not on the lot at this time. As near as we can ascertain, it was a failure at startup and was either
left and the owner eliminated it or we got rid of it as we had to start over. By failure I mean bent
off in boulders at a shallow depth.

Trying to get a COSA on the subject property and Garness is finishing up the final docs from
the 2004 permit, but he and the MOA both said there was an initial well that was needing to be
decommissioned. All I could see is the one well so I assume you did decommission the well in
question. Jeff Garness thought you had maybe started one and got a bit stuck and started
another.
The original permittee, owner was Westlien Erling. In any case the MOA needs the
decommissioning letter. Please get back to me.

bedroom septic system. Per the homeowners request an additional 1250 gallon septic tank is to

The proposed 4 bedroom house will be served by a private well and septic system. Two test
holes where excavated by ENSR Corporation on the property. The draintield will be designed
around the 30 foot radii of these two test holes. We are proposing that a new 1250 gallon septic
tank and a deep trench type drainfield be installed. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. Attached are several soil boring logs from adjacent lots which
confirm no bedrock or impermeable soil to a depth of 18 feet. A test hole will be excavated on
the property during construction to confirm no bedrock or ground water at 18'. Based upon the
soil logs provided to us, an application rate of 0.8 gallons/day/R2 should be used.
2. TRENCH DESIGN:
a. Percolation Rate: 8.0-6.7 minutes/inch
b. Proposed Application Rate: 0.8 gallons/day/R2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day

3. SURFACE WATERS: There are no surface waters within 100+ feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached topography site plan, the average
topography in the area of the proposed drainfield is less than 20%. In short, there are no slope
concerns.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

1 intend to Install a wastewater disposal system at 7600 Wildwood Circle (Wildwood Glenn Subdivision,
Lot 5) in accordance with the design under approved On -Site Permit # SW040382. I am the property
owner and intend to occupy this site as a residence. This letter is provided to demonstrate my experience
and qualifications as it relates to the system construction.
My professional occupation is In the environmental consulting Industry; I am currently a Project Manager
at ENSR International and manage environmental cleanup projects throughout Alaska. My professional
experience includes several hours of excavation experience as both an equipment operator and in
oversight roles. These projects, including excavating test pits/trenches, underground storage tank
removals, large-scale remedial excavations, and site restoration, have required excellent dirtwork skills,
the ability to accurately transfer designs to ground, and attention to safety. I have also participated in the
construction and maintenance of subsurface remediation systems that Involved plumbing of fixtures and
lines. This overall experience has provided me with the knowledge, skills, and attention to detail
necessary to install a conventional septic system on my property.
In addition, for the past four years I have owned and operated a Caterpillar 931 track -type loader with
backhoe attachment. 1 have regularly used this piece of equipment to conduct site preparation work,
foundation excavation, and road building as an owner -builder. I have become very skilled with this
equipment.
I am familiar with On -Site's system installation requirements and attended the Excavator Class on
February 23, 2005. 1 understand the critical importance of establishing and maintaining elevations for all
septic system components and will ensure that the system is installed in accordance with the permit
requirements.

The Environmental Protection Agency (EPA) has established a maximum

private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water

Nitrate is a negatively charged compound of nitrogen and oxygen, which is very soluble in water. Nitrate
is not readily filtered or otherwise removed in the soil and can pass rapidly into ground water wells.
SOURCE: Nitrate is a major component of fertilizer and wastewater. Often the nitrate is in the form of
ammonia or protein first, which through contact with oxygen and certain bacteria, converts to the
oxidized form known as nitrate. Sources of nitrate from wastewater include urea, ammonia cleaners,
food solids, and bacterial cells. It may also result from the breakdown of organic matter buried in the soil.
TOXICITY: Nitrate is generally not toxic to adults or children over the age of two or three years but is
associated with a potentially fatal infant disease called methemoglobinemia. In the digestive system of
young children, nitrate converts to nitrite, which can pass through the intestinal wall into the blood
stream. There it combines with the hemoglobin and interferes with the ability of the blood to carry
oxygen. For this reason, methemoglobinemia is referred to as "blue baby" disease. The EPA limits the
concentration of nitrate in public drinking water supplies to 10 mg/L. The standard has been lowered
from a previous level of 45 mg/L set by the US Public Health Service and the World Health Organization.
TREATMENT: due to its solubility in water and negative ionic charge, filtration and other common home
water treatment systems such as softening, or iron filtration does not readily remove nitrate. The best
method for limiting nitrate in well water is source control. This can include avoiding overdosing of
fertilizer near the well and maintaining good separation distances between septic tank leach fields and
the well. A special anion exchange filter that contains a media with a strong affinity for negatively charged
ions in water, or by a reverse osmosis treatment system or distillation can remove nitrate.
TESTING: Nitrate analysis is usually done by one of the several "wet chemical" methods using a
spectrophotometer to read the final color endpoint. Specific ion electrodes also can be used to detect
the activity of nitrate in water. This laboratory uses several different wet chemical methods approved
under the public water supply laboratory certification program. They also have test kits available, which
the laboratory uses to perform an inexpensive "screening test", and with which the homeowner can
monitor the change in nitrate levels from their well. They recommend comparing the test kit results
against a certified analysis from the lab occasionally to verify the accuracy of the kit. We recommend
using a specially prepared bottle that has been rinsed in hydrochloric acid for collecting samples.
