Hello Jay. This warranty is for the septic system in Paradise Valley with the gabion basket retaining wall that we
just did the upgrade on. I had discussed with you the concern I had as to how A+ Home services installed the
line from the pump vault to the pressure line below the gabion wall. There is a "big" belly in the line that will
retain water and I believe there is a real potential for freezing. The previous owner was able to obtain a
warranty from A+ Home Services. Attached is a copy of the warranty provided by A+ Home Services. The home
has sold, but the new owner is covered by this warranty.

To whom it may concern:
The existing 3 bedroom house is served by a private well ana septic system. The riser on
the existing Advantex tank (fiberglass) has become compromised. Repairing the riser
cannot be done from the inside and an exterior repair cannot be done without shoring -up
a nearby retaining wall. The decision was made to fill the existing tank and pump vault
with concrete and install a new tank and pump vault. The existing trickling filter and
Orenco components will be re -used. The attached drawing shows the proposed location
for the new tank and pump vault.
We are unaware of any adverse impacts this installation would have on adjacent wells or

for Paradise Valley; Block 4, Lot 28
Mr. Crewdson:
A permit was issued to upgrade the Advantex System on April 25, 2013. Today (April
28, 2014), we performed a site visit to perform a septic adequacy test on the existing
drainfield and found the drainfield to be surcharged. We are requesting the referenced
permit be amended to include upgrading the drainfield. Attached is a revised design
which shows the design criteria for the rebuilding of the existing drainfield. The existing
drainfield is a 5 foot wide by 30 foot long. We are proposing existing drainfield be
excavated and that the contaminated material be removed. We anticipate approximately
1 foot of MOA approved leveling sand filter will be added to bring the system back up to
its original elevation and then new drainrock, new 4 inch perforated distribution line with
cleanouts, and new monitoring tubes be installed. We are proposing that the drainfield
be enlarged to 40 feet long by adding 5 feet of new drainfield on each end of the
existing drainfield. It is our understanding that based upon the site visit performed
approximately 2 weeks ago between yourself and me, that the existing slope conditions
would not need to be modified if the drainfield was found that it needed to be upgraded.
The slopes are heavily vegetated and even with the existing drainfield being
surcharged, there was no daylighting of effluent anywhere downhill from the drainfield.
Please amend the permit to include the upgrading of the drainfield.

Ref: Proposed Well and Septic System for Lot 28, Block 4; Paradise Valley Subdivision
ADVANTEXTII TREATIIIENT SYSTEM
To whom it may concern:
1. GENERAL: The proposed 3 bedroom home will be served by a private well and septic
system. Two test holes were excavated on the property. We are proposing to design the septic
system around the 30 foot radius of test hole #1. Due to the limited amount of receiving soils,
and the steep slopes, we are proposing to use an Advantex treatment system and a 5 -wide trench
type drainfield. The well is to be located in the area shown on the site plan.
2. SOILS: See the attached log, which shows the soil classifications, groundwater monitoring,
and the percolation test results.
3. DRAINFIELD APPLICATION RATE: A percolation test was performed in test hole #1
between the depths of 1.5 to 2.0 feet and 3.5 to 4.0 feet and found the rates ranged from 3.3 to 30
minute/inch. The Advantex system has received class III approval and we are going to
conservatively assume an application rate of 3.0 GPD/FV.
4. TRENCH DESIGN:

