Dear Mr. Craig:
An On -Site Water/Wastewater Permit, number SW050389 issued by this office for a single-family
system, will expire on 07 October 06. Permits are valid for 365 calendar days from date of issue.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

To whom it may concern:
The proposed 3 bedroom house is to be served by a private well and septic system. The well is to
be drilled in the area shown on the attached site plan and design drawings. Two test holes were
excavated on the property. The drainfield will be designed around the 30 foot radii of these test
holes. We are proposing that a new 1000 gallon septic tank and dual 5 -wide trench type
drainfields be installed. Comments regarding the design are summarized as follows:
L SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that an application rate of 0.8 gallons/day/!i'
should be used.
2. TRENCH DESIGN:
a. Percolation Rate: 10 minutes/inch
b. Proposed Application Rate: 0.8 gallons/day/ft'
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day

3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached surveyors plot plan, the average slope in
the area of the proposed drainfields is a 5 to 16 percent running northwest to southeast. In short,
there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

In conducting this evaluation, GEG, UD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOR
DSD Guidelines B Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for

engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of Issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
