RE: SEPTIC SYSTEM UPGRADE PERMIT
LEGAL: GRECIAN HILLS BLOCK 1 LOT 4
The property owner has requested we obtain a permit to upgrade the failed absorption field of the
existing Advantex septic system of the above referenced lot. We propose to install a diverter to
a new elevated trench using 2’ of MOA sand and increasing the length compared to the existing
trench. This elevated trench with sand & length should create more effluent absorption.
The soils logs, percolation rates & groundwater monitoring of the existing records of Michael E.
Anderson, PE, Jeffery A. Garness, PE & Steven R. Pannone, PE found in the MOA files have
been utilized for the proposed design. No groundwater was observed in any of these test holes
at excavation or monitoring.
The slopes are moderate at 5-10% at the proposed upgrade location. The lot and area are served
by private water. The design will not impact any of the neighboring properties. Please contact
us if you have any questions.
Sincerely,
Curtis Huffman, P.E.

Attached is the cross section and design criteria for the absorption trench to serve the
Advantex System on Lot 4, Block 1, Grecian Hills Subdivision. The total trench depth
will be 9'. Our recent test hole was placed to 17.5' with no groundwater encountered.
The test hole placed by Pannone Engineering indicated groundwater at 14' below the
surface on May 12, 2005. The bottom of the proposed trench will be 1' above this
elevation.

Attached is the test hole log and percolation test result from the exploration done on the
subject lot last week. It indicates subsurface conditions in the location or the absorption
trench and alternate site are suitable for construction. Please review the results and
advise if you have questions.

The absorption trench is in the same location as the originally designed bed and within
the 30' radius of the Pannone Test Hole. We verified the percolation rate by performing
our own percolation test as shown on the attached soils log. Another test hole is
currently being completed to verify the location of the alternate site.
Pannone's test hole was monitored for groundwater through May of last year and does
not indicate a problem: Based on all information available for this site we feel an
absorption trench is much more desirable and effective to dispose of the septic effluent
from the Advantex system_

No. 3444 F. 1
DATE: April 6, 2007
TO: Joe Goodall
FROM: Mike Anderson, P.E.
SUBJECT: Lot 4, Block 1, Grecian Hills Subdivision
Septic System Permit No. SW060095
We recently visited the site of the new septic system which was designed by Steve
Pannone, P.E. His test hole log shows no groundwater encountered with silt (ML)
underlain by silty gravel (GM). The design calls for an absorption bed to be constructed
in the ML layer. The percolation rate determined in the ML layer was 60 minutes per
inch. We are proposing to place a 5' wide by 4' effective depth absorption trench in
lieu of the bed. It will be constructed partially in the ML layer, but primarily in the GM
layer below, which, we anticipate will percolate faster than 60 minutes per inch.
We will provide you with a percolation test, test hole log and new design in the next few
days. At that time we will request the permit be modified to allow the revised septic
system design.

Well and Septic System Permit Request
Ladies and Gentlemen:
P.O. Box 102954
ge, Alaska, 99510
I am writing to request a permit to construct new well and septic system be issued for
this property. The proposed systems will serve a proposed four-bedroom house. The
house will sit on Lot 4 along with the well, and the septic system will be located on lot 4.
The Owner is currently in the processes of vacating the lot line between these two lots.
The proposed well will be constructed in the northwest corner of lot 4.
I. Soils. See the attached soils log. Ground water was monitored through the
seasonal high period of 2004. Ground water was measured in the monitor at a depth of
5.5 feet below ground surface. Bedrock was not encountered in the test hole. It is my
opinion that the overall soils appearance of the soils, an application rate of 2.0
gallons/day/square feet should be used using a class III advanced treatment system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
A drainage ditch is located north of the proposed system over 200 feet away along the
north side of Upper DeArmoun Road. The proposed systems will maintain at least 100
feet from all surface water and drainage ditches.
4. Topography: The average topography in the area of the proposed septic system is
approximately two percent in the area of the SAS.
5. Future Upgrade Area: I am proposing that the primary field be installed at this
time. A new 1500 gallon Advantex advance treatment system will be installed outside
the 100 -foot well radius. I have shown the proposed location on the site plan.
The proposed installation will not affect the future development of the surrounding or
existing lots. There are no wells or septic systems within 100 feet of the proposed well
location or of the proposed septic location.
If you have any questions or concerns, please contact me at 227-3522 or 272-8218.
Sincerely,
Steven R. Pannone, P.E.

3.3 INSTALLATION
A. Septic Tank
1. After excavation the tank shall be placed on undisturbed soil on a level grade
at a depth as indicated on Drawings. Backfill around the tank with
structural fill. Fill shall be installed in 12 -inch lifts, compacted to 95%

hold tightly in place.
c. Heat Shrink Coupling: Cover entire exposed joint plus an overlap of four
inches on pipe coated on either end. Heat in accordance with
manufacturer's instructions.
4. Installation:
a. Install insulation half -shells and heat -shrink wrap at pipe connections.
Maximum gap in half -shells shall be 1/4inch.
b. Lower assembled insulated pipe carefully into trench. Do not allow
joints to deflect greater than the elastic capacity of the heat shrink
sleeves. If sleeves are found to be damaged, reduce the deflection and
repair the damages sleeves.
c. Tolerances:
(1) Lay mains to indicated line and grade. Perform earthwork in
accordance with the Drawings.
(2) Minor Changes in Direction: Pipe deflection will be allowed for
small changes in direction, provided that the pipe bending radius
is less than 30 times the diameter of the outer jacket of the
insulated pipe system.
C. Drain Field
a) The drain field area shall be excavated to the elevation and dimensions as shown
on Drawing. Remove all organic and silt material to the top of the well -graded

COwner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On -Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
