Certificate of On-Site Systems Approval # OSC 181026
A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 5, Lot 15 of
John Wells 1952 Addition Subdivision. This inspection revealed an arsenic
concentration of 33.6 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On-Site Water and Wastewater Program
website (www.muni.org/onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.

In conducting this evaluation, GEG, UD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the.
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of ail wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for

Certificate of On -Site Systems Approval # 131153
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 5, Lot 15 of
John Wells 1952 Addition Subdivision. This inspection revealed an arsenic
concentration of 25.2 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On -Site Water and Wastewater Program
website (www.muni.org/oqsiLe) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Septic Tank Data
The septic tank on John Wells S/D Block 5 Lot 6&15 was installed in
1982. The permit was issued by the Onsite department with a
permission to place the tank with a 70' horizontal separation distance.
However the tank was placed 55' away. Even with the permission by
the department of 70' the tank would have encrouched other wells in
the area in particular Lot 14 & Lot 16. The tank and leachfield
installed in 1982 that encroached three wells has been in existence
for over 20 years. Watkins Engineering took water samples for each
of the encroaching wells. And all three tested with -in Municipal
standards.

John Wells S/D Block 5 Lot 6&15 does not have a well log to be
found. However Watkins Engineering during their Health Authority
Inspection determinde the well to be constructed in 1956, a total
depth of 186' with casing to 186', satic water at 63' and a flow rate of
4.2 gpm. And water samples tested to be 0 coliform, .1 mg/I nitrates,
and 1 other bacteria, which is with -in Municipal standard. Other well
logs in the area shows there is a confined aquifer with an
impermeable layer of blue clay and hardpan at 40 ft to 140ft. Using
averages and Lot 18 well log I determined 83 feet of impermeable
layer.

Waiver Request for Septic System Wells
To Whom It May Concern:
The existing home on John Wells Bik 5 Lot 15 is served by a well on Lot 15 and a septic
system on Lot 6, which was approved and installed in 1982. The original septic system
was abandoned at that time. The house was built in 1956, and the well was drilled at
that time. The adjacent home on Lot 16 has the well on Lot 16 and septic system on
Lot 5, installed in 1974. The well and septic system for the house to the west, on Lot
14, are undocumented. We respectfully request a waiver for the separation distance
from septic system components to wells, as follows:

Please see the attached site plan for locations of wells and septic systems.
No well logs are available for Lots 14 and 15. There is a request for approval of the well
and septic system for the Lot 15 (the subject lot) from the Greater Anchorage Bureau,
showing the well at a depth of 186 ft. A copy of the request is attached. This depth was
confirmed by the owner, when he changed out the well pump in about 1997. The well
log from Lot 16 is attached, showing a depth of 154 ft. The soil descriptions of "silty
gravel" are inconclusive as a barrier; however, the static water level of 70 ft indicates
that this is a confined aquifer. Three other well logs from the area are attached, with
well depths of 128 ft, 152 ft, and 162 ft. These logs show a layer of blue clay and
hardpan at 40 ft to 140 ft. This layer of Impermeable silt creates a barrier, which
protects the aquifer from septic effluent. This is substantiated by the low static fluid
level in the Lot 15 well of 63 ft on May 3, 2005.

The Lot 14 well is located in a pit, which is the probable cause of the "other bacteria" in
the sample. Had there been septic effluent contamination, nitrates and coliform bacteria
would be present. Wells on Lots 15 and 16 were clean.
A site plan is attached showing ground level elevations at the septic system on Lot 6
and the three wells. The direction of flow at the surface from the Lot 6 soil absorption
system would be down the driveway and into the street, rather than towards the well on
Lot 14. The direction of flow from the septic tank would be into the basement of the
house, before flowing towards the wells. This condition would be noticed and corrected
quickly.
Due to the forgoing facts, I believe that the proposed reduced separation distances
would protect human health and the environment. Therefore, I recommend approval of
this waiver request.
