade area surrounding the absorption trench to drain away. 8. A minimum 2' of accepting soil is required below the drainfield rock for a 5' wide trench. Contractor shall verify this condition prior to placement of the rock. (perforated and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662 or A.B.S. (perforated and solid). Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Co. Styrofoam HI or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco, or equal). 5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed between the final drain rock layer and the native soil layer. 6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the #200 sieve. INSPECTIONS: Municipal Ordinance requires a minimum of two inspections. These inspections must be conducted under the supervision of a professional engineer registered in the State of Alaska. The first inspection must be conducted after the excavation of trenches, beds or pits and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled. The second inspection must be conducted after the placement of the geotextile fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No backfill should be in place at the time of inspection. The four bedroom single family home on Lot 3, Block 1, Campbell Point Subdivision will be reviewed by the Municipality of Anchorage's Onsite Services for approval of the wastewater system. Waivers are authorized as shown below. They are based upon the engineer's recommendation, well logs, drinking water sample results, and past waivers that had also been granted. The separation distance requirement of 150 feet between the Class C source well and the identified part of the treatment and disposal system are reduced as follows: The new 1,250 gallon septic tank is reduced to 132 feet, The on-site soil absorption field is reduced to 126 feet, Any future replacement field which is less than 150 feet must obtain a separate waiver at that time. Theses approvals do not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova Street, Anchorage, Alaska 99501, within 15 days of receiving the decision. Adjudicatory hearings requests must be delivered to the Commissioner of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, P.O. This subdivision has small lots which make meeting the required 150 separation distance impossible between the Class C well and septic systems located on Lot 1 Block 1, Lot 2 Block 1 and Lot 2 Block 2. Based on the following, this waiver is approved: 1. The newly constructed well will meet minimum construction standards. 2. The other wells in the area are a minimum of 267 feet deep and extend through a clay confining layer. 3. The anticipated well flow and associated drawdown is comparatively low, meeting the water demand of 2 single family homes. 4. The onsite septic systems involved will be approved by the Municipality of Anchorage Onsite Program. This approval is for a waiver of required separation distances and the placement of the proposed well only. In order to obtain final operational approval you must complete a "Class C Well Inventory Form". This form may be obtained online at: http://www.state.ak.us/dec/eh/docs/dw/Classc.pdf. Finally, due to the high development densities in this subdivision, it is recommended that this proposed system complete annual nitrate and total coliform bacteria testing.
