We have reviewed our records concerning the placement of the new septic system on
Lot 15, Block 2, Sea Turn Subdivision and find it is located substantially in conformance
with the design documents and the permit. The proposed well location is 100' from the
location of the septic system. The well permit can be reissued for the location shown
on the original permit.
Soil conditions found on the site during construction were as expected dudng the
design phase. The majority of the soils were as noted in Test Hole Nos. 1 and 2. Only
a small percentage were as noted in Test Hole No. 4. We are confident the absorption
rate of .6 gallons per day per square foot is appropriate for the design of the absorption
system.

Separation Distance Waiver
Septic System to Excessive Slope
Dear On Site Services Engineer:.
Surface area available on the subject lot for placement of a septic system is severely
limited by the presence of a stream at the southern border of the lot and the location of
the wells and septic systems on the adjacent lots. The only available area is
approximately 20' from the edge of a slope which exceeds a grade of 25% and drops to
the creek. A waiver is therefore necessary to allow the new septic system to be
constructed within 50' of this break in grade.
The slope varies in grade from 26% to 35% and will be flattened dudng construction to
allow a minimum of 50' from the edge of the pdmary and alternate sites to a break
greater than 25%. A cross section of the finished slope will be furnished with the
codified as built for the septic system verifying the separation distance.
Sincerely,
Michael E. Anderson, P.E.

Lot 15, Block 2, Sea Turn Subdivision
Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:.
The owner of Lot 2, Block 3, Sea Turn Subdivision intends to construct a new four-
bedroom home on the lot. We are therefore requesting a permit be issued for the
construction of a new well and septic system to accommodate the home. The attached
Site Plan and backup documentation identify the location and configuration of the
proposed well and septic system along with the alternate site. No existing wells or
septic systems are closer than 1,00' from the proposed well or septic system. Drainage
arrows are shown on the site plan showing the grade and direction of flow on the lot.
The property drops at a slope of between 25% and 40% at its midpoint toward Rabbit
Creek. Some slope flattening will be required to insure the minimum separation
distance from a slope greater than 25% is attained. The drainage pattern will not be
changed as a result of this project.
The test holes placed on the tract in the area of the absorption trench and alternate site
indicated silty gravel with sand (GM) material. The percolation rate of the soil was
determined to be between 1 and 5 minutes per inch. No groundwater was found during
the placement of the test holes and none was noted during the monitoring period. An
application rate of .6 gallons per day per square foot was used in the design of the
absorption system. We are proposing to place a new 1,250-gallon septic tank and 2-
42' long by 3' wide by 6' effective depth absorption trenches to treat and dispose of the
effluent generated in the house. The elevation of the distribution line in the absorption
trenches will be 4' below the surface and the total depth will be 10'. A minimum of 3' of
till or 2" of insulation and 2' of fill will be placed over the absorption trench and 4' over
the septic tank. The alternate site will be an Advantex Treatment System with an
absorption trench designed for an absorption rate of 3 gallons per day per square feet
or 5' wide x 20' long with an effective depth of 4'.
Lot 15, Block 2, Sea Turn Subdivision
August 20, 2007
Page Two
The ground surface on the lot in the area of the absorption trench is nearly fiat with a
slight bias to the southwest. The new trench will be constructed parallel with the slope
as much as possible in conformance with Municipal requirements. The new septic
system will be constructed a minimum of 100' from all wells and any surface water and
10' the proposed and existing water services in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be cons{ructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.
Sincerely,
Michael E. Anderson, P.E.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 15, BLOCK 2, SEA TURN SUBDIVISION
GENERAL:
The scope of this project includes the procurement and placement of a new
1,250-gallon septic tank at the location shown on the Site Plan. Work also
includes the construction of 2 new 42' long X 3' wide X 6' effective depth
absorption trenches at the location also shown on the Site Plan. The
distribution line in the trench will be placed at 3' below the existing ground
surface. Total depth of the trench will be 9' below the existing ground
surface. The new absorption trenches and septic tank must be placed a
minimum of 100' from all private wells and any surface water in the area and
.10' from the water service line proposed for the lot. A minimum of 3' of cover
Js required over the absorption trench and 4' of cover over the septic tank. A
flow splitter valve must be placed to assure even flow to each trench. Slope
flattening to the south of the absorption trenches will be required. Fill material
must be placed to insure no break in grade greater than 25% is within 50' of
the end of each trench.
Construction shall be in accordance with the approved site plan, design
drawings, Municipal Permit with any special provisions or conditions, and all
applicable State and Municipal Wastewater Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the location
of all lot lines. The Contractor must also vedfy the location of wells in the
area and assure that the separation distances are met.
Unless specifically agreed otherwise, the contractor shall be responsible for
final grading areas subsequently depressed from soil settling. Property
owner shall be responsible for revegetation of affected areas unless
specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified by the
Municipal Department of Health and Human Services for system installations.
Owners installing their own systems must receive prior approval from
D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. A new 1,250-gallon septic tank must be procured from an approved source
and installed at the location shown.
Lot t5, Block 2, Sea Turn Subdivision
August 20, 2007
2. A septic tank is to be constructed by a certified septic tank manufacturer.
Construction shall include two 4" cleanouts for pumping access.
3. The septic tank shall be sufficiently bedded to prevent settling or shifting of
the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches above
final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial
insulation.
6. A foundation cleanout shall be installed one to four feet from the building
foundation. Two cleanouts are required between the tank and the drainfield.
7. Final grading over the tank shall be such that a positive slope exists away
from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfleld shall be constructed to the dimensions shown on the design.
The bottom of the trench shall be within 2" of level.
Distribution piping must be placed level with perforations down atop a level
bed of drainfield rock. Rock should then be placed over the pipe to provide a
minimum of 2" of cover.
3. A silt barrier or geotextile fabdc must be placed between the drainfield rock
and the natural soil backfill
4. Monitor tubes must be 4" in diameter and installed at the locations shown on
the design. The portion below ground must be perforated.
Contractor shall verify the septic tank and drainfleld are a minimum 100' away
from any private water wells in the area, 150' from a Class "C" Well or 200'
from any community well.
Direct bury insulation must be placed over the distribution system if less than
3' of backfill depth is available. Finish grade over the trench must be
mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
A minimum 2' of accepting soil is required below the drainfield rock for a 5'
wide trench. Contractor shall verify this condition prior to placement of the
rock. All pockets of unacceptable materials must be removed and replaced.
Lot 15, Block 2, Sea Turn Subdivision
August 20, 2007
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic tank
manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated and
solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and ASTM D2662
or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct budal polystyrene (Dow
Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
5. A permeable geotextile fabdc (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the
#200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These inspections
must be conducted under the supervision of a professional engineer registered
in the State of Alaska. The first inspection must be conducted after the
excavation of trenches, beds or pits and before the installation of any gravel. A
septic tank may be set in place, but may not be backfilled.
The second inspection must be conducted after the placement of the geotextile
fabric, gravel, distribution piping, standpipes, cleanouts and insulation. No
backfill should be in place at the time of inspection.
~ontractor shall provide a copy of all field survey layout and construction notes
for use in preparing the certified as-built of the completed system.

