Dear Mr. Cross:
The referenced property represents an undeveloped 40,043 square foot parcel. The proposed four (4)
bedroom, single family dwelling on the lot as shown on the attached site plan, Plate 2. Our design
data is summarized in the following dialog.
1. SOILS: Attached is a copy of the soil log/percolation test data and Site Plan. The log
documents a Silty Sand and Gravel (GM -SM) and a Sandy Gravel with Silt (GP -GM)
soil profile from a depth of 2 feet below the ground surface to a depth of 8.0 feet.
This is followed by a Silty Sand with Gravel (SM) down to a final test pit depth of
15.5 feet. The water table was encountered at a depth of 10.8 feet below the ground
surface after monitoring through 9/28/04. The receiving soil has a percolation rate
26.7 minutes per inch.
2. NVIDE DRAIN FIELD TRENCH DESIGN:
a. Percolation Rate: 26.7 minutes per inch.
b. Allowable application Rate: 0.6 gallons/day/R?

c. Number of Bedrooms: 4.
d. Design Flow: 600 gallons per day (at 150gallons/bedroom).
e. Minimum Absorption Area: 1,000 s.f.
f. Max Total Depth (below original grade): 5.5 ft. (3.0 R. cover with 4" rigid
insulation, 2.5 ft. sewer rock).
g. Width: 5 ft.
h. Minimum Length: two trenches at 64 feet per each at 10 feet on center.

There are no surface waters within 300 feet of the proposed septic system. There are
no streams or drainages on the property.
4. TOPOGRAPHY:
The septic site slopes to the south west at 3 to 5, percent for 100 feet. There are no
slopes exceeding 25% in the immediate area
5. IMPACTS:
On the basis of our site visit and the available site information, there are no current
well and septic developments within 125 feet of this system. Accordingly, there will
be no adverse impacts on adjacent wells or septic systems nor restrictions to future
development of adjacent lots.
6. WELL and SEPTIC INSTALLATION:
The well installation was completed March 1, 2005 bySullivan Water Wells (copyof
well log attached). The septic system designed herein shall be constructed in
accordance with the most recent AMC 15.65 by an approved septic system installer.
The installations shall be completed in accordance with the requirements of the On
Site Well and Septic Permits as well as our documents attached. The overall Eaton
Park subdivision development plan has already been submitted to the DHHS

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply andlor wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. 1 further verify -that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.

A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
4 of Eaton Park subdivision. This inspection revealed a nitrate concentration
of 5.78 milligrams per liter (mg/L) was reported for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 mg/L for public drinking
water systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Please see the
attached "Nitrate Fact Sheet" for important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
