An On -Site Water/Wastewater Permit, number SW030318, issued by this office for a single-family
system, will expire on August 18, 2004 This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $115.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $460.00 for a wastewater permit and $175.00 for a well
permit.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lots 12 and 13, Block 9, Paradise Valley Subdivision intends to construct
a new four-bedroom home on the lot. We are therefore requesting a permit be issued
for the construction of a new well and septic system to serve the home. The well will be
placed on Lot 12 and the house and septic system on Lot 13 The attached Site Plan
and backup documentation identify the location and configuration of the new well,
septic system and alternate site and the parameters used in the design. Also identified
on the plans are the location of the surface drainage, test holes and the wells and
septic systems on the surrounding lots. Drainage arrows and spot elevations are
shown indicating the current drainage patterns. The drainage patterns will be
maintained after construction.
The test holes placed on this lot revealed silty gravel with some sand. The percolation
rate in the upper layers ranged from 3 minutes to 5 minutes per inch. Layers below 5'
were tighter with more silt and percolated at a rate of 45 minutes per inch. No
groundwater was noted in either test hole and none was noted during the monitoring
period. We are proposing to place a 1,500 -gallon Advantex Treatment System with an
add on lift station on the lot. Effluent will be pumped to a 5' wide by 40' absorption bed
with 4' of gravel beneath the distribution pipe. Mounding will be provided over the
trench to provide protection from freezing.
We have designed the trench for an application rate of 1.5 gallons per day per square
foot. A portion of the trench will be constructed in the upper looser material and the
remainder in the tighter soil.
The ground surface on the lot slopes as shown on the site plan from north to south at
slopes ranging from 20% and above. The absorption trench will be constructed parallel
with the slope as much as possible in conformance with Municipal requirements. All
components of the new septic system will be constructed a minimum of 100' from any

surface water and 100' from the well on this and adjacent lots. Some grading may be
required to assure the separation distance is met from surface water.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 13, BLOCK 9, PARADISE VALLEY SUBDIVISION
GENERAL:
The scope of this project includes the procurement and placement of a
new 1,500 Gallon Advantex Treatment System with an AX -20 -Mode
1a 40x94 Pod to serve a new four-bedroom home. System will be
placed at the location shown on the Site Plan. The new septic system
also includes the construction of a new 40' long X 5' wide X 4' effective
depth absorption trench at the location also shown on the Site Plan
and at the configuration shown. The distribution line in the bed will be
placed at 2' below the existing ground surface. The total depth of the
trench will be 6' below the surface. The new system must be placed a
minimum of 100' from the existing well on the lot and those on
adjacent lots and 100' from any surface water located near or on the
lot. Grading may[ be required to divert any surface water and provide
a minimum 100' separation between the surface water and all
components of the septic system.
2. Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. The Advantex Treatment System must be installed in conformance
with specifications and regulations furnished by the manufacturer,
Anchorage Tank and Orenco Systems Incorporated. No variances
from the manufacturer's specifications will be allowed without
permission from the manufacturer and Municipal representatives.
2. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.

4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.
6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the bed shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
6. Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).

3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
SAND GRADATION:
1. Sand used to level the new absorption bed must have the following
gradation:
50% Passed the No. 4 Sieve
2% Passed the No. 100 Sieve
1% Passed the No. 200 Sieve
2. The sand source must be approved prior to use.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as -built of the completed system.

consideration of the mutual covenants contained herein, the parties to this Memorandum of
Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants
permission to Owner to utilize and operate an Advanced Wastewater Treatment System

2. Definitions.
A. Alteration. Any change to the design or ftmction of an AWWTS that
includes the installation or removal of any parts, components or pieces
not included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "A�NIC") 15.65
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC
Patel of 5
15.65. These approvals certify that the systems are adequate for the
homes that they support and meet the codes that were in place at the time
of system construction.

inhibit the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction). An On -Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating). An Advanced Wastewater Treatment Systcm
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all requirements
of this agreement, the conditions of the Operating Permit, the
requiroments.of the On -Site Wastewater System Construction Permit
and all relevant provisions of AMC 15.65.
I Fee. Owner shall pay to Municipality an annual fee of
(S "x.00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.

4. Term. The term of this Memorandum of Understanding shall be for the life of the
AWWTS. The term begins on the date of approval by the Municipality of the installed system
and shall continue while the AWWTS system is in use or operational or until the property is
---sold-ortitlers transferred b"wner-and-a-new-certificate-of-on-site-approval-is-issued-to-the
new owner or transferee of the property.
S. Alterations Installation and Removal of Additional Equipment. Owner agrees not
to make any alterations, removal of parts or additions to the AWWTS without a Construction
Permit from the Municipality.
6. 1llaintenance and Repairs.
k. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner
during the term of this Memorandum of Understanding, and any extensions or renewals
thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3)
adjustment(s), (4) replacement costs, and (5) inspection costs. Further, owner agrees to
comply with all applicable ordinances, laws, regulations, rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the terms and
conditions contained in the Owner's ANVWTS Operating Permit. This schedule shall be
submitted to the Municipality annually upon the renewal of the permit. The schedule of
maintenance and repair contained in the Owner's AWWTS Operating Permit is:

. C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the systems.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
G. Owner ,agrees that the relevant provisions of the standard specification
guidebook for AWWTS is the governing professional guidelines for the construction, .
maintenance and repair of the Owner's AWWTS.
7. Nonwalver. The failure of either party at any time to enforce. a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in
any way affect the validity of this Memorandum of Understanding or any part hereof, or the
right of such party thereafter to enforce each and every provision hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, .modified or
changed by a writing, executed by authorized representatives of the parties, with the same
formality as this Memorandum of Understanding was executed and such writing shall be
attached to this Memorandum of Understanding as an amendment.

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed invalid
by a court of competent jurisdiction shall not invalidate the remaining provisions of the
Memorandum of Understanding.

Dear Mr. Setters:
On March 14, 1983 my company drilled on Lot 13 Block 9 Paradise Valley
Subdivision. The soils log that was made at the time indicates refusals
at 6.5, 8, and 15 feet. The refusals at 6.5 and 8 feet were caused by
large cobbles as indicated on the soil log. The refusal at 15 feet
could have been caused by cobbles or by bedrock. Further drilling would
be required to determine the cause of for refusal at 15 feet.
The Municipality of Anchorage will require your on-site septic system be
placed at a depth no greater than 9 feet if there is bedrock at 15 feet.
I would recommend further exploration with a backhoe when the septic
system is being constructed is a greater depth is desired.
