No. 04-01041 Dear Mr. Roth: P.O. Box 2749 Palmer, Alaska 99645 As you know, the owner of the above referenced lot received a building permit for construction of a duplex. After construction had proceeded to the point of installation of the roof of the structure, the municipality admitted their error in issuing the building permit for a duplex in a subdivision zoned for single family dwellings and canceled the permit. Our firm documented the excavation of two test holes on the lot (see attached Soil / Percolation Tests), labeled AKTH#1 and AKTH#2 on the attached site plan. It can be seen that due to soil and groundwater conditions it is unlikely that a conventional septic system can be installed at this site. However, the site is developed, and so based on the site conditions, a holding tank is recommended for the onsite septic system. The tank will consist of a 3,000 -gallon Anchorage Tank holding tank, that can be converted into an Advantex Treatment System tank if necessary for future use. There are other issues with this lot that concern the drainage of surface runoff in the area around this site. The owner, Mr. Jerry Haines, will be working with MOA Right of Way to attempt to fix the problem and hopefully alleviate some of the existing groundwater issues on his lot. Once this drainage issue has been adequately addressed, we will monitor the onsite groundwater monitor tube for groundwater levels. If it is possible to achieve a 2 -feet layer of unsaturated soils suitable for an onsite soil absorption field, we will propose at that time to convert the holding tank into an Advantex Treatment System with discharge to an onsite soil absorption field. Holding Tank Design The following is a summary of the proposed design of the holding tank: Number of bedrooms: 3 Minimum required tank size: 2,000 -gallon Engineers • Planners • Surveyors Mr. Dan Roth, MOA Page 2 January 27, 2005 Proposed tank: 3,000 -gallon Anchorage Tank, coated Total depth, bottom of tank (max): 6.3 ft (tank dia) + 4 ft soil cover =10.3 ft Depth of tank in groundwater. 3.5 ft Ballast: 4 ft soil cover is adequate Topography The topography of this lot is relatively flat throughout the southwest portion of the lot, then gradually sloping down toward the north. There is adequate usable area for an onsite septic system and replacement system. Surface Water There are no surface waters within 100 feet of the proposed holding tank. Well This property will be served by an onsite well. Because of the work that proceeded under the initial building permit, an onsite well was drilled (see attached Site Plan). Apparently there were problems with the well installation which will be repaired. Documentation of the repairs will be submitted to your office upon completion. The location of the onsite well is greater than 100 feet from the proposed location for the holding tank, and also for the existing septic system on the adjacent lot (Lot 9 to the south). Lot 1, Block A, across Banner Circle, and Lots 1 and 2, Block B, to the east, are undeveloped lots. Development on Lot 10 will not preclude development of wells and septic systems on neighboring lots in the future. Well separation distances: See attached COSA form. Well to holding tank is 113', and well to septic line is 106'. 2. Well depths: See attached report by Arctic Pump & Well, Inc. The driller, Jim Sullivan, ran a camera down the casing and confirmed well casing extends to 40'+with no perforations. 3. Riser NE of house: This riser was a groundwater monitor tube. The tube has been removed, confirmed by Mr. Brandon Jones, an engineering technician with our firm. The attached As Built survey confirms this information (see attached). 4. Holding Tank Profile: See attached drawings. 5. Second Tank Compartment: There is an access manhole for pumping the 2nd compartment. 6. Certified Well Driller: The owner hired Jim Sullivan, Arctic Pump& Well, Inc., to correct the deficiencies with this well. The attached report indicates the items addressed by Mr. Sullivan, including: • Removal and disposal of the wooden well house; • Running a camera down the casing to confirm the well is cased to 40' with no perforations; • Installing a new well pump, Franklin Model 10FRDO5P4-2W230, 1/2 HP, to depth 41 ft below top of casing(see attached pump information); • Installing a pitless adaptor, Model B-10, on the casing 1 ft above grade due to artesian aquifer; • Confirming casing is 36"above grade; • Installing sanitary seal; • Placing electric wires into conduit; Septic System Evaluation Page 2 Lake Hill Acres#6 Block A Lot 10 Engineer Report • Disinfecting the well upon completion; • Grouting the casing to 10 ft below the pitless adaptor, as approved by you (see photo); • Placing fill material around the casing in order to achieve surface grading away from the casing for 10 ft in all directions. 7. Holding Tank and High Water Alarm: The holding tank is described on the attached COSA form. The tank is a 3,000-gallon Anchorage Tank lift station with floats but no lift pump. Mr. Brandon Jones, an engineering technician with our firm, confirmed by observation that the floats and the high water alarm are operational. Also included are previously submitted water test results, including Bacti, Arsenic, and Nitrate/Nitrite(see attached). It is our opinion this satisfies the concerns regarding the well on this property.