Note: The distribution line is to be 1 V4 inch pvc pipe with V4 inch holes drilled every 11 inches
on center (30 holes total).
5.ORENCO PACKAGE SYSTEM: The STEP tank with the Advantex Treatment System will
be manufactured by Anchorage Tank & Welding to meet the latest design criteria established by
Orenco Systems, Inc. The point of contact at Anchorage Tank is Lowell McNutt. As with their
standard STEP tank, it is equipped with a high water alarm per M.O.A requirements.
6. SURFACE NVATER: There is no surface water within 100 feet of the proposed septic
system upgrade.
7. TOPOGRAPHY: As can be seen on the attached topography site plan, the proposed
drainfield is to be installed on a 10-20 percent slope. Directly below the proposed drainfield the
topography drops down to a 35-40 percent slope. The contractor at the time of construction will
fill this slope in with fill to create a slope of less than 25 percent 50 feet downhill from the
proposed drainfield and 35 feet from the alternate site. We request a 35 foot waiver from the
alternate site to a slope greater than 25 percent slope.
8. WAIVER TO IMPEARMEABLE SOILS: An impermeable layer was found at a depth of
6 feet in test hole #1. We are requesting that the vertical separation distance between the bottom
of the drainfield, and the impermeable soil layer, be waived to 4 feet. Justification for this
reduced separation distance is summarized as follows:
The effluent being discharged to the drainfield is from an Advantex treatment system,
which has CBOD and TSS levels that are expected to be less than 10 mg/L. In addition,
the total nitrogen levels are expected to be less than the effluent from a lined Intermittent
Sand Filter (effluent from an ISF can be discharged into a drainfield that is only 4 feet
from bedrock and impermeable soils)
There will still be a 4+ feet separation distance between the bottom of the drainfield and
groundwater
There will be no discernable benefit (reduced BOD, TSS, or total nitrogen) achieved by
passing the effluent through 6 feet of soil vs. 4 feet of soil. It is generally agreed upon
that the pathogen reduction occurs within the first 4 feet of soil, hence the 4 feet required
separation distance to groundwater. It is reasonable to assume that by the time the
effluent passes through 4 feet of the insitu 30 & 3.33 minute per inch soil that the BOD
and TSS levels will be essentially zero (0). It is further assumed that there will be no
more reduction in total nitrogen achieved by building the mound several feet higher. The
highly treated effluent will probably not see further reduction in nitrogen levels without
introduction of a carbon source (as is the case with ISF effluent).
Based upon the facts presented we are requesting that the vertical separation distance between
the bottom of the proposed drainfield and an impermeable soil layer be reduced to 4 feet.
9. CLOSING: I am open to any suggestions from your department that would be an
improvement to the proposed system. I am unaware of any negative impacts that this installation
would impose on adjacent wells, or septic systems. If you have any questions, please call us at

SUBJ: Waiver request to reduce the separation of the bottom of absorption field to
impermeable layer
Alaska Water & Wastewater Consultants, Inc. have requested that
the separation distance from the bottom of the proposed absorption field to
impermeable soil be 4 feet instead of the required 6 feet.
This waiver request is approved for the reasons outlined in the
engineer's waiver request letter dated April 9, 2003 and the following
reasons:
$ A water table does not appear in the local area thus even if the
treated effluent from the Advantex system encounters the impermeable
layer prior to full treatment the diagonal movement in the highly silty soils
would certainly obtain full treatment.
$ The closest water well is 200 feet away and probably no other
future water wells will be drilled closer than 100 feet away.
$ The density of houses in the area are on approximately 3/4 acre
lots thus density should not be high considering the tightness of the soil.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines 8 Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of a# wails and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage ofthe family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate ofhow long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any

MAINTENANCE AND REPAIR AGREEMENT

THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AWWTS), described as
located at (legal description).
2, Definitions,
Alteration. Any change to the design or function of an AW WTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC') 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing pails, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed,
Permit. An On -Site Wastewater Disposal Permit as required by AMC
1.5.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AWWTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations, Installation and Removal of Additional Equipment. Prior to
performing any alterations to an AWWTS, the owner agrees to obtain an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AWWTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AWWTS
service and maintenance provider approved by the municipality and the
manufacturer of the AWWTS for the entire term of the AWWTS In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs,
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
H. Owner agrees that the AW WTS installation and maintenance requirements as
provided by the A W WTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AW WTS.
6. Nomvaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment,
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

b. Municipality: Director, Community ➢evelopment or designated authority
C. Any attempt to amend, modify, or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement,
9. Severability. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the

Ref: Conditional COSA for Paradise Valley; Lot 28, Block 4.
We are requesting a conditional COSA for a 3 bedroom house. The conditional items are
summarized as follows:
• The septic tank cannot be pumped at this time do to icy road conditions. It will be pumped in
the spring of 2008.
• There were slope stability issues above the drainfield that were resolved by installing a
gabion basket retaining wall. Fill was added to meet slope requirements. The slopes will be
topsoiled a seeded in the spring of 2008.
