Ref: Waiver Request and Health Authority Approval for Campbell Point Subdivision; Lot 9, Block 2.
The new 4 bedroom house is served by a private well and septic system. A design was submitted to your
department on 12/21/04 showing the well in the extreme northeast corner of the lot. The well was then
drilled on 5/2/05 by Alpine drilling. At the time of as -built documentation it was found that the lot lines
showing on the GeoNorth MOA plat maps are scaling approximately 8 feet longer than the actual width
of the lot. The well was staked by a certified land surveyor and the well was drilled approximately 88
feet from the septic tank located on Lot 8, Block 2 and approximately 95 feet to the drainfield to the
property to the south. We request you grant a 88 feet separation distance waiver from the well to the
septic tank on Lot 8, Block 2; and a 95 feet separation distance waiver from the well to the drainfield on
Lot 8, Block 2,. The following items are justification for the waivers:
The lot slopes downhill from the well to the septic tank and drainfield. If the septic system was to
overflow, it is not possible for the effluent to travel toward the well head.
The location of the septic system is in a very visible area so that if any effluent was to surface, it
would be noticed and the problem corrected.
The other path of contamination is subsurface migration of wastewater should the tank begin to leak. As
can be seen on the attached well logs, the static water levels range from 150-240. The geological profile
of the subject and surounding wells consists of clay, silt, and sandy soils that have served to inhibit the
migration of untreated wastewater into the aquifer. Recent water sample results indicated nitrate levels
to be 0.1 mg/L and coliform bacteria results to be undetected. Based upon the aforementioned facts, it
appears that there is minimal risk associated with the 88 feet and 95 feet separation distance waivers.

General
1. Total coliform, other bacteria, or nitrates were not detected for the water sample collected from the well.
2. At the time of as -built documentation it was found that the lot lines shown on the GeoNorth MOA plat were
Inaccurate. The well on the subject property was inadvertently installed within 100 feet of the wastewater
disposal system on Block 2, Lot 8. The well was completed on May 7, 2005 by Alpine Drilling. The well is 301
feet in depth with solid steel casing to 301 feet. A static water level of 230 feet was measured in the well at the
time of completion.
3. The wastewater disposal system on Block 2, Lot 8 was installed in September 1981. The absorption field is
36 feet in length, with an effective depth of 7 feet, and a total depth of 16 feet from original grade. The septic
tank is constructed of steel with a capacity of 1,250 gallons.

The proposed 4 bedroom house will be served by a private well and septic system. Two test holes
where dug on the property. The drainfield will be designed around the 30 foot radii of these test
holes. We are proposing that a new 1250 gallon septic tank and also a deep trench type drainfield
be installed. Comments regarding the design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. The natural insitu sandy soils will act as a sand filter. Attached
are drilling logs for surrounding wells, which show natural sand and clay lenses that should serve
to protect the auquifer.
2. TRENCH DESIGN:
a. Percolation Rate: <1-1.3 minutes/inch
b. Proposed Application Rate: 1.2 gallons/day/R2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 500 W
f. Total Depth: 10 feet (max.)

h. Width: 2.5 feet

3. SURFACE WATERS: There are no surface waters within 100+ feet of the proposed septic

4. TOPOGRAPIIY: The average topography in the area of the proposed drainfield is
approximately 10-15% running northeast to southwest. In short, there are no slope concerns.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identilable features. The operational life of all welts and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.

In conducting this evaluation, GEG, LID. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the Lima of the test, and separation
distances measured to readily identifiable features. The operational lite of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory lost
results do not guarantee future performance of the system, nor do they guarantee that
there are no Bidden defects or encroachments. GEG, LTD. can therefore not provide