It is my intent as the homeowner to construct a septic system on the property
6120 Azalea Dr
Lot 15, block 2, Sea Turn sub.
I have 20 years experience operating different types of equipment (backhoes,
excavators, loaders, dozers, boom trucks, eot.). I excavated the foundation, the sewer
and water lines for the house I built and currently live in. I have worked as a lineman for
Norcon and Alaska Communications Systems building and maintaining underground
plant during the past 15 years. I have built one new and rebuilt one other septic system,
both of which are still working fine.
I will be building the system and will be on the property the entire time it is
being placed. I am planning to borrow an excavator from Jason Hodges for the project.
If for some reason that becomes unavailable due to scheduling conflicts, I will rent one
from Airport or United rentals.
The project has been designed by; will be inspected and as-built by Anderson
engineering.

Lot 15, Block 2, Sea Turn Subdivision
Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 15, Block 2, Sea Turn Subdivision intends to construct a five-bedroom
home on the property. We are therefore requesting a permit be issued for the
construction of a new well and septic system to serve the home. The Site Plan and
backup documentation identify the location and configuration of the proposed well,
septic system and alternate site. No conflicts between the new septic system and other
systems in the area have been noted. An existing well is located on the lot to the west
which will impact placement of the septic system. Care must be taken to insure the
100' separation distance is met. Existing drainage arrows and the current drainage
pattern are also identified on the Site Plan. The drainage pattern will be maintained
after development of this lot is complete.
Test holes placed near the location of the absorption trench and alternate site indicated
silty sand and gravel. The accepting soils were tested and found to have percolation
rates of 30 to 60 minutes per inch. No groundwater was found during excavation and
none was noted during the monitoring period. Our design includes the placement of a
new 1,500-cjallon septic tank along with 2 - 54' long by 3' wide by 8' effective depth
absorption trenches to collect and treat the sewage generated in the house. The
bottom of the trench will be 11' below the surface. The distribution pipe will be 3' below
the surface. The alternate site will be an Advantex treatment system to qualify for a
reduction in trench length due to the limited surface area available. A minimum of 3' of
cover will be placed atop the trenches and a minimum of 4' of cover will be placed atop
the new septic tank.
The ground surface on the lot slopes at grades from 2% to 5% from northeast to
Lot 15, Block 2, Sea Turn Subdivision

Page Two
southwest in the area of the absorption trenches. The lot slopes more radically toward
Rabbit Creek at the back of the house. The absorption trenches will be constructed
parallel with the slope of the ground surface as much as possible in conformance with
Municipal requirements. The new septic system will be constructed 100' from all wells
and surface water, It will also be constructed a minimum of 10' from any water service
in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future,
The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.
Sincerely,
Michael E. Anderson, P,E.

the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches above
final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct burial
insulation.
6. A foundation cleanout shall be installed one to four feet from the building
foundation, Two cleanouts are required between the tank and the drainfleld.
7. Final grading over the tank shall be such that a positive slope exists away
from the septic tank.

The bottom of the trench shall be within 2" of level.
Distribution piping must be placed level with perforations down atop a level
bed of drainfield reck. Rock should then be placed over the pipe to provide a
minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfleld rock
and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations shown on
the design. The portion below ground must be perforated.
Contractor shall verify the septic tank and drainfield are a minimum 100' away
from any private water wells in the area, 150' from a Class "C" Well or 200'
from any community well.
Direct bu~j insulation must be placed over the distribution system if less than
3' of backfill depth is available.
