The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)
Certificates are valid for one year for properties served by Class A or B wells or a public water system. The
Municipality of Anchorage is not responsible for errors or omissions In the professional engineer's work.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Health Authority Approval Guidelines for this application, shows that the on-
site water supply and/or wastewater disposal system is(are) safe, functional and adequate for the number of
bedrooms and type of structure indicated herein. I further verify that based on the information obtained from the
Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or

ENCROACHMENT NON -OBJECTION AND AGREEMENT
THIS AGREEMENT, made and entered into this day of . 20DA, by
and between MATANUSKA ELECTRIC ASSOCIATION, INC., an Alaskan non-profit
member owned electric cooperative of Palmer, Alaska, hereinafter tailed •First Party',
and Trmothv A. Banach of P.O. Box 951, Dlginoham. AK 99576, Alaska, hereinafter
called 'Second Party', WITNESSETH THAT:

NOW, THEREFORE: for and in consideration of their mutual agreements and
other good and valuable consideration, the receipt of which Is hereby acknowledged,
First Party, for Itself, its successors and assigns, waives its objections to Second Partys
encroachment on the said easements for the purpose mentioned, subject to the
following conditions:
1. Second Party will hold First Party harmless from liability for arty Injury
or damage to any person or property which may result from such encroachment
2. The said encroachment will be without prejudice to First Party's full
enjoyment of any and all rights it may have in and to such easements.
3. Any facilities or property of First Party which are damaged or destroyed
as a result of such encroachment vAl be repaired or replaced. The cost of repair or
replacement will be paid entirely by Second Party.
4. Second Party will comply with all applicable safety codes and
regulations.
5. Any expenses which First Parry may Maur In the relocation or
modification of its faculties to accommodate the said encroachment will be paid entirely
by Second Party.
By Its execution of this Agreement, Second Party agrees, for Itself, its
successors and assigns, that its encroachment upon the named easements as
heretnabove mentioned wig be subject to the foregoing conditions.
IN WITNESS WHEREOF, the parties have caused this Agreement to be
executed by their duty authorized representatives and agents as of the day and year
first above mentioned.

By this document Matanuska Telephone Association, Inc. (MTA) declares that it has no objection to the
encroachment of a water line within the platted 10' Utility Easement upon Lot 2, Thunderbird Falls
Subdivision, Anchorage Recording District, Third Judicial District, State of Alaska, situated in Section 36,
T16N, R1 W, S.M.
Please be advised that MTA through the issuance of this document does not forfeit any of its rights to the use
of the area cited. In the exercise of these rights MTA will, if needed, upgrade, maintain, repair, and/or replace
buried or aerial telecommunications facilities within the easement. Any repairs that may be required to the
encroachment as a result of utility construction will be bome by the property owner of record. This document
does not authorize the placement of any additional encroachments within the easement area. Property owners
are required to obtain utility locates before doing any kind of work in the utility easements and will be liable
for any damages caused by their construction work in the easements.
This document is in no way, an aereement to vacate any portion of the utility easement and should not be
intemreted as such.

Dear Property Owner:
The Anchorage Water & Wastewater Utility received your request for a letter addressing
AWWU's position regarding the existing well's service line that encroaches into utility
easements located on the referenced lot. An as -built drawing with well location, service
line location and easement locations was submitted with the request.
AWWU water and sanitary sewer mains are not available to the referenced lot.
AWWU is not using the utility easements and has no objection to the encroachment.
